Abstract

FIELD: medicine.  The invention relates to medicine, in particular to gynecology.

 The problems of obstetrics and gynecology, for many reasons, are of interest not only to physicians. It is no secret that at present, with low birth rates, we have a high mortality rate, including child and maternal. Negative growth is noted in all regions of Russia. One way to solve the problem is primarily associated with the prevention of gynecological diseases and the treatment of infertility. Thus, high-quality treatment of gynecological diseases is not only a medical, but also a major social problem.

 The main part of gynecological diseases includes hormone-dependent and inflammatory diseases. Although a wide range of remedies is offered for their treatment, the search continues for new, more effective remedies and methods of treatment.

 A serious problem in this area today are, in particular, endometriosis and uterine fibroids. About thirty years ago, a disease such as endometriosis was extremely rare. Today, half of menstruating women are affected by endometriosis (Medicine for All, 1 (12), 1999 - Women's Health Formula).

 Genital endometriosis is responsible for every third case of female infertility.

 The rapid increase in the frequency of its distribution in recent years makes even more active and intensive work on the search and elimination of causes.

Endometriosis refers to benign tumor formations and arises as a result of atypical germination and functioning of endometrial cells - the internal mucous membrane of the uterus, in which two layers are distinguished: basal and functional. According to the depth of penetration into the myometrium, there are four degrees of diffuse distribution of the uterine endometriosis:
I degree - the germination of endometriosis to a shallow depth;
II degree - the spread of the process to the middle of the myometrium;
III degree - involvement in the pathological process of the entire uterine wall;
IV degree - involvement in the process of parietal peritoneum and neighboring organs.

 With the II-IV degree of spread of endometriosis, muscle fiber hyperplasia is observed, which contributes to an increase in the uterus (its size depends on the number and size of the cystic cavities of the endometriotic tissue and the severity of muscle element hyperplasia). Often there is endometrial hyperplasia, as well as a combination of uterine endometriosis with myoma.

 The main treatment method is surgical. In some cases, drugs are widely used that allow bloodless to "remove" or cause atrophy (disappearance) of foci of endometriosis in any organ. In particular, hormonal agents are used, against which the release of ovarian hormones is inhibited, which stimulate monthly cyclic growths and rejection of the endometrium and, accordingly, endometriotic formations. In recent years, various drugs have been sought and applied that block the growth of endometrioid heterotopy cells. Along with the positive, they often had a negative effect.

 The most modern method of conservative treatment of endometriosis is the use of gonadotropin-releasing hormone agonists (a-GnRH), whose activity is 100 or more times higher than endogenous GnRH.

 Uterine fibroids are one of the most common benign tumors developing in the muscular membrane of the uterus (myometrium). Uterine fibroids come from smooth muscle tissue. Mostly it consists of elements of muscle tissue (80%); a small part is connective tissue (15-20%). The "core" of the tumor is formed from smooth muscle elements, with further development acquires a mixed growth pattern, forming a fibroid. Uterine fibroids are characterized by a wide variety of clinical course.

 Currently, the main methods of treating fibroids are radical surgical (removal of the affected organ), minimally invasive surgical, conservative treatment, as well as the use of medications in preparation for surgery.

 The basic principle of drug therapy for uterine fibroids is the suppression of ovarian secretion of estradiol by hormonal agents.

 Conservative hormonal therapy for uterine fibroids is aimed at preventing the growth of the tumor, reducing its size, as well as stopping the symptoms of the disease. Complete tumor regression, as a rule, is not achieved. To date, of the whole variety of hormonal drugs used to treat uterine fibroids, progestogens, or “pure” progestogens, antiprogestins, antigonadotropins, and gonadotropin releasing hormone agonists retain their practical value.

 One of the most modern methods of conservative treatment of uterine fibroids is the use of gonadotropin releasing hormone (GnRH) agonists, such as zoladex (see the above source).

 The above treatment methods, such as the surgical method and the method of conservative hormonal therapy, are quite complex, expensive and are often accompanied by undesirable side effects.

 It has long been used as one of the alternative methods of treating gynecological diseases in medicine, phytotherapy, that is, therapy with the help of herbal remedies.

