Abstract
Description
The invention relates to the field of medicine and pharmaceuticals and is suitable for the treatment of tuberculosis with protionamide-based drugs.
The widespread deterioration of the environmental situation and the decline in the social well-being of the population in several countries of the world in recent years have led to an increase in the incidence of tuberculosis, which in some areas is beginning to take on the character of an epidemic.
The main chemotherapeutic agents for treating various forms of tuberculosis are first-line drugs: isoniazid and its derivatives, antibiotics (streptomycin, kanamycin, rifampicin), which have high bacteriostatic activity against tuberculosis mycobacteria (MD Mashkovsky. Medicines. M: 1993 g. ., vol. 2, p. 366). However, with their use, the resistance of tuberculosis mycobacteria develops quite rapidly. In this case, drugs of other chemical groups are prescribed: drugs of the second row (reserve). One of the most active drugs in this group is protionamide, which, although inferior to isoniazid and anti-TB antibiotics in tuberculostatic activity, but acts on mycobacteria resistant to other anti-TB drugs. In addition, it has better tolerance.
In order to prevent the development of drug-resistant mycobacteria in the treatment of tuberculosis, there are two ways: the creation of combined dosage forms (US Pat. Germany No. 3911263, US Pat. No. 4005207) and anti-TB drugs, which include drugs of various groups, or the patient is given a combination of anti-TB drugs different classes.
The disadvantages of combined dosage forms of anti-TB drugs include the need to create large-sized dosage forms, as their preparations are effective only in doses exceeding 80-150 mg. In addition, in the case of the development of resistance of mycobacteria to one of the components, the meaning of the use of such dosage forms is lost.
The second way - a combination of single drugs in the treatment of patients - is simpler and more effective, because It allows the attending physician, changing periodically combinations of anti-TB drugs, to avoid the development of resistant strains of mycobacteria. This is the direction chosen in the treatment of tuberculosis. Therefore, the creation of dosage forms of individual anti-TB drugs is very urgent.
Protionamide is included in the list of essential medicines. It is known (Mashkovsky M.D. Medicines. M., 1993, v. 2 p. 372) that protionamide is released in the form of tablets (dragees) of 0.25 g.
The closest technical solution is the pharmaceutical composition described in US Pat. RF №2166942, which has an anti-tuberculosis effect and consists of a core including protionamide, microcrystalline cellulose, calcium stearate, a cellulose derivative and target additives, and a shell comprising a cellulose derivative, dye and excipient as an active substance. As target additives, potato or corn starch is used, and hydroxypropyl methylcellulose is used as a cellulose derivative. The composition is made in the form of coated tablets. The shell contains hydroxypropyl methylcellulose, tween-80, dye orange. and titanium dioxide.
The disadvantage of this composition is the unsatisfactory (less than 75% for 45 min) release of the active substance and the low tablet strength in the manufacture.
The aim of this invention is to obtain a composition that meets the requirements of the current State Pharmacopoeia of the XI edition, which has increased stability during storage and quickly releases the active substance in the gastrointestinal tract.
This goal is achieved by using a pharmaceutical composition that has an anti-tuberculosis effect and consists of a core comprising protionamide, microcrystalline cellulose, calcium stearate, a cellulose derivative, target additives, and a shell comprising a cellulose derivative, dye and excipient as an active ingredient. Hydroxypropyl cellulose is used as a cellulose derivative, calcium hydrophosphate dihydrate and sodium croscarmellose are used as target additives, a copolymer of vinyl pyrrolidone with vinyl acetate is used as an auxiliary substance in the shell in a ratio of 6: 4, in the following ratio of ingredients,

The proposed composition and selection of the ratio of ingredients found experimentally and is optimal, providing the necessary strength and disintegration in water. The pharmaceutical composition is in the form of a hard coated tablet. The presence of the shell of the claimed composition gives the stability of the composition during storage.
154.The pharmaceutical composition in the form of a tablet is obtained by the following technology: the sieved powders of protionamide, calcium phosphate and microcrystalline cellulose are loaded into the mixer, mixed thoroughly, moistened with a hydroxypropyl cellulose solution, mixed until the moisture is evenly distributed and granulated. The granules are dried, ground, dusted with a mixture of calcium stearate and croscarmellose sodium and tableted. The core tablets are coated with a shell prepared on the basis of hydroxypropyl cellulose and a vinylpyrrolidone-vinyl acetate copolymer at a ratio of 6: 4 with the addition of a pigment suspension of talc, titanium dioxide and yellow iron oxide. In appearance, the tablets satisfy the requirements of the Global Fund XI, issue 2, p. 154.
Studies were conducted on the compatibility of protionamide with hydroxypropyl cellulose and a copolymer of vinylpyrrolidone with vinyl acetate at 6: 4. As a result of the study, a positive result was obtained: they achieved good physical and chemical stability of the product both in the production process and during storage. It was unexpectedly found that when refusing certain fillers, often used as auxiliary materials, such as starch, the stability problem was solved.
2.The results of the experiment are summarized in tables 1 and 2.
