Abstract

FIELD: medicine, psychiatry, psychotherapy. SUBSTANCE: at conducting rational psychotherapy a suggestion for fear and/or prohibition and/or aversion and negative behavior and sympathetic suggestion to a positive one are developed in accordance with which a suggestion type should be determined. A suggestion formula is developed which includes four components: basic, certain situational, logical and descriptive ones, in two variants: complete and multiple. A patient is put into hypnotic state and suggestive impact is performed by manifold repeating a suggestion formula. Simultaneously, mechanic stimulation of sensitive sites is conducted along with peroral intake of nicotinic acid. In patients dependent on psychoactive substances and obesity the action of nicotinic acid is supported by corresponding real stimulants due to an impact upon sense organs. It is necessary to carry out 1-10 seances. During intervals a patient is recommended to perform autosuggestive impact. EFFECT: improved stable result with no side effects and limits. 2 cl, 1 ex

Description

 The invention relates to medicine, namely to psychiatry, to methods of psychotherapeutic effects in the treatment of alcoholism, drug addiction, tobacco smoking, obesity, and also various borderline mental disorders.

 To date, in connection with the increasing prevalence of these psychophysical abnormalities, the development of effective methods for their treatment is becoming increasingly important.

 A preliminary analysis of the anamnesis and personality characteristics is carried out. Then they conduct rational psychotherapy, substantiate the need for cure, and increase their motivation for recovery. Suggestive effect is carried out in the awake or hypnotic state of the patient using the techniques of conditioned reflex reinforcement of suggestions. The “layering” of the human psyche, the preservation of old attitudes, concepts and motivations can cause the patient to fight consciously or subconsciously with the development of desirable for other reflexes and stereotypes of behavior.

 The objective of the invention is to create a universal and more effective method of psychotherapeutic effects, which allows to obtain a stable effect in a short time.

 With a psychotherapeutic effect on patients with substance dependence and obesity, the effect of nicotinic acid is reinforced by the corresponding real stimuli by exposing them to the patient’s sensory organs.

 The method is highly effective. The achieved effect is stable. Side effects from the application of the method have not been identified. The method has no restrictions associated with age or intellectual development. It can be used both in case of individual psychotherapy, and in group, and in any conditions - in medical institutions or at home. It goes well with any other type of psychotherapy.

 The method has a wide range of indications - neurotic disorders, personality pathology, psychopathy, pathological personality formation, alcoholism, drug abuse, nicotinism, obesity and their prevention. Among the contraindications, only endogenous mental diseases and hemorrhagic conditions can be noted. It should be noted that nicotinic acid is an effective anti-sclerotic, antihypoxic, lipotropic and vasodilator drug and has a general healing effect on the patient's body, contributing to the treatment of concomitant metabolic disorders, blood circulation, etc.

 The advantage of the method is also the ability to educate patients on the methods of psychotherapeutic self-action, which allow them to cope with stresses and negative internal psychological states on their own, without the help of a doctor.

 The method is as follows.

 Preliminarily analyze the patient’s complaints and medical history, psychological characteristics of his personality. Among the latter, special attention is paid to assessing the orientation of the personality, attitudes, motivations, hierarchy of life values in terms of their impact on the development of the disease, on the one hand, and in terms of resources, opportunities that can ensure recovery, on the other.

 When conducting rational psychotherapy, the patient enhances the motivation for recovery by comparing the subjectively attractive aspects of a healthy life with the negative manifestations and consequences of the disease, taking into account previously identified personality characteristics and anamnesis. Depending on psychoactive substances and obesity, the patient uses the “iatrogenization-hypochondriacisation” technique, the main essence of which is to exaggerate the consequences of the disease and the severity of the disorders found in the patient. During the conversation, the essence of the method is explained to the patient: it is reported that it includes hypnotic effect, positive self-programming, the use of drugs that enhance the memorization of medical information (nicotinic acid is used as a placebo), and the effect on biologically active points through acupressure (pseudo-depression, true the purpose of processing points is the formation of a conditioned reflex that suppresses abnormal behavior), which is presented to the patient as activating the memorization of medical information ation, stimulating the body's resources and the vast pathological needs.  Taking into account all the features of the patient’s personality, the selected settings, types of suggestion and treatment objectives, they formulate a suggestion formula for auto- and heterosuggestive effects, which includes four components: basic, concrete-situational, logical and figurative.

