Изобретение относится к медицине, а именно к хирургии, т.е. к способам замещения раневых дефектов кожи. The invention relates to medicine, namely to surgery, i.e. to methods for replacing wound skin defects.
Значение дерматопластики неоспоримо велико и обусловлено возможностью закрытия ран с благоприятным воздействием на их заживление. Кожная пластика позволяет значительно сократить сроки регенерации ран, уменьшить выраженность или полностью избежать образования грубого рубца с обеспечением функциональной и косметической реабилитации больных. Определение показаний для той или иной разновидности кожной пластики диктуется конкретными условиями: площадью, конфигурацией и глубиной раневого дефекта, наличием загрязненности, инфицированности или нагноения раны, фазой раневого процесса, эластичностью окружающих рану кожных покровов, близостью важных анатомических структур и т.д. При выборе показаний к операции из всего многообразия видов кожной пластики более оптимальным считается тот, который в наиболее короткий срок наиболее простым путем даст полноценное закрытие раны (Н.Н. Блохин, 1955; Г.А. Даниелян, 1957; В.С. Антипенко, 1962, 1975; В.Г. Вайнштейн, 1969; М. В. Громов, 1975; М.И. Костюченок, 1981; M. lselin, 1962; H. Cosse, 1964; M.L. Lewin, 1969; Ch. Radovas, 1976 и др.). The value of dermatoplasty is undeniably large and is due to the possibility of closing wounds with a beneficial effect on their healing. Skin plastic surgery can significantly reduce the time of wound regeneration, reduce the severity or completely avoid the formation of a rough scar with the provision of functional and cosmetic rehabilitation of patients. The determination of indications for a particular type of skin grafting is dictated by specific conditions: the area, configuration and depth of the wound defect, the presence of contamination, infection or suppuration of the wound, the phase of the wound process, the elasticity of the skin surrounding the wound, the proximity of important anatomical structures, etc. When choosing indications for surgery from the whole variety of types of skin grafting, the most optimal is the one that in the shortest possible time will give the most complete closure of the wound (NN Blokhin, 1955; G.A. Danielyan, 1957; V.S. Antipenko , 1962, 1975; V.G. Weinstein, 1969; M.V. Gromov, 1975; M.I. Kostyuchenok, 1981; M. lselin, 1962; H. Cosse, 1964; ML Lewin, 1969; Ch. Radovas, 1976 and others).

Несвободная кожная пластика предпочтительнее свободной в силу более оптимальных биологических условий заживления (адекватная васкуляризация лоскута, более полное соответствие его реципиентной области и т.д.). Однако и она имеет существенные недостатки, к которым относится возможность замещения лишь сравнительно небольших дефектов кожи, необходимость мобилизации краев раны и выполнения дополнительных разрезов. Сh. Radovas (1976) предложил очень перспективный и эффективный метод постепенного возмещения дефекта кожи путем создания ее запаса имплантацией и раздуванием физиологическим раствором сцликонового баллончика, а уже из созданного запаса кожи выкраивать нужный для пластики лоскут. Этот метод нашел широкое применение в частности в пластическом замещении дефектов кожи волосистой части головы, однако и он не лишен недостатков: нередкое обнажение из-под мягких тканей силикрнового баллончика и частые трофические расстройства в месте его имплантации, а также необходимость последующих сложных расчетов по выкраиванию фигурных лоскутов. This method has found wide application, in particular in the plastic replacement of skin defects of the scalp, but it is not without drawbacks: frequent exposure of soft silicone canisters and frequent trophic disorders at the site of implantation, as well as the need for subsequent complex calculations to cut out the curly flaps.
В качестве одного из аналогов можно привести метод кожной пластики местными тканями, описанный З.Ф. Нельзиной (1980). Он подкупает своей простотой и сводится к одномоментному закрытию раневого дефекта путем мобилизации краев кожи вокруг дефекта, а если этого оказывается недостаточно - к дополнительным разрезам и перемещению кожи из соседних участков: встречными треугольными лоскутами по Лимбергу (1929, 1935), ротационным методом кожной пластики и др. Осуществляется метод кожной пластики следующим образом. После ПХО раны производится мобилизация ее краев, а также послабляющие разрезы, дающие возможность закрыть дефект, причем для большей мобилизации кожи последнюю нередко отделяют от фасции. Поставленная цель реализуется тем, что, не дожидаясь полного купирования гнойного процесса в ране (при наличии последнего), осуществляется дробное нанизывание краев раны на две спицы Киршнера и "шнуровка" раны в допустимом натяжении, по спадении которого тем же путем производится дальнейшее стягивание раны вплоть до полного ее закрытия. Техническое осуществление предлагаемого изобретения заключается в следующем. Под местной анестезией вдоль обоих краев раны, отступя от них 0,7-1,5 см, проводят пару спиц Киршнера таким образом, чтобы они проходили под- и надкожно, т.е. чтобы кожа по краям раны была дробно на них нанизана (фиг. 1). При этом имеет существенное значение соотношение длины под- и надкожных отрезков спиц. Так, если под кожу погружена большая часть спиц, то нагрузка на единицу длины краев раны при последующем их стягивании распределяется более равномерно. Однако при этом соотношении возрастает вероятность трофических расстройств краев раны из-за передавливания их сосудов. Поэтому при хорошем кровоснабжении и небольшой толщине и плотности кожных покровов целесообразно придерживаться соотношения 1:1. Затем концы спиц скусывают и загибают, а под выступающие отрезки обеих спиц проводится любая монофиламентная нить (лучше лавсановая или тонкая леска), перекидывающаяся с одной стороны на другую и обратно, т.е. производится "шнуровка" раны (фиг.2). Натяжением за оба конца нити осуществляют сближение спиц Киршнера и стягивание краев раны с уменьшением площади раневого дефекта. Во избежание трофических расстройств и прорезывания кожи на спицах это натяжение не должно быть избыточным. Концы нити связывают между собой провизорным узлом. Через некоторое время натяжение кожи ослабевает из-за ее эластических свойств, а также действия эффекта Илизарова (регенерация и увеличение объема тканей напряжением растяжения). Поэтому через каждые 2-3 суток производят "перешнуровку" раны с возобновлением натяжения (фиг.3). Таким образом производится экстензионное закрытие дефекта кожи до полного соприкосновения краев раны. При достижении последнего, не расслабляя натяжения нити, накладывают вторичные швы на рану (фиг. 4), а "расшнуровку" осуществляют постепенно по мере заживления раны и укрепления рубца. Therefore, with good blood supply and a small thickness and density of the skin, it is advisable to adhere to a ratio of 1: 1. Then the ends of the needles bite and bend, and under the protruding segments of both knitting needles, any monofilament thread (preferably dacron or thin fishing line) is carried out, spreading from one side to the other and vice versa, i.e. produced "lacing" of the wound (figure 2). By pulling at both ends of the thread, the Kirschner spokes are brought together and the edges of the wound are pulled together with a decrease in the area of the wound defect. In order to avoid trophic disorders and teething on the knitting needles, this tension should not be excessive. The ends of the thread are connected together by a pharmacist unit. After some time, the skin tension weakens due to its elastic properties, as well as the action of the Ilizarov effect (regeneration and increase in tissue volume by tensile stress). Therefore, every 2-3 days produce “lacing” of the wound with the resumption of tension (figure 3). Thus, the extensional closure of the skin defect is made until the wound edges are in full contact. When the latter is reached, without relaxing the thread tension, secondary sutures are applied to the wound (Fig. 4), and the “lining” is carried out gradually as the wound heals and the scar is strengthened.
Преимуществами предлагаемого способа кожной пластики являются следующие:
1) возможность устранения достаточно крупных дефектов кожи без необходимости в дополнительных разрезах и свободных трансплантатов;
2) отсутствие необходимости мобилизации краев раны, при которой уменьшается их васкуляризация из-за нарушения связи с подлежащими тканями, а также выкраивания сложных фигурных лоскутов для закрытия дефекта;
3) возможность использования на инфицированных и гнойных ранах, не дожидаясь купирования воспаления и завершения фазы санации раневого процесса.The advantages of the proposed method of skin plastics are the following:

