Abstract
Description
The invention relates to medicine, namely to traumatology and orthopedics, and is intended for fixation of bone fragments of the condyles.
Springer-Veriag. - 1996. - с.200-290). There is a method for osteosynthesis of condyles using plates which are indicated for fixing large and medium fragments. (M.E. Muller et al. Guide to Internal Osteosynthesis. Springer-Veriag. - 1996. - pp. 200-290). In the presence of small fragments, the dimensions of which are smaller or slightly larger than the diameter of the screws for the plate, their fixation in this way can lead to the breaking of fragments.
Springer-Veriag. - 1996. - с.179-194). There is a combined method for fixing large and medium condyle fragments with a plate and small fragments — separately with screws, the diameter of which corresponds to the size of the fragments and does not lead to their splitting (M.E. Muller et al. Manual for internal osteosynthesis. Springer-Veriag. - 1996. - p. .179-194). The disadvantage of this method, which we adopted as a prototype, is that it is not possible with a separate screw, while maintaining optimal interfragment efforts, to create stable fixation without the risk of splitting small fragments from twisting and bending loads. In addition, fixing separately with a plate and screws does not create a single fixing system for the plate - a screw.
Such disadvantages are deprived of the proposed method, which consists in fixing large and medium fragments with the main plate and small ones using brackets bent at different angles, which are connected as a whole with the plate using screws.
The aim of the proposed method is to create stable fixation of fragments and accelerate the recovery of joint function.
To achieve this goal, osteosynthesis of the main fragments (large and medium sizes) is carried out using a plate (straight, curly or otherwise) and fixing screws (or screws). Then, small fragments are fixed with a bracket bent at an angle of 90 ° (the angle can be changed arbitrarily), moreover, the bracket is fixed at one end to the fragment and the other to the main plate.
The proposed method in comparison with the prototype allows you to fix all the fragments (large, medium and small) with a single fixing system plate - staples, which creates a stable fixation and allows you to start early movements in the joint. In addition, with screws that fix small fragments, it is possible to create optimal inter-fragment compression without the risk of cracking them.
Так как скобы имеют разную величину граней, то адаптировать их к пластине удается легко (фиг.1).The method is as follows: in the presence of comminuted fracture of the condyle of the bones, large and medium fragments are first of all compared, which are fixed by a plate or several plates with screws or screws. Then, small fragments are compared, fixed with a bracket in the form of a curved plate (the angle between the ends of the bracket can be changed arbitrarily) with a screw so that the hole of the bracket coincides with the hole in the plate. For this purpose, from one of the plate holes closest to the small fragment, the screw or screw and the clamp are pre-screwed in an overlap, fixed through this hole to the plate. Since the brackets have different sizes of faces, it is easy to adapt them to the plate (Fig. 1).
(фиг.2а и б).The thickness of the main plate is different depending on the size of the fragments and the location of the fracture. So in case of fracture of the condyle of the shoulder, its thickness is 2 mm, and the thickness of the bracket is 1 mm. On the main plate there are holes countered for the heads of screws with a diameter of 4.3 mm, and on the bracket one of the holes for a similar screw and the other for a screw with a diameter of 2.5 to 3.5 mm. (figa and b).
Clinical example.
Patient V., 45 years old, was admitted to the department of the institute with a diagnosis of closed multifragmental fracture of the condyle of the left shoulder of the SZ type (according to AO classification). The patient underwent surgery according to the method described above. Checked fixation on the operating table, which was stable. After 7 days, the gypsum splint was removed and active and passive movements in the elbow joint were started. 6 weeks after surgery, the fracture fused and joint function by this time was 80% of normal.
