Abstract
Description
The invention relates to medicine, specifically to pediatric surgery, and is intended for the treatment of various forms of megacolon, excluding Hirschsprung’s disease, for example dolichosigma, megarectum, neurogenic megacolon species.
There is a method of treating megacolon, which consists in conducting electrical stimulation of the left half of the colon through the anterior abdominal wall with diadynamic currents with a frequency of 50-100 Hz against the background of diet therapy and the use of therapeutic enemas (A.I. Lyuneshkin. Children's coloproctology. - M .: Medicine, 1990, p. 189). The disadvantages of the method include a large irritating effect on the skin even with a very low current strength (0.5 - 1 mA). In this regard, electrical stimulation in children is carried out at small currents, which significantly reduces the depth and effectiveness of exposure. In addition, with the generally accepted method of electrical stimulation of the intestine, both the parasympathetic and sympathetic parts of the autonomic nervous system are excited, and it is known that the excitation of the sympathetic part causes inhibition of intestinal motility.
In the literature available to us, the main studies of the nervous system responsible for the function of the colon were carried out either at the level of the nervous apparatus of the intestinal wall, or with a crude organic pathology of the central nervous system (spinal hernia, spinal cord injuries) with a pronounced clinic of both vegetative and somatic disorders. We have conducted studies that have revealed in 56% of children with dolichosigma myelopathy, which is a lesion of segments of the spinal cord in which the center of defecation is localized, which manifests itself only as a violation of the function of the rectum. Based on these conditions, it can be concluded that the generally accepted methods of electrical stimulation are insufficiently effective due to the fact that the effect either occurs on the damaged receptor apparatus (with hypogangliosis), or the point of application does not coincide with the site of the greatest disturbances (with myelopathy).
The objective of the invention is the provision of the possibility of treating nonaganglionic forms of megacolon in children, increasing its effectiveness, as well as improving and accelerating the rehabilitation of patients undergoing complex reconstructive operations on the colon, through the application of the method of central electrical stimulation.
The objective of the invention is achieved by introducing into the epidural space through the external sacral holes of the electrodes, followed by stimulation by sinusoidally modulated currents with a frequency of 50-150 Hz of the pelvic nerves. Sessions lasting 2 to 10 minutes are held daily for 10 to 14 days.
The invention is illustrated in the drawing. The method is as follows. According to physiology and anatomy, the parasympathetic defecation center 1 is located in 2 to 4 sacral segments of the spinal cord 2. The rectum 3 receives inversion due to nerves of the same segments 4, which pass in the epidural space of the sacral canal, reach the rectum, passing through 2-4 internal sacral openings. Access to these structures is fundamentally possible through the external openings on the sacrum. The supply of electrodes to the pelvic nerves 5 allows you to influence precisely the pathological focus, and not on the wall of the rectum 6, which is either intact (with myelopathy) or has an impaired receptor apparatus 7, which reduces the effectiveness of stimulation. After the introduction of the electrodes (we use PEVI-3, used for pacemaking) under the control of the Relief-1 system, a test electrostimulation session is performed with the Amplipulse-4 apparatus, which checks the correct position of the electrodes (with accurate installation, there are contractions of the muscles of the back of the thigh ), portability of the procedure, as well as individually selected exposure parameters (frequency, duration, modulation level, type of work) that most effectively enhance the tone and contractility of the intestine and. Sessions are held daily for 10 to 14 days. After that, the electrodes are removed.
Clinical trials have confirmed the effectiveness of the proposed method of electrical stimulation in cases of nonaganlionic forms of megacolon in children, which made it possible to achieve a stable clinical improvement in rectal function, confirmed by objective functional studies in patients with impaired distal colon, as well as concomitant pathology of the caudal part of the spinal cord.
