Abstract
Description
Field of technology to which the invention relates
The invention relates to neurosurgery, namely to methods for preventing postoperative swelling of soft tissues.
State of the art
д. After neurosurgical operations that include manipulation of the structures of the orbit (for example, removal of a cranio-orbital tumor), in most cases, swelling of the intraorbital soft tissues of varying severity is observed. Swelling can lead to negative consequences: pain, impaired trophism and mobility of the eyeball, blurred vision, etc.
There is a known method for the prevention of postoperative swelling of the soft tissues of the orbit, including performing surgical access to the structures of the orbit and skull base affected by the tumor, removing the tumor, reconstructing the bone defect, closing the wound and administering glucocorticoids (hydrocortisone or dexamethasone), while glucocorticoids are administered intravenously or orally for several days after surgery.
This method is characterized by the following disadvantages.
д. [1].Firstly, the introduction of glucocorticoids into the systemic circulation is accompanied by an increased risk of their side effects: Cushing's syndrome, adrenal insufficiency, hyperglycemia, dyslipidemia, cardiovascular pathologies (angina pectoris, heart failure, etc.), osteoporosis, mental disorders, immunosuppression, etc. d. [1].
Secondly, when administered systemically, the passage of glucocorticoids to their destination is limited, thus reducing the effectiveness of their main pharmacological effects (in this case, the anti-inflammatory effect).
Disclosure of the Invention
The technical result of the proposed invention is to minimize the side effects of glucocorticoids and increase the effectiveness of preventing postoperative swelling of the soft tissues of the orbit.
To achieve the specified technical result, a method has been developed for the prevention of postoperative swelling of the soft tissues of the orbit, including surgical access to the structures of the orbit and skull base affected by the tumor, tumor removal, reconstruction of the bone defect, wound closure and administration of glucocorticoids, characterized in that a thin elastic catheter made of bioinert polymer; a canal is formed in the lateral wall of the orbit; the distal end of the catheter is perforated and inserted through the specified channel into the orbit, placing it in the fatty tissue between the superior and lateral rectus muscles of the eye; a counter-aperture is applied in the posterior frontal or parietal region and a straight tunnel is made under the temporal muscle in the direction from the orbit to the counter-aperture; the proximal end of the catheter is passed through the tunnel towards the contraperture and brought out; the proximal end of the catheter is treated with an antiseptic and a sealing plug is put on it; in the postoperative period, a solution of glucocorticoids is injected into the catheter, with the first injection carried out immediately after the operation, and subsequent injections are carried out once a day for five days; upon completion of the course of glucocorticoid administration, the catheter is removed.
A radiopaque silicone lumbar catheter can be used as a catheter.
An aqueous solution of hydrocortisone or dexamethasone can be used as a glucocorticoid.
The use of a thin elastic catheter allows glucocorticoids to be administered directly into the area of their destination (the soft tissue of the orbit), which increases their main anti-inflammatory effect and reduces the risk of side effects.
Forming a channel in the lateral wall of the orbit allows the distal end of the catheter to be inserted into it.
The location of the distal end of the catheter in the fatty tissue between the superior and lateral rectus muscles of the eye, as well as the removal of the catheter through the formed straight tunnel under the temporal muscle in the direction from the orbit to the contraperture applied in the posterofrontal or parietal region, ensures reliable temporary fixation of the catheter in the orbit; Moreover, if necessary, it is possible to easily remove the catheter by simply pulling it.
The administration of glucocorticoids through an installed catheter immediately after surgery and daily for five subsequent days provides the amount of glucocorticoids necessary and sufficient to prevent the development of early postoperative edema.
Thus, the proposed method allows us to minimize the side effects of glucocorticoids and increase the effectiveness of preventing postoperative swelling of the soft tissues of the orbit.
Carrying out the invention
A method for preventing postoperative swelling of the soft tissues of the orbit includes surgical access to the structures of the orbit and skull base affected by the tumor, tumor removal, reconstruction of the bone defect, wound closure and administration of glucocorticoids. To administer glucocorticoids, a thin elastic catheter made of a bioinert polymer is used. A channel is formed in the lateral wall of the orbit, the distal end of the catheter is perforated and inserted through the specified channel into the orbit, placing it in the fatty tissue between the superior and lateral rectus muscles of the eye. A counter-aperture is applied in the posterior frontal or parietal region and a straight tunnel is made under the temporal muscle in the direction from the orbit to the counter-aperture. The proximal end of the catheter is passed through the tunnel towards the contraperture and brought out; the proximal end of the catheter is treated with an antiseptic and a sealing plug is put on it. In the postoperative period, a solution of glucocorticoids is injected into the catheter, with the first injection carried out immediately after surgery, and subsequent injections are carried out once a day for five days.
The proposed method is used in the early postoperative period in patients who have undergone any neurosurgical operation involving the orbit (for example, removal of a cranio-orbital tumor).
During the operation, after immersing the patient in anesthesia and optimal positioning, surgical access is performed to the affected structures of the orbit and base of the skull, the tumor is removed and the bone defect is reconstructed; These actions are performed according to generally accepted algorithms.
Next, take a thin elastic catheter made of a bioinert polymer (for example, a radiopaque silicone lumbar catheter) and make several side holes in its distal end using scissors. A channel is formed in the lateral wall of the orbit for passing the catheter using a bur; the distal end of the catheter is inserted into the fatty tissue of the orbit through the made channel, placing the catheter in the fatty tissue between the superior and lateral rectus muscles of the eye. In the posterior frontal or parietal region, a counteraperture is applied using a scalpel. A straight tunnel is laid under the temporalis muscle in the direction from the orbit to the contraperture using a clamp; This clamp is used to fix the proximal end of the atheter and remove it through the tunnel through the counter-aperture. The proximal end of the catheter is treated with an antiseptic and a sealing plug is placed on it. The wound is sutured using generally accepted methods.
In the postoperative period, a glucocorticoid solution containing 15±5 mg of prednisolone or 4±8 mg of dexamethasone is injected into the catheter through a plug. The first injection is carried out immediately after surgery, and subsequent injections are carried out during dressings once a day for five days. Upon completion of the course of glucocorticoid administration, the catheter is removed; to do this, it must be carefully pulled by the plug, measuring the force to prevent the catheter from breaking.
Clinical examples
