Abstract
Description
The invention relates to the field of medicine and medical technology, in particular to surgical dentistry, and can be used in the treatment of pathological perapical processes.
А61С 8/02, 1987 г.)Known endodontic-endoaxial implants made in the form of a rod with a thread, and the rod is located in the root canal and the crown of the tooth. (RF Patent No. 2288727, class A61C 8/00, 2004 and German Patent No. 3637061, class A61C 8/02, 1987)
Known implants are used in cases:
- хронического периодонтита с деструктивными процессом в области верхушки корня, а также с резорбцией корня;- chronic periodontitis with a destructive process in the region of the apex of the root, as well as with root resorption;
- радикулярной кисты (резекция или ампутация корня, сопряженная с цистэктомией);- radicular cysts (resection or amputation of the root associated with cystectomy);
- переломы корня зуба, сопровождающиеся необходимостью удаления фрагмента корня;- tooth root fractures, accompanied by the need to remove a root fragment;
- осложнения при эндодонтическом лечении зубов (перфорация, перелом эндодонтического инструмента), требующие резекции и ампутации корня.- complications in endodontic dentistry (perforation, fracture of the endodontic instrument) requiring resection and amputation of the root.
A disadvantage of known endodontic-endooosal implants is the inappropriateness of their use in the above cases of dental diseases and injuries of the root of the tooth in the presence of a fixed orthopedic integumentary design.
The objective of the invention is to maintain a complete non-removable orthopedic integumentary structure that meets functional and aesthetic requirements, with various types of surgical interventions on the root of the tooth.
The technical result is achieved due to the fact that the apex-dental implant is made in the form of a monolithic titanium rod with a cross-sectional diameter of 1.2 mm, having intraosseous and intracanal parts, while the intraosseous part is made with a round profile thread of different heights, and gradually increases in towards the root part of the implant with the length of the depressions between the threads, and the intra-channel part has a circular shape in cross section and is 5 mm in height regardless of the implant's size.
The invention is illustrated by drawings, where figure 1 shows an apex-dental implant, figure 2 - apex-dental implant in the oral cavity.
The apex-dental implant is a monolithic titanium rod with a cross-sectional diameter of 1.2 mm, having an intraosseous (endo-axial)
Depending on the type of surgical intervention, the total length of apex-dental implants can have the following dimensions:
резекция верхушки корня - 9 мм, ампутация корня на 1/3 его длины - 11 мм, высокая ампутация корня на его длины - 13 мм.resection of the apex of the root - 9 mm, amputation of the root by 1/3 of its length - 11 mm, high amputation of the root by its length - 13 mm.
Apex-dental implant is installed as follows.
Depending on the clinical situation, appropriate surgical procedures are performed (resection, amputation or high amputation of the root, removal of the root fragment, etc.). Then, through the intraoperative bone defect, the root canal is prepared with an ultrasonic endodontic tip using curved diamond tips, gradually increasing their diameter. Depending on the group of the tooth, an apex-dental implant of the appropriate size is selected and adapted to the prepared hole in the root canal of the tooth. Then, with the help of a plugger for retrograde filling, the mineral trioxide aggregate ProRoot is introduced into the channel. The implant is inserted into the canal with an intra-root part of 5 mm. The intraosseous part is located freely, touching the lower wall of the intraoperative bone defect. A bone defect is filled to 70% of its own volume with osteoplastic material. The mucoperiosteal flap is mobilized, the wound is sutured tightly.
