Изобретение относится к области медицины, а именно к выявлению предрасположенности к сахарному диабету, его скрытого течения, а также других нарушений углеводного обмена. The invention relates to medicine, namely to identify a predisposition to diabetes, its latent course, as well as other disorders of carbohydrate metabolism.
Известно, что нарушение способности тканей использовать глюкозу лежит в основе ряда заболеваний, в частности сахарного диабета. О способности тканей утилизировать глюкозу судят по косвенным показателям: по разнице содержания глюкозы в крови, притекающей к органу и оттекающей от него, по скорости снижения гликемии после углеводной нагрузки и после введения инсулина, по количеству глюкозы в моче с углеводной нагрузкой и без нее. It is known that a violation of the ability of tissues to use glucose is the basis of a number of diseases, in particular diabetes mellitus. The ability of tissues to utilize glucose is judged by indirect indicators: by the difference in glucose in the blood flowing to and from the organ, by the rate of decrease in glycemia after carbohydrate loading and after administration of insulin, by the amount of glucose in urine with and without carbohydrate load.
Указанные способы трудоемки, их сложно адаптировать для массовых обследований. Способов непосредственного измерения утилизации глюкозы тканями, пригодных для широкого внедрения в медицинскую практику, авторами не обнаружено. These methods are time-consuming, they are difficult to adapt for mass screening. The authors have not found any methods for directly measuring glucose utilization by tissues suitable for widespread use in medical practice.
Аналогом способа авторы предлагают экспериментальные исследования академика А.М.Уголева ("Физиология и патология пристеночного (контактного) пищеварения". Л. : Наука, 1973, стр.18-36), в которых изучали скорость перехода различных пищевых веществ, в том числе глюкозы, из растворов этих веществ, пропускаемых через отрезок кишки. Указанные исследования позволили конкретизировать механизмы усвоения отдельных пищевых веществ кишечной стенкой, однако они не пригодны для внедрения в медицинскую практику, поскольку слишком трудоемки в исполнении и требуют экстирпации исследуемых тканей. The authors of this method offer experimental studies of academician A.M. Ugolev ("Physiology and pathology of parietal (contact) digestion". L.: Nauka, 1973, pp. 18-36), in which the transition rate of various food substances was studied, including glucose, from solutions of these substances passed through a piece of intestine. These studies made it possible to specify the mechanisms of assimilation of certain food substances by the intestinal wall, however, they are not suitable for implementation in medical practice, since they are too laborious to perform and require extirpation of the studied tissues.
В качестве прототипа авторами использован способ определения утилизации глюкозы клетками крови, помещенными в искусственно созданную среду, содержащую определенное количество глюкозы. О скорости утилизации авторы (В.Г.Куницын, М.Ф.Некрасов, В.И.Феденков. "Определение скорости утилизации глюкозы во взвеси эритроцитов". Лабораторное дело, 1990, N 9, стр.27-29) судили по скорости убыли глюкозы из инкубационной среды в течение определенного срока инкубации. Указанное исследование констатирует возможность непосредственного изучения способности клеток утилизировать глюкозу из искусственно созданного "депо", т.е. из среды инкубации эритроцитов. The authors suggest a method for determining the utilization of glucose by tissues in the gingival fold of the oral cavity. Предлагаемый авторами способ состоит из трех этапов. The method proposed by the authors consists of three stages.