 For example, in patent RU 2045955 C1, 10.20.1995, a treatment method is described in which an infusion of common wormwood grass, bird of highlander grass, St. John's wort grass, perforated yarrow grass, wild strawberry leaf, warty birch leaf, peppermint leaf, lingonberry leaf is prescribed ordinary, rhizomes with the roots of elecampane high, licorice root bare, hops common fruit, the color of calendula officinalis, chamomile flowers, common fennel fruits, dioica nettle leaf.

 In the patent RU 2145232 C1, 02/10/2000, a method is described for treating uterine fibroids with the administration of a specific course, which includes taking collections of medicinal plants, vitamins and minerals.

 However, herbal medicine, as a rule, is not sufficiently effective, requires prolonged use, or provides only temporary compensation for violations.

 Another of the known methods used in gynecology is hirudotherapy, which is currently becoming increasingly common.

 So, there is a method of treating a number of diseases, including gynecological, such as infertility, by placing leeches, previously stimulated with an ionized lidase solution, at biologically active points, topologically or reflexively associated with the function of the damaged organ, 2-52 leeches for 31- 366 sessions, daily or discrete (patent RU 2106133 C1, 03/10/1998).

 There is also a method of treating endometrial hyperplasia by placing on the vaginal vaults on acupuncture points of 7-27 leeches for 31-90 sessions (RU 2104686 C1, 02.20.1998).

 These methods are not effective enough, require a long period of use.

 The objective of the present invention is to develop a highly effective method of treating a wide variety of gynecological diseases, reducing treatment time.

 This problem is solved by the development of a method of treatment, which is based on a combination of courses of hirudo and phototherapy.

 Treatment is based on baseline data: ultrasound and laboratory data.

 Patients are prescribed a course of hirudotherapy by staging from 5 to 15 leeches per session on the vaginal arches, perineum, anus and acupuncture points above the projection of the liver, spleen, adrenal gland, pancreas and thyroid gland with a course of 10-25 sessions, conducted after 1-10 days. The exposure time is determined depending on the location of the leeches. The time of their removal is determined by the beginning of peristaltic movements of the leech if it is placed on acupuncture points and the blood is completely saturated with blood if it is placed on the vaginal vaults, perineum and anus. Simultaneously with the course of hirudotherapy, patients are prescribed phyto-collection in the form of an infusion of dry plant material containing 0.5-10 wt.% Sweet clover, at the rate of 15-30 g of dry material per day.

The invention can be used in the treatment of a wide range of gynecological diseases, including:
- uterine fibroids;
- retentious ovarian cysts;
- adenomyosis;
- external genital endometriosis;
- chronic endometritis;
- chronic salpingo-oophoritis;
- primary infertility;
- varicose veins of the pelvis;
- miscarriage;
- endometrial hyperplasia.

 In the treatment of chronic endometritis, salpingo-oophoritis, the most effective is the appointment as a herbal infusion of medicinal plants, including sweet clover, yarrow, common oregano, medicinal calendula, centaury, large plantain, Japanese kelp thallus, knotweed grass, horsetail grass, nettle leaf, nettle leaf sage officinalis, coltsfoot leaf, chamomile flowers. At the same time, hirudotherapy begins from the 5th day of herbal medicine with a course of 16 sessions, preferably after 1-2 days. Leeches are placed in the navel, along the edge of the right costal arch, xiphoid process, coccyx, inside the vagina, perineum, lateral vaginal arches, on the reflex points C3, Th4, VC-2, BG-4, VB-29, VG-20, E -36, RP-6, VC-3, R-18, V-31, F-11, VC-4, VC-7, R-5, F-27, E-29, VC-1.

 In the treatment of uterine fibroids, adenomyosis, it is most effective to use the infusion of medicinal plants, including sweet clover, heart-shaped linden fruit, incense root, dioecious nettle, strawberry leaf, hanging birch buds, naked licorice root. At the same time, hirudotherapy begins from the first day of herbal medicine. Leeches are placed on the lateral vaginal arches, anus, navel, along the edge of the right costal arch, crotch, as well as reflex points C3, Th4, VC-2, BG-4, VB-29, VG-20, E-36, RP-6 , VC-3, R-18, V-31, F-11, VC-4, VC-7, R-5, F-27, E-29, VC-1.

 In the treatment of primary infertility, it is most effective to use infusion, including medicinal plants, as a herbal medicine: medicinal sweet clover, St. John's wort, field horsetail, elecampane root, calendula flowers, fennel fruits, and birch buds. At the same time, hirudotherapy begins on the third day of herbal medicine with a course of 8 sessions. Leeches are placed along the edge of the right costal arch, lateral vaginal arches, on the reflex points VC-2, BG-4, VB-29, VG.