 The basic component defines the general, fundamental line of positive behavior in the future, the “philosophy of life”.

 A specific situational component determines the patient's behavior in a specific provocative situation (for example, stress).

 The logical component includes the rationale for a positive model of behavior and the personality's capabilities for its implementation.

 The figurative component - images, metaphors, symbols characterizing positive or negative patterns of behavior, or the process of recovery, getting rid of the symptoms of the disease, the influence of negative factors.

 Two versions of the therapeutic formula of suggestion are made up: the full one, which includes the maximum set of suggestive phrases (15-20 or more sentences) and images, and the short, compact one, consisting of the minimum amount of linguistic and figurative material (one or more keywords, one or two three phrases, motto, one or two images). Both versions of the formula are used both with hetero- and autosuggestive exposure.

 They explain to the patient that phrases should be without a “not” particle (affirmative), and the images should be attractive and competitive with respect to negative perceptions. The texts are pronounced by patients imperatively (in a command manner) or passively (as if from the side, hardly noticeable to consciousness) depending on individual preferences. In addition to this, the patient is taught methods of anticipating the situation (some time before it) and synchronous with it (parallel, accompanying the situation) self-hypnosis.

 Before conducting sessions with the suggestion of self-destruction reactions, in the case of the resumption of stereotypes of negative behavior when dependent on psychoactive substances, the patient takes a receipt where he confirms his voluntary consent to such treatment. After treatment, such patients are given a certificate stating the treatment method and medical contraindications (for example, alcohol-containing drugs after treatment for alcoholism).

 Suggestive therapeutic effect is carried out with immersion of the patient in a state of hypnosis (according to any of the schemes accepted in psychotherapy) to an individually accessible depth.  In the treatment of addiction to psychoactive substances (tobacco, alcohol) or obesity and the use of frightening, prohibitive or obverse suggestions, the effect of the treatment of sensitive points (before the sensation of pain) and the action of nicotinic acid are reinforced with the corresponding real irritants, by exposing them to the patient: alcohol - smelling smell alcohol or vodka, a sense of their taste (alcohol or vodka lubricates the lips, the patient’s tongue), 1-2 drops are dripped onto the tongue; tobacco - chewing tobacco, inhaling tobacco smoke, food - give a piece of sugar or, if possible, "microdose" of the product, causing addiction in the patient. At the same time, intolerance, inadmissibility or danger, or an aversion to these substances and behaviors are imperatively emphasized.

 In case of drug addiction and substance abuse, it is allowed to only present, recall your experience with these substances.

 In case of neurotic disorders and psychopathies of the inhibitory circle, mild, gentle acupressure effects and small doses of nicotinic acid are used. Depending on psychoactive substances, obesity and gross psychopathies of an excitable type, especially when combined with frightening, prohibitive or aversive suggestions, acupuncture effects are carried out more intensively using large doses of nicotinic acid.

 It is important to note that the patient should not be aware of the true nature of "acupressure" and "medicine to enhance memory."

 In total, from 1 to 10 such sessions are carried out. In parallel, patients are taught self-hypnosis according to any of the generally accepted schemes.

 In the period between the sessions and after their completion, the patient is advised to independently carry out an autosuggestive effect using the previously compiled formula of suggestion and methods of synchronous and advanced self-hypnosis.

 At the same time, in a state of hypnotic trance, the patient repeats the treatment formula, recalls the words of the doctor, and presents healing images. The treatment of sensitive points (acupressure) and the administration of nicotinic acid (issued by the doctor in bottles or bags with disguise inscriptions) can be performed by some patients in a state of self-hypnosis, others in a state of wakefulness. In the latter case, the number of acupressure effects during the day is not limited, taking nicotinic acid is carried out 2-3 times a day. At the same time, autosuggestion is performed. Autosuggestion without acupressure and nicotinic acid can be used an unlimited number of times a day, including in the morning and before bedtime using phase pro-wake conditions.

 As an alternative or addition to nicotinic acid, xanthinal nicotinate (1 tablet of 0.15 g each) can be prescribed for absorption in the oral cavity (the drug is bitter in taste and has a distracting and reinforcing setting of prohibition, intimidation and aversion of action).

