Однако при осуществлении этого способа дистракцию тканей начинают не ранее чем через 20-25 дней после соединения концов нерва, что требует проведения за 4-6 суток до указанного срока повторного нарушения целостности кости. После анестезии и обработки операционного поля известными приемами производят остеосинтез пораженной конечности с фиксацией - в зависимости от клинической ситуации - одного или одновременно двух смежных сегментов компрессионно-дистракционным аппаратом. Осуществляют ревизию зоны поврежденного нервного ствола и при наличии показаний выполняют нейроррафию. При анатомическом перерыва нерва не более чем на 1,5-2,0 см мобилизуют его проксимальный и дистальный отрезки. После этого на уровне предполагаемых "освежающих" срезов обоих концов нерва ультразвуковым микрохирургическим скальпелем выполняют циркулярные коагуляционные разрезы наружного эпиневрия, что служит профилактической мерой для предупреждения кровотечения из концов нерва, снижает травматичность, уменьшает количество и продолжительность хирургических манипуляций. По линии этих разрезов пересекают пучки нервных волокон острым лезвием или зубчатыми микроножницами, а затем через наружный и внутренний эпиневрий концов нерва между самыми крупными пучками проводят трансневральный провизорный шов нитью 7/0-8/0 на атравматической игле и, затягивая его, максимально сближают концы нерва, следя за тем, чтобы лигатура не прорезалась. Исходя из величины остающегося диастаза выполняют резекцию соответствующего участка кости, остающиеся костные фрагменты сближают и фиксируют в положении плотного контакта. Вслед за этим с использованием оптического увеличения путем затягивания трансневрального шва производят окончательное сближение концов нерва до контакта торцов пучков нервных волокон и появления между ними капиллярного слоя жидкости, обеспечивающего их плазматическое "склеивание". При достижении последнего, что в условиях хорошего гемостаза и ретракции плазменного сгустка (количество тромбоцитов в 1 мл периферической крови не менее 170 тыс., количество эритроцитов не более 4,9 млн, фибриноген не более 270) наступает через 10-40 секунд, на нерв накладывают эпипериневральные и эпиневральные дефинитивные швы. После сближения костных фрагментов в спице-стержневом аппарате нитью 7/0 выполнен трансневральный провизорный шов. Культи нерва сближены, торцы пучков нервных волокон приведены в положение контакта. Отмечено их отчетливое плазматическое склеивание.