The invention can be illustrated by the following examples:
Example 1. Patient S., 31 years old.

 DZ: Primary infertility, adenomyosis, chronic endometritis, chronic salpingoophoritis, adhesions in the appendages, antiphospholipid syndrome.

 Survey data. Ultrasound of the pelvic organs transvaginal: the uterus is normal in size. The cavity is not deformed. The structure of the endometrium is heterogeneous, coarse-grained, cellular in the inner layers. The right ovary is 32 • 42 • 51 mm, the left ovary is 41 • 52 • 48 mm, the contours are thickened. Adhesion process is defined on the right.

 Hemostasiogram: ABP 80, APTT 42. Lupus anticoagulant: ABP 110. Chemical and structural hypercoagulation. Test for lupus anticoagulant positive.

 The patient is prescribed a course of hirudotherapy and herbal medicine.

 Phyto-collection based on medicinal sweet clover 10%. Also included in phyto-collection: centaury 10%, oregano 10%, yarrow 8%, plantain large, leaf 10%, thallus Japanese kelp 3%, knotweed grass 5%, horsetail grass 8%, nettle leaf 5%, sage leaf medicinal 10%, coltsfoot leaf 10%, chamomile, flowers 6%.

 This herbal collection is prescribed in the form of infusion 1: 100 1/2 cup 3 times a day before meals for a course of two months.

 Hirudotherapy has been used since the fifth day of taking herbal medicine.

Hirudotherapy session 1:
Navel 2 pcs. 5 along the edge of the right costal arch leeches. The maximum exposure.

4 leeches, exposure 15 min: C3, Th4, xiphoid process.

Session 3 after 2 days:

Session 4 after 1 day:
5 leeches, 15 min exposure: E-36 (Tssu-San-Li), RP-6 (San-Yin-Jiao), VC-2 (Zhu-chi), R-18 (Shi-Guan), V-31 (shan-liao).

Session 5 after 2 days:
4 leeches, exposure 20 minutes: coccyx - 2 pcs., F-11 (yin-lian), VC-4 (guan-yian).

 Session 6 after 3 days.

 6 leeches, 20 min exposure: VC-7 (yin-jiao) - 2 pcs., R-5 (shui-qiuyan), E-27 (da-ju), E-29 (guan-lai) - 2 pcs.

 Sessions 7-11.

 Intravaginal staging of leeches of 8 pcs. with maximum exposure and an interval of 2 days.

Session 12 after 5 days:
10 leeches with maximum exposure in the perineum.

Session 13 after 5 days:
5 leeches with maximum exposure: VC-1 (hui-yin), anus.

Sessions 14, 15, 16:
7 leeches with maximum exposure to the lateral vaginal arches with an interval of 3 days.

 Survey data. Ultrasound of the pelvic organs transvaginal: the uterus is normal in size. The cavity is not deformed. The structure of the endometrium is homogeneous, coarse-grained, cellular in the inner layers. The right ovary is 35 • 20 • 21 mm, the left ovary is 30 • 18 • 20 mm. On the right, the adhesion process is not defined.

 Hemostasiogram: ABP 65, APTT 32. Test for lupus anticoagulant negative.

 After 1.5 months, pregnancy occurred, after 9 months. - childbirth.

 Example 2. Patient K., 44 years old.

 DZ: Uterine fibroids 7-8 weeks, adenomyosis, varicose veins of the small pelvis, adhesions in the region of the appendages.

 Survey data. Ultrasound of the pelvic organs: the uterus is enlarged up to 7-8 weeks of pregnancy, the contours are even, the structure of the myometrium is heterogeneous due to hypoechoic inclusions. On the front wall of the interstitial myomatous node 18 mm, the structure of the myometrium is heterogeneous, the cavity is not deformed, the ovaries are normal, the veins of the small pelvis and myometrium are dilated.

 The patient is prescribed a course of hirudotherapy and herbal medicine.

 Phyto-collection based on medicinal sweet clover 10%. Also included in phyto-collection: linden fruitfulness of heart-shaped 10%, frankincense root 20%, dioecious nettle leaf 10%, strawberry leaf 20%, birch buds hanging 10%, licorice root bare 20%.

