Abstract
Description
The invention relates to the field of medical equipment and is used in traumatology-orthopedics to replace a pathologically altered hip joint.
There are a significant number of analogues of the proposed device, for example, "Hip Endoprosthesis" [1], on the spherical head of which cylindrical recesses are made and balls are placed in each of them. However, with large amplitudes of movement, the balls can drop out of the recesses, as well as jamming the balls in the sharp corners of the recesses, where they will be set under their own weight and prevent rotation, blocking the movement of the joint.
Closest to the claimed technical solution is the "Hip Joint Endoprosthesis" [2], considered as a prototype in which balls are placed between the surface of the head and the bottom of the cavity enclosed in a hemispherical cage with holes fixed to the end surface of the cavity. The disadvantage of the described device is the wear of the hemispherical surface of the acetabulum in the places of rotation of the balls.
The essence of the claimed invention consists in the combination of distinctive features sufficient to achieve the desired technical result, namely, to increase the durability of the joint by reducing wear on the surface of the cavity of the prosthesis.
The essential features of the claimed object are that the balls made of two sizes do not completely fill the space between the head, the bottom of the cavity and the ring holding the balls with the possibility of their random movements with the movements of the head held by a flange fixed together with the ring at the end of the cavity.
The invention allows to obtain an endoprosthesis with increased functional reliability and an increased service life due to uniform wear of the hemispherical cavity.
The presence of balls of two sizes with an incompletely filled space between the surfaces of the head and cavity provides their free movement in different directions with the support function of balls of larger diameter and the separating function of balls of smaller diameter.
The device is shown in the attached drawing.
It contains a cavity 1, a spherical head 2, which is held in a cavity 1 by a flange with a flange 3 through a ring 4 with screws 5, as well as a neck 6 and a leg 7. In the gap between the head 2 and the bottom of the cavity 1 there are balls of larger diameter 8 and smaller diameter 9 in such a way that there is a free space 10, changing its position during operation of the device. The number of balls of smaller diameter should be greater for their full separating function. This allows for the circulation of balls to obtain non-repeating trajectories of motion, which significantly reduces the local wear of the bowl of the cavity and increases its durability.
During movements, the head 2 carries with it balls 8 and 9, which rotate at different speeds in accordance with their different diameters, so that the separating balls 9 are more or less evenly distributed between the support balls, forming a surface of balls on the spherical surface of the head 2 s minimum energy of resistance to her movements. In addition, the minimum resistance energy is provided by rolling friction that occurs between the supporting 8 and separating 9 balls.
The device is used as follows.
Lateral access exposes the area of the hip joint. The surrounding scarred altered paraarticular tissues are excised, and a pathologically altered femoral head and neck are resected. A bone marrow canal of the thigh is drilled, where the endoprosthesis leg is inserted and fixed. They process the acetabulum of the pelvis, install the pelvic component of the endoprosthesis in it and fix it. The prosthesis head is set into an artificial cavity. The wound is sutured in layers.
Postoperative rehabilitation of the patient is carried out according to the general principles of treatment for patients who underwent hip joint surgery.
