Abstract
Description
The invention relates to medicine, and more particularly to ophthalmic surgery and can be used to treat postoperative complications.
The known method of anesthetic management of intraocular operations, described in the work "An improved method of anesthesia to provide eye operations." The method consists in puncture of the pterygopalatine fossa with a blockade of the second branch of the trigeminal nerve and the pterygopalatine node located in it and subsequent puncture through the lower orbital fissure of the orbit and the introduction of an anesthetic there. However, this method will not allow to completely and effectively solve the problem of reducing postoperative complications.
The technical problem solved by the invention is the reduction of postoperative complications.
The specified technical problem is solved by the fact that the method of treatment of postoperative ophthalmological complications consists in puncturing through the pterygopalatine fossa and lower orbital fissure of the orbital cavity and administering the drug, additionally, the catheter is inserted through the needle into the orbital cavity and fixed, after which the drug is administered in the form of a solution an opioid peptide-enkephalin, for example, dalargin.
The method is as follows.
Under aseptic conditions, after local infiltration anesthesia is performed, a puncture is performed through the pterygopalatine fossa and lower orbital fissure of the orbit cavity with a 1 mm diameter and 80 mm long needle. After puncture through a needle, a catheter is inserted into the cavity of the orbit. The catheter is fixed to the skin with a band-aid. Subsequently, a solution of an opioid peptide, for example, enkephilin dalargin in a volume of 1 ml at a dose of 0.001 g, is introduced through a catheter into the orbit cavity.
The use of substances of a number of opioid peptides can significantly stimulate the reaction of antibody formation, which in turn leads to an increase in the body's immune response to the inflammatory process and reduces the time of postoperative rehabilitation of patients.
The method is characterized by the following examples:
Example 1. Patient M., 54 years old, DS: post-traumatic cataract of the left eye. VIS = OD / OS = 1.0 / 0.005 nk.
10/15/93 produced the extraction of traumatic cataract, implantation of an intraocular lens (model T26). The first day - the condition corresponded to the severity of the operation. On the second day, a fibrin effusion appeared on the lens in the pupil area. In addition to traditional local treatment, orbit catheterization was performed. For two days, a solution of the opioid peptide enkephalin dalargin in a dose of 0.001 g in a volume of 1 ml was injected into the orbit cavity twice a day.
For three days of treatment, the reaction is completely stopped. VIS = od / os = 1.0 / 0.5 at discharge.
Example 2. Patient N. 65 years. DS: mature cataract of the right eye, initial left eye. VIS = od / os = 0.02 nk / 0.1 -2.0D 0.3.
VIS=od/os=0,4/0,1 -2,0D v 0,3. The course of the operation without features. On the first day after the operation, fibrin effusion in the pupil zone was observed, the level of hypopion in the anterior chamber was 2 mm. The first day the treatment is traditional with the inclusion of subconjunctival injections of antibiotics. Positive dynamics is weak. On the second day, the orbit cavity was catheterized. The administration of opioid peptides (dalargin) has begun. After two days, the reaction is completely stopped. At discharge, VIS = od / os = 0.4 / 0.1 -2.0D v 0.3.
The application of the proposed method allows to reduce the trauma of nerve endings in the pterygopalatine fossa during punctures, to exclude daily punctures.
