Предлагаемое изобретение относится к области медицины, а именно к неврологии и профессиональной патологии, и может быть использовано для дифференциальной диагностики синдрома вегетативной дистонии (СВД) токсического и нетоксического генеза.The present invention relates to medicine, namely to neurology and occupational pathology, and can be used for the differential diagnosis of autonomic dystonia syndrome (SVD) of toxic and non-toxic genesis.
До настоящего времени недостаточно разработаны вопросы дифференциальной диагностики СВД при воздействии токсических веществ и СВД нетоксической природы. Существуют методы дифференциальной диагностики СВД токсической и нетоксической природы, включающие в себя клиническое неврологическое обследование, психологическое исследование эмоционально-волевой сфер, электроэнцефалографию, ЯМРТ головного мозга [2, 3, 7, 9, 11]. Однако эти методики имеют недостаточную точность для дифференциальной диагностики вегетативной дистонии от воздействия комплекса токсических веществ и синдрома вегетативной дистонии нетоксической природы.To date, issues of differential diagnosis of SVD under the influence of toxic substances and SVD of non-toxic nature have not been sufficiently developed. There are methods for differential diagnosis of SVD of toxic and non-toxic nature, including clinical neurological examination, psychological study of the emotional-volitional spheres, electroencephalography, and brain NMR [2, 3, 7, 9, 11]. However, these techniques are not accurate enough for the differential diagnosis of autonomic dystonia from exposure to a complex of toxic substances and autonomic dystonia syndrome of non-toxic nature.
Задачей изобретения является повышение точности дифференциальной диагностики СВД при воздействии токсических веществ и СВД общесоматической природы.The objective of the invention is to improve the accuracy of differential diagnosis of SVD under the influence of toxic substances and SVD somatic nature.
Поставленная задача решается путем проведения пациенту с синдромом вегетативной дистонии теста стандартизированного многофакторного исследования личности (СМИЛ) [10] либо исследования показателей перекисного окисления липидов и антиоксидантной защиты с определением уровней неэстерифицированных жирных кислот (НЭЖК) и активности пероксидазы крови [3, 4] и анализа полученных показателей при проведении теста СМИЛ по формуле:The problem is solved by conducting a standardized multivariate personality study (SMIL) test for a patient with autonomic dystonia syndrome [10] or by studying lipid peroxidation and antioxidant defense indicators with determination of levels of non-esterified fatty acids (NEFA) and blood peroxidase activity [3, 4] and analysis obtained indicators during the SMIL test by the formula:

Рассчитав диагностические коэффициенты F по предлагаемым формулам, полученное значение сравнивают с константой. При значении F> константы диагностируют синдром вегетативной дистонии токсической этиологии, а при значении F< константы - синдром вегетативной дистонии нетоксического генеза.Having calculated the diagnostic coefficients F according to the proposed formulas, the obtained value is compared with a constant. Сравнительный анализ заявляемого решения с прототипом [3] показывает, что предлагаемый способ отличается от прототипа проведением дискриминантного анализа полученных данных психологического и биохимического исследования.A comparative analysis of the proposed solution with the prototype [3] shows that the proposed method differs from the prototype by discriminant analysis of the obtained data of psychological and biochemical studies.
Известно использование дискриминантного уравнения при прогнозировании ишемического инсульта у больных дисциркуляторной энцефалопатией гипертонического генеза, где информативными показателями являются факторы риска [8]. В предлагаемом решении в качестве информативных применяются иные показатели, которые используются для осуществления диагностики, а не прогноза. Способа дифференциальной диагностики СВД токсической и нетоксической этиологии, включающего дифференциальную диагностику токсического СВД по комплексу предлагаемых показателей, в доступной нам литературе не выявлено. Таким образом, предлагаемый способ соответствует критерию «изобретательский уровень».It is known to use the discriminant equation in predicting ischemic stroke in patients with hypertensive genital discirculatory encephalopathy, where risk factors are informative [8]. Thus, the proposed method meets the criterion of "inventive step".
Предлагаемые параметры позволяют дифференцировать СВД при воздействии токсических веществ с высокой степенью точности - 93%.The proposed parameters allow us to differentiate SVD under the influence of toxic substances with a high degree of accuracy - 93%.
Разработанный способ прост в применении, при этом используется минимальное количество предлагаемых показателей.The developed method is easy to use, while using the minimum number of proposed indicators.

Пациенту проводят либо тест стандартизированного многофакторного исследования личности (СМИЛ), либо исследование состояния перекисного окисления липидов и антиоксидантной защиты (определение содержания НЭЖК и активности пероксидазы) и определяют их числовые значения.The patient undergoes either a standardized multivariate personality study test (SMIL) or a study of the state of lipid peroxidation and antioxidant protection (determination of the content of NEFA and peroxidase activity) and determine their numerical values.

Дискриминантный анализ проводился в группах больных с СВД токсического и нетоксического генеза. Пациентами с СВД токсической природы являлись пожарные, подвергшиеся воздействию комплекса химических веществ (винилхлорида, окиси углерода, диоксинов, фуранов и др.) [1]. Достоверность полученных информативных показателей в результате дискриминантного анализа представлена в таблице 1.The discriminant analysis was carried out in groups of patients with SVD of toxic and non-toxic genesis. У больного П. в анамнезе нет острой интоксикации комплексом токсических веществ, но он принимал участие в ликвидации пожара на ОАО "Иркутсккабель". Предлагаемый способ дает возможность дифференцировать СВД токсической этиологии с достаточной точностью и использованием минимального числа наиболее информативных диагностических показателей, способствуя тем самым уменьшению объема параклинических исследований.The proposed method makes it possible to differentiate SVD of toxic etiology with sufficient accuracy and using the minimum number of the most informative diagnostic indicators, thereby contributing to a decrease in the volume of paraclinical studies.
