Abstract
Description
The invention relates to medicine, namely to enodocrinology, therapy, surgery, obstetrics and gynecology, and can be used in clinical practice for examination, treatment, observation of patients, as well as for identifying persons with risk factors.
A known method for diagnosing the functional state of the pituitary-adrenal system (GNS), which consists in a separate determination of the concentrations of adrenocorticotropin (ACTH), somatotropin (STH), aldosterone (ALD), plasma cortisol [1] regardless of gender, age, and also without assessment combination of concentrations of ACTH, STH, ALD and cortisol at the same time.
Earlier in our works (2 6) and other studies (7 9), significant differences in the concentrations of STH, AL0d, and cortisol were revealed depending on gender and age. Literature data on the study of the functional state of GNS on the concentrations of ACTH, STH, ALD in the blood plasma of healthy women aged 20 34 liters, depending on the level of cortisol, we did not meet.
The objective of the invention to identify differences in the concentrations of ACTH. STH, ALD in the blood plasma of healthy women of reproductive age, depending on the level of cortisol concentration in order to increase the accuracy of determining the functional state of GNS.
The method is as follows.
1. The radioimmunological method of analysis determines the concentration of ACTH, STH, ALD, cortisol in the blood plasma of the examined women.
2. Concentrations of cortisol are divided into 3 groups: the 1st group with a low level of cortisol, the 2nd group with an average level of cortisol, the 3rd group with a high level of cortisol.
1). 3. According to the level of cortisol, the respondents are divided into groups (Table 1).
2). 4. Using statistical analysis, the average concentrations of the hormones M, the error of the average m, and the reliability of various P are determined by the Mann-Whitney method (Table 2).
Example. A total of 159 healthy women donors with no diseases of reproductive age 20 34 L (average age 26, 64 + 0.31) with a minimum of 10 years of experience in Novosibirsk were examined.
The functional activity of GNS was evaluated by the concentrations of ACTH, STH, ALD, and plasma cortisol.
Blood was taken from the cubital vein with disposable needles on an empty stomach in the supine position, i.e., in a state of physiological rest from 8 to 10 a.m. The resulting blood was centrifuged for 15 min at 1500 rpm, the plasma was separated and stored at a temperature of -20 o C until analysis, but not more than 2 weeks, thawing once.
The results of the study of the functional state of the GNS, are presented in table. 2 indicate a significant difference in the concentration of STH between groups 1 and 3 depending on the level of cortisol 2.23 ± 0.43 ng / ml and 5.67 ± 1.10 ng / ml, respectively, at P <0.05 and ALD between 1 and 3 groups - 108.85 ± 11.48 pg / ml and 142.61 ± 10.64 pg / ml, respectively, at P <0.01 In healthy women.
Thus, the presented data show an improvement in the diagnostic effect in comparison with the prototype and allow us to assess the state of GNS by the gluco-mineralocorticoid function of the adrenal cortex and somatotropic pituitary gland in women.
For an accurate diagnosis in women, it is necessary to determine the concentration of ALD, STH in blood plasma, depending on the level of cortisol.
A method for assessing the functional state of GNS by the concentration of ALD, STH in blood plasma in women depending on the level of cortisol is of great importance in interpreting the data, choosing therapeutic tactics, and determining the prognosis of many diseases. This determines the need to improve the accuracy of both the study itself and the method for assessing the state of GNS.
A positive fact is that the data were obtained on a sample of practically healthy female donors with at least 10 years of experience in Novosibirsk, which allows the results of the study to be used as a regional norm for women aged 20 34 liters in Western Siberia and used in clinical practice in the examination, treatment, observation of patients, as well as to identify individuals with risk factors.
