Abstract
Description
The invention relates to medicine, namely to traumatology and orthopedics.
Currently, in clinical practice, treatment of defects in articular surfaces is carried out only by replacing them with cartilage allografts or autografts. The cultivation (modeling) of hyaline cartilage in the patient's body for the subsequent replacement of defects in articular surfaces has never been carried out before.
So there are known methods for restoration of articular cartilage in a clinic, the essence of which is the transplantation of cartilage allografts (see, for example, source: I.U. Salmagambetov "Monocomponent and combined plate in surgery of bone-articular tuberculosis", Bulletin of surgery.- 1983. - T . 130, - N 4. - P. 102-108) and autografts taken from other joints (see, for example, source: S. M. Voronchikhin, Speeches in debate. - Questions of reconstructive surgery, traumatology and orthopedics. - Sverdlovsk : 1959. - T. 7. - S. 158-159). However, non-viable foreign cartilage is transplanted during allotransplantation, and significant damage is irrevocably damaged to the patient’s other joint during autotransplantation, which is the main obstacle to the use of the methods in the clinic.
There is also a method of replacing cartilage defects (see, for example, source: I. G. Marakush, “Autoarthroplasty of the hip joint with hyperplasia and defects of the femoral head,” Orthopedics, traumatology and prosthetics. - 1985. N 2. - P. 49-50 ) taken by us as a prototype. The essence of the method consists in the replacement of defects with cartilaginous autografts, taken from the non-support sections of the articular surfaces of the same joint.
The disadvantage of the above method is its unacceptability in the absence or insufficiency of free non-supportive articular surfaces in the joint.
The objective of the proposed invention is the restoration of articular cartilage by modeling (growing) hyaline cartilage in non-loaded areas of the joint for subsequent use of cartilage for autotransplantation.
The task is achieved in that the cartilage autograft is grown (simulated) in non-loaded areas of the joint by means of a closed closed fracture-subluxation, for example, an elbow process; creating and maintaining a focus of irritation on the surface of the bone, devoid of articular cartilage until the formation of sufficient size of the cartilage surface.
The drawing shows a scheme of intervention on the elbow joint and the formation of articular cartilage, where:
position 1 - General view of the fracture-subluxation of the ulnar process of the ulnar bone,
position 2 - micropreparation of the slice in the sagittal plane of the intact humerus,
position 3 - a micropreparation of a slice in the sagittal plane of the humerus with a portion of the grown hyaline cartilage (marked by an arrow),
position 4 is a general view of the transplantation of undeveloped cartilage.
The method of restoration of articular cartilage is as follows. The area of the elbow joint of the rabbit is anesthetized with 5.0 ml of a 0.5% solution of novocaine. The branches of Liston's nippers are applied to the area of the ulnar process. By gradually compressing the branch of the nippers, a complete closed intraarticular fracture of the ulna is carried out in the region of the ulnar process. Complete dissociation of the ulnar process with the ulnar bone is done manually by the swinging movements of the ulnar process. Due to the reduction of the 3rd head muscle of the shoulder between the bone fragments, diastasis of about 5 mm is formed, the bone fragment passes into the position of subluxation. External limb immobilization is not imposed. When the animal moves, a bone fragment acts on the area of the humerus, devoid of cartilage. Already after 2 months, on the posterior surface of the distal humerus, a site of a newly formed hyaline cartilage of sufficient size is determined, which retains its structure for at least 6 months after the intervention. The resulting cartilage is replaced by a defect in the articular surface of the same joint by autotransplantation.