 Complaints about mood instability, increased fatigue, irritability, self-doubt, a sense of inferiority, disturbed night sleep, increased need for alcoholic beverages, a lack of ability to control the amount of alcohol, loss of memory of individual events that occurred during intoxication, binges of up to 2-3 weeks, low mood, a sense of fear, tremor, sweating, pain in the heart in the morning with binge, decreased by alcohol.

 An analysis of these complaints suggests that the patient has emotional lability, on the one hand, and the syndrome of psychophysical dependence on alcohol, on the other, and both sides reinforce each other.

 When analyzing the anamnesis, it was revealed that the patient's father abused alcohol, was treated, children often saw his father drunk, they were afraid of him. Family upbringing was strict. He grew up impressionable, sensitive, fearful, and at the same time ambitious, considered himself better than others. He studied easily, graduated from high school, college. He lived in Bashkiria, the last 15 years in Moscow. He left an engineering profession, is engaged in the book business, has no steady success. 7 years old is in a civil marriage with a successful woman. He has been abusing alcohol for the past 10 years - he was looking in him for reassurance from experiences in connection with life crises and stimulation of thought (when drunk, he thought easier, had good ideas, raised his mood). In the last 4 years, the above complaints appeared, relations with his wife worsened, and practically stopped working.

 An analysis of the anamnesis shows the factors that contributed to the disease: heredity, negative childhood experiences, sensitivity combined with high self-esteem and claims, the presence of a contrast between one’s own failures and the wife’s success, the euphoric and activating effect of alcohol. The patient also said that he doesn’t want to change his little things in life and if he does not achieve very great success in business (financial and public), then he should not live. However, the implementation of grandiose projects lacked will, organization, patience and emotional stability.

 On the other hand, the patient’s positive qualities, which were used as resources capable of ensuring recovery, turned out to be: a well-developed sense of duty, high erudition, interest in psychology and self-hypnosis, a fairly good orientation to the highest social and spiritual values, and denial of the “alcohol route” in life. The suspiciousness in relation to one’s own health, which was found in the patient, was also used as a kind of resource during the “iatrogenization-hypochondriacis”.

 When conducting rational psychotherapy in conversation with the patient, the recovery motivation was enhanced by comparing the attractive aspects of a healthy life (the ability to achieve success in the areas of life that interests him, self-realization, building self-confidence, emotional stability, and the ability to healthy psychological stimulation of activity, thinking and mood ) with those negative, repulsive sides and the consequences of alcoholism, which were unacceptable for him as well (growing degradation and fiasco in all planes of existence, isolation from a normal human environment, early and unsightly death, etc.).

 Reception of "iatrogenization-hypochondriacisation", including the detection of a really non-existent alcoholic lesion of the heart, liver, nervous system, aggravation of the alcoholism station (it was reported that the completion of the 2nd stage and the "appearance" of the 3rd precursors) and the appointment of an “appropriate” treatment (which actually had a healing effect).

 During the conversation, the patient was also explained that the essence of the proposed method is the presence of hypnotic effects (in an accessible form it was told about the psychophysiology of hypnosis), positive self-hypnosis-self-programming, the use of drugs (amino acids) and effects on biologically active points (acupressure) to activate the memorization of treatment information, stimulation of body resources and suppression of pathological needs. It was also suggested that, in addition to these effects, the method can have a devastating effect on the body, up to the provocation of life-threatening conditions with the resumption of alcoholization. The possibility of this effect was explained by the ability of the human brain to fix and implement such suggestions-programs, fixed with the help of hypnosis, drugs and acupressure and in connection with the action of direct and reverse psychosomatic connections.

 The result of this conversation was the formation of a patient’s combination of several therapeutic and correctional psychological settings, which further determined the corresponding types of suggestion: frightening with the fear of the consequences of alcoholism in general and the consequences of the resumption of alcohol after treatment in particular; prohibitive intake of alcohol on the basis of fear and a rational understanding of the inappropriateness of alcoholization, aversive - forming an aversion to the stereotype of a drunken life based on the consequences of alcoholism and subsequent conditioned reflex reinforcement, sympathetic - stimulating motivation for a sober life and psychological harmony based on an understanding of their value and expediency to achieve your own goals.

