One of the options for implanting screws in the lumbar spine is translaminar implantation. This implantation is technically difficult to perform due to the variability of the individual structure of the vertebra and is possible only when using navigation.

Known individual navigation guide for implantation of screws in the lumbar spine from a unilateral access, presented by Cao Y et al. and Zhen-XuanShao et al. [Cao Y, Zhang W, Liang Y, Feng Z, Jiang C, Chen Z, Jiang X. Translaminar facet joint screw insertion with a rapid prototyping guide template: a cadaver study. ComputAssist Surg (Abingdon). 2019 Dec;24(l):l-6. doi: 10.1080/24699322.2018.1542027. Epub 2019 Jan 21. PMID: 30661418.], containing a guide tube and a support pad for the arch, interarticular part, joint and transverse process on one side of the vertebra. This guide is the closest analogue.
The disadvantage of the closest analogue is the insufficient fixation of the guide. the possibility of displacement in the axial and sagittal planes.
The technical result of the utility model is a reliable fixation of the guide without displacement in the axial and sagittal planes.
The specified technical result is achieved in an individual navigation guide for the formation of routes in the vertebra under the screws of the lumbar spine from a unilateral access, which is a monolithic frame made of a biocompatible material, created on the basis of computed tomography data of the spine using additive technologies, containing a support pad on the arch, interarticular part , a joint and a transverse process on one side of the vertebra, repeating the relief of this zone of comparison with the vertebra, and a guide tube for forming a route in the vertebra along the translaminar implantation trajectory on the contralateral side of the vertebra, installed coaxially with the planned implantation trajectory at the design stage of the guide, characterized in that it has an additional support platform for the spinous process, repeating the relief of this zone of comparison with the vertebra, and an additional guide tube for forming a route in the vertebra along the transpedicular trajectory rii on the ipsilateral side, installed coaxially with the planned implantation trajectory at the design stage of the guide.
The study showed that an additional support platform for the spinous process and an additional hollow guide tube for implanting screws along the classical transpedicular trajectory on the ipsilateral side of the vertebra both fix the guide and prevent displacement in the axial and sagittal planes.
The essence of the utility model is illustrated in Fig., which shows a general view of the device.
Individual navigation guide, which is a monolithic structure 1, contains two
The
The design of an individual guide is carried out on the basis of a virtual individual 3D model in the .stl format, obtained from the data of multislice computed tomography of a particular patient.
At the design stage, the optimal reference points of the future guide on the surface of the vertebra are determined, as well as the planned trajectory of screw implantation. A simple mesh (cube or cylinder) is placed on top of the spinous process and positioned to contain part of the spinous process. Then, using the boolean operation "subtraction", the volume occupied by the spinous process is subtracted from the mesh and an identical relief is formed. Then the “uniform thickening” tool with an indicator of -0.5 is applied to the resulting relief. The finished elements of the guide (support pads, guide sheaths and frame) are combined using the boolean operation “combine”.
из биосовместимого материала, который подвергается стерилизации (PLА, фотополимерные смолы, титан и др.)After designing, the guide is printed using additive technologies (FDM, SLA, SLS, etc.) from a biocompatible material that is subjected to sterilization (PLA, photopolymer resins, titanium, etc.)
The device is used as follows.
During the operation, after careful skeletonization of the posterior structures of the vertebra, the navigation guide 1 is applied to the vertebra with

An individual navigation guide for screw implantation in the lumbar spine from a unilateral approach was used in 2 cases. In all cases, there was a reliable fixation of the guide, without displacement in the axial and sagittal planes.
