Известны способы прямой реваскуляризации миокарда при ИБС. Наиболее эффективными методами при этом являются аортокоронарное шунтирование с помощью аутовены, маммарно-коронарный анастомоз, а также использование верхней брыжеечной или других артерий для артериализации окклюзированных коронарных артерий. Однако эти методы имеют отрицательные стороны, приводящие к тромбозу шунтов, такие как повреждение стенки трансплантата при заборе и обработке, чрезмерная длина трансплантата и перегиб шунта, перекрут шунта по оси, натяжение шунта из-за его недостаточной длины, аномалия клапанного аппарата венозного трансплантата. Вместе с тем, методы прямой реваскуляриоации миокарда неэффективны при выраженном стенозирующем атеросклерозе коронарных артерий на всем протяжении, либо дистальном характере поражения сосудов, а также при патологии восходящего отдела аорты ("no-touch aorta"). Известен способ артериализации коронарного синуса, получивший впоследствии имя Бек-2 (Сердечно-сосудистая хирургия / Под ред. В.И. Бураковского, Л.А. Бокерия. М.: Медицина, 1996, с. 534). Операция выполнялась в два этапа, при первом накладывался аутовенозный шунт или прямой анастомоз между аортой и коронарным синусом, а при втором производилось сужение коронарного синуса. Операция была вскоре оставлена, так как она давала плохие результаты и сопровождалась высокой летальностью по причине, главным образом, венозного застоя сердца. Предлагаемый способ заключается в наложении артериовенозного свища на максимально проксимальном уровне артерии и соответственно дистальном уровне вены, с последующей перевязкой дистального (оттекающего) отрезка вены сердца. Наиболее удобовыполнимый метод анастомоза - "бок-в-бок", после мобилизации сосудов. Уровень наложения свища должен быть проксимальнее окклюзированных участков коронарных артерий. Перевязка дистального отрезка вены не должна нарушать общего венозного оттока крови сердца. Выполняют срединную стернотомию. Перикард вскрывают продольно. Раздельную канюляцию полых вен и восходящей аорты выполняют по обычной методике. Начало ИК с гипотермией 28-30oС. При фибрилляции сердца пережимание аорты и введение кардиоплегического раствора в корень аорты (антеградно) и далее ретроградно через правое предсердие. Находят на проксимальном уровне огибающую ветвь ЛКА и выделяют ее на протяжении 1-1,5 см. На этом же уровне и параллельно выделенной артерии, выделяют Vena posterior ventriculi sinistri. Накладывают артериовенозный свищ диаметром не менее 5 мм "бок-в-бок" на масимально проксимальном уровне артерии (соответственно дистальном уровне вены). Затем перевязывают лигатурой дистальный (оттекающий) участок вены до ее впадения в коронарный синус. Проводят согревание больного и профилактику воздушной эмболии. Восстанавливают сердечную деятельность разрядом ЭДФ. Конец ИК. Деканюлизация. Гемостаз. Дренажи перикарда и переднего средостения. Послойное ушивание раны грудной клетки.Perform median sternotomy.