Больной X. 39 лет поступил в Республиканский ортопедо-травматологический центр (РОТЦ) 15.03.99 с диагнозом: огнестрельное сквозное пулевое ранение верхней трети правого бедра с оскольчатым переломом бедренной кости и дефектом кожи задненаружной поверхности поврежденного сегмента. Patient X. 39 years old was admitted to the Republican Orthopedic and Traumatology Center (RTC) on March 15, 1999 with a diagnosis of a gunshot through bullet wound of the upper third of the right thigh with comminuted fracture of the femur and a defect in the skin of the posterior surface of the damaged segment.

В РОТЦ при поступлении налажено скелетное вытяжение и проточное дренирование растворами антисептиков. 18.03.99 трубчатый дренаж удален и под местной инфильтрационной анестезией по краям раны по задненаружной поверхности верхней трети бедра (размер раны составляет 5,5 на 6,0 см) в продольном направлении под- и надкожно проведена пара спиц Киршнера. При этом соотношение под- и надкожных отрезков спиц было равным. Концы спиц скусаны и загнуты, а под надкожные отрезки спиц проведены концы стерильной тонкой рыболовной лески. Рана "зашнурована" и умеренно стянута с уменьшением дефекта на половину (3,0 на 6,0 см). Расстройств васкуляризации краев раны при этом не отмечалось. На рану передневнутренней поверхности (входное отверстие сквозной раны) под местной анестезией наложены вторичные швы. Наложены асептические повязки на обе раны. Затем каждые 3 суток во время перевязок осуществлялась "перешнуровка" задненаружной раны до полного закрытия дефекта кожи. 26.03.99 под местной анестезией на рану наложены вторичные швы. Рана зажила без осложнений, а полная "расшнуровка" с удалением обеих спиц Киршнера произведена 02.04.99. Скелетное вытяжение снято 12.05.99 при наличии адекватного "первичного охвата" зоны перелома костной мозолью хорошей плотности, и в кокситной гипсовой повязке больной выписан на амбулаторное лечение. На месте заживших огнестрельных ран имеется тонкий и неспаянный с подлежащими тканями рубец. Результат лечения расценен как хороший. Skeletal traction was removed on 05/12/99 in the presence of an adequate "primary coverage" of the fracture zone of bone marrow of good density, and the patient was discharged for outpatient treatment in a coxite plaster cast. In place of the healed gunshot wounds, there is a thin and undesired scar with the underlying tissues. The result of the treatment is regarded as good.