Taking into account the psychological characteristics of the patient discussed above, the selected attitudes and types of suggestion, a suggestion formula was developed for hetero- and autosuggestive effects, which included four components:
1. Basic (defining the general, fundamental line of behavior in the future):

4. Shaped component (with inclusion of aversion):

It was explained to the patient that when varying phrases it was necessary to maintain their affirmative character (without the “not” particle), autosuggestion should be carried out both imperatively and in the form of a passive, inconspicuous pronunciation of the formula, as well as ahead of the situation (some time before it ) or in parallel, synchronously with the situation.

 Before the session with the suggestion of self-destruction reactions in case of resumption of alcoholization, the patient received a receipt in which he confirmed his voluntary consent to this treatment. After the session, he was issued a certificate informing about the treatment and medical "contraindications" (prohibition of alcohol-containing drugs).

 The suggestive effect was carried out with the patient immersed in a state of hypnosis according to the classical scheme up to the 2nd stage for 1 hour. During the suggestive effect, the previously suggested suggestion formula was repeated many times, both its full version and the short one.

 When suggesting a frightening, prohibitive and aversive attitudes, the conditioned-reflex direction was used: in the form of mechanical, tactile irritation of the sensitive points-exit zones of the trigeminal nerve branches, the area of the superciliary arches and wings of the nose, tragus auricles, He-Gu points on the hands; closer to the end of the session, nicotinic acid was given (10 tablets of 0.05 g with 200 ml of water); 2 minutes after the patient took nicotinic acid, he was given, to actively smell, the smell of cotton wool, abundantly moistened with alcohol. The suggestion of fear, prohibition, aversion and acupressure settings intensified as the pharmacological (vasodilating) effect of nicotinic acid increased, accompanied by reddening of the skin, a burning sensation, itching, and heat.

 In total, three such sessions were conducted. At the same time, the patient was given instructions for conducting classical self-hypnosis and he easily mastered it, when immersed in a trance, it repeats the compiled and memorized versions of the autosuggestion formula. They are also repeated in an awake state at a given time. In parallel, as necessary, conducts self-processing of sensitive points.

 Observation of the patient in dynamics shows a stable remission throughout the year. He copes with the difficulties of his business. He became more confident, balanced, more active, sleeps well at night. He is indifferent to alcohol, feels free among drinkers. Relations with his wife have improved. Complaints about the state of health does not show, pleased with the achievements.

The method of psychotherapeutic intervention, including the analysis of the anamnesis and personality characteristics of the patient, rational psychotherapy, suggestive exposure using techniques of conditioned reflex reinforcement, characterized in that in the process of rational psychotherapy they select and formulate an attitude of fear and / or prohibition, and / or an aversion to negative behavior and a sympathetic attitude towards the desire for positive behavior, in accordance with the chosen attitudes, determine the type of suggestions - frightening, and / and whether it is prohibitive, and / or aversion and sympathetic, taking into account which they compose a formula of suggestion for an auto- and heterosuggestive effect, which includes four components: a basic one that defines the general line of positive behavior, a specific situational one that determines the patient’s behavior in a specific provocative situation, logical substantiating a positive model of behavior and figurative, reflecting images, metaphors and symbols characterizing positive and negative models of behavior, teach the patient methods ahead of the situation and synchronous self-hypnosis with it, suggestive action is carried out in the hypnotic state of the patient, during which the formula of suggestion is repeated many times, and conditioned reflex reinforcement is carried out by mechanical stimulation of the sensitive points of the exit zones of the trigeminal nerve branches of the superciliary arches, base and wings of the nose, tragus auricles, mastoid processes, ulpar regions and biologically active points of He-Gu on the hands, as well as by oral administration of nicot into the patient's body of a new acid, activating a suggestive effect as its pharmacological effect intensifies, the entire course of treatment includes from 1 to 10 sessions, while in the period between the sessions and after they are completed, the patient is recommended to independently carry out an autosuggestive effect using the previously compiled formula and advancing and synchronous techniques auto-suggestion.  2. The method according to p. 1, characterized in that there are two versions of the formula of suggestion - a complete and short one, including a motto or keywords that accurately and succinctly determine a positive model of behavior, and use them for both hetero-and autosuggestive exposure.  3.
