Abstract
Description
The invention relates to medicine, namely to traumatology and orthopedics, and is used for prosthetics of the hip joint.
There are a significant number of analogues of the proposed device. These include the hip joint prosthesis (1), in which, in order to reduce resorption phenomena and exclude alternating lever loads, the diaphyseal shank in the proximal part is equipped with a hemispherical thrust bearing, and the head is made in the form of a ball mounted between a hemispherical thrust bearing and a cup with the possibility of free rotation around three orthogonal axes.
A disadvantage of the described device is the presence of a large contact area in the moving unit with both a hemispherical thrust bearing and an acetabulum cup, which prevents the ball from rotating in the reference phase of the step.
Closest to the claimed technical solution is the "Hip Joint Endoprosthesis" (2), considered as a prototype. On the surface of the ball-shaped head of the prototype, cylindrical recesses are made, and balls are placed in each of them. However, in this design, with large amplitudes of motion, balls can drop out of the recesses.
In addition, it is possible to jam balls in the sharp corners of the recesses, where they will be installed under their own weight and thus prevent rotation, blocking movement in the joint.
The present invention allows to obtain an endoprosthesis with reduced friction between the elements of an artificial joint, an increased resource and increased functional reliability.
The essence of the claimed invention consists in the combination of essential features sufficient to achieve the desired technical result, namely, reducing friction between the surfaces of the head and cavity of the prosthesis to prevent device instability in bone tissue.
The essential features of the claimed object are that between the surface of the head and the bottom of the cavity there are balls enclosed in a hemispherical form of a cage with holes smaller than the diameters of the balls, while the cage is fixed to the end surface of the cavity and covers the balls in such a way that it does not touch the surface of the head and bottom of the cavity, located on one side of the equator balls closer to the head.
The presence of the cage ensures a constant arrangement of the balls relative to each other, thereby achieving uniform pressure on the surface of the head and the bottom of the cavity at any relative position of the articular surfaces.
Placing the cage on one side of the equator of the balls and closer to the surface of the head with hole sizes in the cage smaller than the diameters of the balls ensures that the latter are kept within the cavity when it may be necessary to replace the movable component of the prosthesis (head with neck and leg).
The device is shown in the attached drawing. It contains a cavity 1, a hemispherical holder 2 fixed on it with holes filled with balls 3 for rotation, a spherical head 4, a neck 5, a leg 6. The holder is fixed to the end of the cavity 1 with screws 7. The gap between the surface of the cavity and the facing to it, the surface of the cage is larger than the radius of the ball, the holes in the cage are smaller than the diameter of the ball.
The device is used as follows.
The area of the hip joint is exposed, scarred capsule and paraarticular tissues are excised. The proximal femur dislocated from the cavity is resected. The acetabulum of the pelvis is processed and installed in it, fixing in one of the accepted ways, the cavity of the endoprosthesis 1 together with the balls 3 held in its cavity by the cage 2. The prosthesis leg 6 is installed in the channel formed in the femoral diaphysis. The head 4 is pushed into the cavity 1, covered with a holder 2 with balls 3. The wound is sutured in layers.
