Abstract
Description
The invention relates to medicine, namely to urology, and relates to a method for the surgical treatment of vesicoureteral reflux in children.
П.К. В. 125-126/. A known method of treating vesicoureteral reflux according to Glenn-Anderson'y. The terminal part of the ureter is secreted by intravesical access. The ureter is released directly from its entrance into the bladder; a submucous tunnel is modeled towards the neck of the bladder, and the ureter is fixed to the surrounding muscles with small absorbable sutures. / "Bubble-ureter reflux in children," ed. prof. P.K. Yatsyk and prof. B. Zvara. M .: Medicine, 1990, p. 125-126 /.
However, Glenn-Anderson’s operation is indicated for ectopic ureters and wide Lietot triangles, where there is a relatively large distance between the mouth of the ureter and the internal opening of the urethra, moreover, the operation is effective only at low degrees of vesicoureteral reflux.
576-577). Closest to the proposed method for the treatment of vesicoureteral reflux by technical nature is the method of Politano-Leadbetter, described in the literature in 1985. Extraperitonially open the bladder. The mouth of the ureter is excised with a circular incision along with the intramural part to the outer wall of the bladder. The length of the mobilized part of the ureter is 3-5 cm. 3 cm above the mouth open the mucous membrane; through its defect, the clamp is passed through the detrusor and the mobilized ureter is captured, which is brought inside the bladder; then create a tunnel under the mucosa to the old estuary, along which the ureter is carried out and hemmed with nodal catgut sutures in the area of the estuary. The defect of the muscular membrane and mucous membrane is sutured (Surgical surgery with topographic anatomy of childhood. M: Medicine, pp. 576-577).
However, the known antireflux surgery does not prevent retrograde flow of urine from the bladder to the ureter, since the submucosal tunnel is not always possible to form a sufficient length, especially when the mobilized ureter expands.
The disadvantage of this method is the fact that the technical stages of neoimplantation of the ureter do not provide prevention of ureteral retraction in the postoperative period, due to the lack of a cushioning mechanism in the ureter-cystic segment.
The technical result of the proposed method is the elimination of reflux due to duplication of the passive valve locking mechanism of the ureter-cystic segment and the prevention of ureter retraction in the postoperative period.
The technical result is achieved by performing cystotomy, dissection of the ureter, suturing of the detrusor and the mucous membrane of the bladder.
New in achieving this goal is that the fixation of the ureter is carried out sinusoidally under the mucous membrane of the bladder with parallel anchor sutures.
Fixation of the implanted ureter sinusoidally provides elongation (in comparison with the rectilinear location of the ureter in the prototype method) of the valve zone of the ureter-vesiculate segment due to the curved path of the ureter axis. The sinusoidal bend of the ureter extends 1.5-2 times the effective valve area from the mouth of the ureter to its transition into the detrusor.
Also new is the fact that the submucosal sinusoidal bend of the ureter has a changing modulus of elasticity and the application of hydrostatic pressure exerts a cushioning effect in the phase of expulsion (or filling) of the bladder, thereby ensuring the functional adaptation of the ureter-bladder segment of the growing child’s body. Sinusoidal implantation of the ureter reduces the likelihood of ureteral retraction in the postoperative period, as it minimizes the cranial traction of the mouth, as it forms a more structured and voluminous submucosal tunnel, anchor sutures limit the mobility of the ureter wall, and form a submucous tunnel.
Comparative analysis with the prototype shows that the claimed method for the treatment of vesicoureteral reflux is characterized in that the fixation of the ureter is carried out sinusoidally under the mucous membrane of the bladder with parallel anchor sutures. Thus, the claimed method of treatment of vesicoureteral reflux meets the criteria of the invention of "novelty." A new set of features ensures the achievement of a high positive effect consisting in the elimination of reflux, duplication of the passive valve locking mechanism of the ureter-cystic segment and the prevention of ureter retraction in the postoperative period.
The essence of the proposed method for the treatment of vesicoureteral reflux is illustrated by the drawing, where 1 is the Lieto triangle, 2 is the mouth of the implanted ureter, 3 is the sinusoidal bend of the ureter in the submucosal tunnel, 4 and 5 are the curvature adaptive anchor sutures on the sides of the ureter.
A method for the treatment of vesicoureteral reflux is as follows. The position of the patient on his back with a flat roller, placed under the pelvis. Access - transverse overhead. The bladder is opened in the midline. The ureter is tubed with a polyethylene catheter, then an intramural section of the ureter is isolated with an incision of the mucous membrane around the mouth, followed by its dissection. The ureter is removed from the bladder by removing a catheter from it, which is replaced with a silk holder. The resulting detrusor defect is sutured (Vicryl 3 ° C). Departing from the location of the mouth by 4-5 cm along the intramural ureter, the mucous membrane of the bladder is dissected and a submucous tunnel is created with scissors in the direction of the mouth. Then the detrusor is pierced and the muscles are widely spread with a clamp. The ureter is pulled inside the bladder by the latch and is pulled into the formed submucous tunnel with a clamp, laying it in a curved, sinusoidal shape. The distal segment of the ureter is cut off. Vicryl (6 ° C) seams on the atraumatic needle connecting the mucous membrane of the ureter and the bladder at the site of the former mouth. The dissected mucosa is sutured at the site of the ureter to the bladder. Parallel anchor sutures on the sides of the ureter simulate the sinusoidal axis of the submucosal tunnel. The bladder incision is sutured with dexon (3 ° C) with a two-row suture. The wound is sutured in layers. The bladder is emptied through continuous urethral drainage.
Operation progress: Pfannenstiel incision, longitudinal cystotomy; the right mouth of the ureter is dilated, gaping. Tubation of the right ureter with a polyethylene catheter (F = 12). The contouring section of urothelium distinguishes the intramural ureter and its dissection to a length of 6 cm. The detrusor defect is sutured (Vicryl 3 ° C). Departing from the mouth by 4 cm along the intramural ureter, urothelium was dissected and a submucous tunnel was formed with scissors in the opposite direction. The detrusor is dissected and the muscles are wide apart with a clamp. The ureter is pulled into the bladder by the handle and, with the help of a clamp, is guided into the submucosal tunnel and laid curvilinearly in the form of a sinusoid. Vicryl (6 \ oo) on the atraumatic needle stitched joints connecting the urothelium of the ureter and the bladder at the site of the former mouth. Parallel anchor sutures on the sides of the ureter simulated the sinusoidal axis of the submucosal tunnel. Cystorography (dexon-3 \ oo) double-row seam. The wound is sutured in layers. The bladder is drained by the Foley urethral catheter, I. The postoperative period was uneventful. The urethral catheter was removed on day 3. Healing by first intention. Suppressive antibacterial and anti-inflammatory therapy was carried out for 1 month. A follow-up examination after 9 months found the elimination of vesicoureteral reflux on the right and an improvement in renal function according to dynamic gamma scintigraphy. No urinary infection detected, no complaints.
