Field of technology to which the utility model relates
The utility model relates to the field of medicine, namely to otorhinolaryngology, and is used to create a permanent hole in the eardrum in the treatment of diseases of the middle ear and eustachian tube.
State of the art
In practice, to provide a long-lasting hole in the eardrum, tympanic shunts (tympanostomy tubes) made of various biocompatible materials (titanium, silicone, fluoroplastic) are used, which are installed into the incision of the eardrum. However, the procedure for installing/removing a shunt can be significant in conditions of complex geometry of the ear canal, especially with some anatomical features of the ear canal (for example, narrowing). Typically, the shunt is installed using ear pliers, with which the shunt is fixed to the flange. In this case, the jaws of the forceps must be thin enough to fit into the shunt hole. In addition, when installing/removing the shunt, there is a risk of it falling into the tympanic cavity. In some cases, the eardrum shunt falls out on its own. However, more often, after a certain time, removal of the shunt is required, which can also present technical difficulties, for example, with an anatomically narrow ear canal. In addition, in children, the shunt removal procedure often requires general anesthesia.
There are various modifications of tympanostomy tubes.
A known tympanostomy tube, Tuebingen model with wire (Heinz Kurz GmbH Medizintechnik, Germany: https://gelios-medical.ru/2016/11/tubingen-2/). Tympanostomy tubes of this type are equipped with a wire that makes it easier to remove the shunt if it falls into the tympanic cavity. The disadvantage of this model is the inconvenience of installing the shunt (the wire “clings” to the walls of the ear canal). Also, when the shunt is left in place for a long time, sulfur masses can accumulate in the area of the wire.
The closest in technical essence is the Touma Moretz ventilation tube, (Medtronic Inc., USA: http://emp77.ru/upload/products_f/f_176_260.pdf), on the opposite ends of which flanges are made, and on one of them at an angle to The flange plane has an eyelet. The disadvantage of this device is that there is only one way to remove the shunt, using ear pliers, since there is no hole in the ears of this type of shunt.
The technical problem of expanding the arsenal of technical means for creating a stable hole in the eardrum is solved by creating a tympanic shunt, which provides the medically necessary duration of manipulation and reduces the likelihood of using anesthesia.
Disclosure of utility model
The technical result is to increase the efficiency of the tympanic shunt due to the possibility of medical manipulations of varying durations and reducing or eliminating the use of anesthesia.
The technical result is achieved due to the fact that the tympanic shunt consists of a ventilation tube, on the opposite ends of which there are flanges, and on one of them there is an eyelet at an angle to the plane of the flange, and the eyelet is equipped with a hole. Moreover, the hole in the ear has the shape of a circle segment.
The shunt can be made of titanium or other materials (silicone, fluoroplastic, titanium (ASTM F67 Medical Grade)) used for the manufacture of tympanostomy tubes.

Implementation of a utility model
Installation of the shunt is carried out by inserting the edge of the inner flange 3 into the incision of the eardrum with subsequent pressure on the shunt, as a result of which the inner flange is “tucked” into the incision, similar to inserting a button into a slotted loop on clothing.
A feature of removing the claimed shunt is the possibility of using various instruments. So, the shunt can be removed in two ways:
1. By fixing the shunt behind the ear with 1 ear pliers and then removing it.
2. Using a microhook or a curved needle inserted into hole 5 located in the area of ear 1, followed by traction of the shunt in the lateral direction.
Thus, the claimed tympanic shunt provides an expansion of the arsenal of methods for shunt removal and reduces the duration of the removal procedure, which in some cases makes it possible to avoid the use of anesthesia.
