Abstract
Description
The invention relates to medicine, in particular ophthalmology, and is intended for preoperative diagnosis of the possibility of inflammatory complications after cataract extraction.
A known method for predicting inflammatory complications (ed. St. N 1649442 from 05.15.91), taken as a prototype, which consists in determining the level of S antibodies in the lacrimal fluid of patients with uveal cataract before surgery. Detection of S-antibodies at a concentration of 1:32 or more is an unfavorable factor that causes a high risk of inflammatory complications after cataract extraction. However, in cases of cataract of another etiology (glaucoma, diabetes) in the absence of S-antibodies in the lacrimal fluid of patients before surgery, a high percentage of inflammatory complications is noted, which reflects the lack of information of the method.
The frequency of inflammatory complications after extraction of complicated cataracts with a combined eye pathology (diabetes, glaucoma, retinal pigment dystrophy) according to the literature remains at a high level of 3-12%. A significant role of cytokines in the regulation of the immune response and the development of postoperative complications is proved.
The aim of the invention is the selection before surgery of patients with a high risk of inflammatory postoperative complications and the determination of tactics of preoperative anti-inflammatory therapy to reduce the risk of complications; improving the accuracy of the method of immunological examination of patients.
The goal is achieved in that in a method for predicting inflammatory complications after cataract extraction, an immunological study of the lacrimal fluid is carried out using an enzyme-linked immunosorbent assay (ELISA) for the presence of one of the cytokines of tumor necrosis factor α human TNF α and when detected at a concentration of at least 4.8 ng / ml predict the development of inflammatory complications.
The method is as follows.
The tear fluid is taken by the melanger 1 day before the operation. The reaction is carried out on a standard plate of 96 microcuvettes using an enzyme-linked immunosorbent assay based on monoclonal antibodies to human TNF-a (All-Russian Research Institute for High-Purity Preparations, St. Petersburg). At the beginning, the first antibodies to TNF α at a dilution of 1: 100 are applied to the walls of polystyrene cuvettes and fixed with a buffer to immobilize antibodies for 1 h at room temperature. Then, the liquid is removed from the cells by suction and the board is washed with washing buffer. The studied samples of tear fluid are added to the wells of the plates at a dilution of 1: 100 and 1: 200 (at least 2 parallels) in a volume of 50 μl. In cell A 10 and A 2 B 2 B 10 is made by 50 l of standard TNF α, rastitrovannogo multiple of two buffer sets, starting with a concentration of 4 ng / ml. The microplate was incubated for 1 h at room temperature and continuously shaken, after which it was again washed with buffer solution.
Then, 50 μl of 2 antibodies to TNF α at a dilution of 1: 100, with a treatment enzyme (horseradish peroxidase) are added to each well and incubated again for 1 h at room temperature and shaken. Wash the microplate and rinse the plate twice with distillate.
A solution of ORD (orthophenyl dialine) and 3% H 2 O 2 is prepared and 50 μl of this solution is added to each well. Incubated for 10-15 minutes in the dark at room temperature. The reaction is stopped with a 1 molar solution of H 2 SO 4 , adding 50 μl to the cell. The optical density of the solution is determined at a wavelength of 490 nm on any automatic reader by setting the absorbance O for cell A. A calibration curve is prepared according to the TNF standard α and the monokine concentration in the sample is calculated from it.
The rationale for the proposed method are the results obtained from the examination of 50 patients with complicated cataracts against the background of glaucoma, diabetes, etc.
50. By a known method (determination of S-antibody levels in the tear fluid), an unfavorable prognosis of inflammatory complications was canceled in 4 patients (8%) out of 50.
After uncomplicated cataract extraction in 50 patients in 13 cases (26%), inflammatory complications were noted in the form of an exudative reaction with the appearance of the Tyndall phenomenon and fibrin filaments in the moisture of the anterior chamber.
Analysis results of the study of TNF α levels in the tear fluid of patients before surgery, that 11 (85%) of 13 patients with inflammatory complications in the tear fluid detectable TNF α at a concentration of not less than 4.8 ng / ml.
(P<0,05).In 37 patients without inflammatory complications TNF α in the tear fluid or is undefined, or concentration was below 4.8 ng / ml, significantly lower than in patients with inflammatory complications (P <0,05).
Example 1. Patient K. Diagnosis: type II diabetes mellitus, complicated cataract OD. Before surgery, visual acuity OD 0.01 n / a.
Проведена неосложненная экстракция катаракты с имплантацией ИОЛ (Т26).Before surgery, indicators in a tear: TNF concentration α 5 ng / ml. Uncomplicated cataract extraction with IOL implantation (T26) was performed.
In the postoperative period, diffuse corneal edema, descemetitis, fibrin filaments in the moisture of the camera. Thus, a high concentration (2.0 ng / ml) of TNF α before surgery reflected a high risk of developing inflammatory complications after cataract extraction.
Patient P. Diagnosis: operated glaucoma, complicated cataract OS. Prior to surgery, visual acuity OS 0.04 n / a. Before surgery, indicators in a tear: concentration of TNF α 0.5 ng / ml. Uncomplicated cataract extraction with IOL implantation (T26) was performed. From the first day after surgery - an arreactive course, visual acuity 7 days after surgery 0.4 sf - 1.5 0.8.
Thus, when the concentration of TNF α less than 4.8 ng / ml, namely 0.5 ng / ml, there was an arreactive flow p / o period.
The method allows for preoperative immunodiagnostics of the risk of developing inflammatory complications after cataract extraction. This is important for preoperative preparation and the choice of cataract extraction method.
The positive effect of the proposed method is to increase the accuracy of the prognosis of inflammatory complications. So, according to the method taken as a prototype, the prognosis of complications was 31% (out of 13 patients with inflammatory complications, 4 had a high content of S antibodies), while when determining the level of TNF α in the indicated concentration, the prognosis for the development of complications was 85% (in 11 patients out of 13 with postoperative complications).