1. Подготовка тест-носителей глюкозы (ТНГ). На листочки фильтровальной бумаги 1,0•1,5 см, приклеенные водостойким клеем к Г-образным держалкам и простерилизованные сухим жаром при 120-140oС, наносится по 0,02 мл 8% раствора глюкозы (1,6 мг сухого вещества). Количественные характеристики наносимого на ТНГ раствора глюкозы авторами определены опытным путем. После высыхания на воздухе ТНГ заворачивают небольшими партиями в целлофан с указанием на рукоятке держалки номера и серии ТНГ. Подготовленные таким образом ТНГ можно хранить неопределенно длительное время в сухих, желательно герметичных, пеналах, удобных для транспортировки.1. Preparation of glucose test carriers (TNG). 2. Экспозиция ТНГ в верхней губо-десневой складке. После 4-кратной "очистки" складки языком (два движения по часовой стрелке и два против) ТНГ вставляют на 10 минут в складку до упора. Пациента необходимо предупредить, что нельзя допускать даже небольшою смещения ТНГ, так как это значительно влияет на конечный результат тестирования. Ровно через 10 минут ТНГ извлекают, бумажную часть пинцетом отделяют от держалки и помещают ее в пробирку, содержащую 1,0 мл дистиллированной воды. Перевод глюкозы из бумажной части в раствор продолжается 10 минут при постоянном встряхивании пробирки, после чего бумажную часть выбрасывают, а полученный раствор глюкозы готов для следующего этапа, т. е. для определения в нем глюкозы. При необходимости (например, при массовом тестировании) ТНГ после экспозиции можно высушить на воздухе и исследовать даже спустя несколько суток. 2. Exposure of TNG in the upper lip-gingival fold. The transfer of glucose from the paper part to the solution lasts 10 minutes with constant shaking of the tube, after which the paper part is discarded, and the resulting glucose solution is ready for the next step, i.e., to determine glucose in it. If necessary (for example, during mass testing), TNG after exposure can be dried in air and examined even after a few days.
3. Определение содержания глюкозы в ТНГ после перевода ее из бумажной части в воду (непосредственно после экспозиции или спустя некоторое время) можно производить любым методом изучения концентрации глюкозы в водных растворах. Например, при необходимости быстрого тестирования авторы использовали портативный гпюкометр "Уан тач бэйсик" компании "Джонсон и Джонсон". (Сертифицировано МЗ и МП РФ, регистрационное удостоверение N 95/160). О степени утилизации глюкозы тканями судили по разнице содержания ее в ТНГ после экспозиции в сравнении с неэкспонированными. Эту разницу выражали в виде коэффициента поглощения глюкозы (КПГ), представляющего собою процент убыли глюкозы из ТНГ. 3. This difference was expressed as glucose absorption coefficient (GCG), which is the percentage loss of glucose from TNG.
С целью оценки возможностей способа было проведено тестирование двух групп людей. In order to assess the capabilities of the method, two groups of people were tested.
1. Девять практически здоровых молодых людей (студентов) в возрасте 19-25 лет, из них 3 мужчин и 6 женщин (контрольная группа). 1. Nine healthy young people (students) aged 19-25 years old, including 3 men and 6 women (control group).
2. Одиннадцать женщин в возрасте 40-70 лет, поступавших в разное время в эндокринологическое отделение городской больницы 6 г. Твери на лечение инсулиннезависимого сахарного диабета. В контрольной группе величина КПГ колебалась от 12,8 до 65,8. Указанные крайние значения КПГ были отброшены, а, исходя из остальных семи наблюдений, была получена предварительная "норма", равная 45,5±02,3. В группе больных ИНСД величина КПГ колебалась от 3,8 до 47,4, среднее значение составило 24,2±05,6. Из приведенных данных следует, что степень (скорость) утилизации глюкозы тканями в обеих группах значительно варьировала, однако общая направленность изменений достоверно (Р<0,05) указывает на ослабление утилизации глюкозы тканями больных ИНСД. 2. The authors believe that their proposed method speeds up, reduces the cost and simplifies the procedure for diagnosing diabetes mellitus and carbohydrate metabolism disorders predisposing to the development of this disease, since it eliminates the need for taking and processing blood, urine, stress tests, and allows conducting mass examinations in order to organize preventive work , and also facilitates the operational control of the effectiveness of treatment.

Больная М. , 59 лет, обратилась в поликлинику с жалобами на боли в эпигастральной области и диспептические явления. Диагностировано обострение язвенной болезни желудка. В анамнезе у отца пациентки сахарный диабет. При проведении тестирования предлагаемым способом отмечено уменьшение КПГ на 57,6% в сравнении с указанной выше временной "нормой", что расценивается как указание на возможность регистрации предлагаемым способом наследственных особенностей углеводного обмена, снижающих устойчивость тканей к повреждающим факторам без манифестации диабетических признаков.Clinical example N 1

Твери по поводу инсулиннезависимого сахарного диабета с 24.09.01 по 12.10.01 (история болезни N 5784). При почасовом определении гликемии на фоне приема антидиабетических пищевых добавок проводилось параллельное тестирование предлагаемым способом.
