Известен способ открытой миотомии двухсуставных мышц, когда производится вмешательство на уровне сухожилия, которое удлиняется за счет пластики собственными тканями (ахиллопластика по Краснову А.Ф.) или с использованием аллотрансплантатов (лавсанопластика по Мовшовичу И. Я. ). Для восстановления функций оперированной конечности требуется от 4-х до 6-и недель (Ортопедия и травматология. Т.3//под ред. Ю. Г. Шапошникова, 1997. - С. 455). Способ подкожной миотомии осуществляется следующим образом: из положения больного на животе путем переразгибания в тазобедренном и коленном суставах натягиваются мышцы-сгибатели голени. Пальпаторно определяется седалищный бугор. По методике В.Ф. Войно-Ясенецкого определяется проекция седалищного нерва. Строго по латеральному краю седалищного бугра перпендикулярно к коже вводится миотом до контакта с костью. Выполняется интраоперационная диагностика положения инструмента гальваническим током (сила тока 2-4 мА) и при отсутствии мышечных сокращений послойно отсекаются мышцы от седалищного бугра, контролируют полноту миотомии, следя за напряжением сухожилий в подколенной ямке. По окончании оперативного приема многом извлекается. Гемостаз давящей повязкой на 12 часов. Рана заживает на 7-е сутки, когда и целесообразно начать восстановительное лечение с использованием электромиостимуляции на большие ягодичные мышцы, пассивные и активные движения в поясничном отделе позвоночника, тазобедренном и коленном суставах.