 The patient is prescribed this phyto-collection in the form of a 1: 100 infusion, 1/2 cup 3 times a day before meals for a course of two months.

 From the third day of taking herbal medicine, hirudotherapy was used.

Sessions 1-6:
At intervals of a day, the leeches are placed on the lateral vaginal arches using a one-time Cusco gynecological mirror with 8 leeches with a maximum exposure.

Session 7 after 6 days:
5 leeches with maximum exposure: VC-1 (hui-yin), anus.

Session 8 after 3 days:
5 leeches with maximum exposure along the edge of the right costal arch.

Session 9 after 5 days:
10 leeches with maximum exposure to the perineum.

Session 10 after 5 days:
5 leeches with an exposure of 15 min: E-36 (zu-san-li), RP-6 (san-yin-jiao), VC-3 (zhu-ji), R-18 (shi-guan), V-31 (shan-liao).

Sessions 11, 12 after 5 days:
10 leeches on the perineum with maximum exposure.

Sessions 13, 14, 15:
On the lateral vaginal arches, 7 leeches with a maximum exposure with an interval of 3 days.

On the front wall of the interstitial myomatous node 6 mm, the structure of the myometrium is homogeneous, the cavity is not deformed, the ovaries are normal, the veins of the small pelvis and myometrium are not dilated.

 After 2 months, the patient underwent 6 sessions of hirudotherapy.

Session 1:

Session 2 after 3 days:
4 leeches with maximum exposure: C3, Th4, xiphoid process.

Session 3 after 5 days:
6 leeches with an exposure of 10 min: VC-2 (Qu-Gu), VG-4 (Min-Men), VB-29 (Ju-Liao-gu), VG-20 (Bai-Hui) - 2 pcs.

Session 4 after 4 days:
5 leeches with an exposure of 15 min: E-36 (zu-san-li), RP-6 (san-yin-jiao), VC-3 (zhu-ji), R-18 (shi-guan), V-31 (shan-liao).

Session 5 after 2 days:
4 leeches with an exposure of 20 minutes: coccyx - 2 pcs., F-11 (yin-lian), VC-4 (guan-yuan).

Session 6 after 3 days:
6 leeches with an exposure of 20 min: VC-7 (yin-jiao) - 2 pcs., R-5 (shui-quan), E-27 (da-ju), E-29 (guan-lai) - 2 pcs.

 Example 3. Patient G., 35 years old.

 DZ: Primary infertility, adenomyosis, retention cyst of the left ovary.

 Survey data. Ultrasound of the pelvic organs: the uterus is enlarged with clear contours, the cavity is not deformed, the structure of the myometrium is heterogeneous due to hypoechoic inclusions. The right ovary is 34 • 25 • 25 mm, the left ovary - a rounded formation with a diameter of 75 mm with a thin capsule and hypoechoic content is visualized. There are no internal partitions or wall overlays.

 The patient is prescribed a course of hirudotherapy and herbal medicine.

 Phyto-collection based on medicinal sweet clover 5%. Also included in phyto-collection: St. John's wort 5%, field horsetail 10%, elecampane root 10% high, calendula flowers 25%, fennel fruits 10%, birch buds hanging 35%.

 The patient is prescribed this phyto-collection in the form of a 1: 100 infusion, 1/2 cup 3 times a day before meals for a course of two months.

 From the third day of taking herbal medicine, hirudotherapy was used.

Session 1:
Navel - 2 leeches, along the edge of the right costal arch - 5 leeches. The maximum exposure.

Session 2 after 3 days:
6 leeches with an exposure of 10 min: VC-2 (Qu-Gu), VG-4 (Min-Men), VB-29 (Ju-Liao-gu), VG-20 (Bai-Hui) - 2 pcs.

Sessions 3-8 with an interval every other day:
staging leeches on the lateral vaginal arches using a one-time gynecological mirror according to Cuzco, 8 leeches with a maximum exposure.

 Survey data. Ultrasound of the pelvic organs: the uterus is normal in size with clear contours, the cavity is not deformed, the structure of the myometrium is homogeneous. The right ovary: 30 • 21 • 25 mm, the left ovary: 34 • 25 • 27, pathological formations were not detected.

 The presented examples confirm the high efficiency of the method.

 The method can be widely used in medical practice.
