Abstract
Description
The invention relates to medicine, mainly to cardiology, and can be used to prevent ischemic and postischemic heart damage in humans. It is known that cardiovascular diseases are in first place among the causes of mortality, with the main cause of mortality of cardiological patients coronary heart disease (CHD) and its complications.
Known drug thymogen used in the clinic as an immunomodulator. It does not affect the immunity of healthy individuals, does not cause toxic reactions in a dose 10,000 times higher than the therapeutic, does not have chronic toxicity. During its experimental and clinical study, no side effects were detected (Timogen / recommendations of the Pharmacological Committee of the Ministry of Health of the USSR for medical use dated January 13, 1989 /. L. 1989, p.10). The authors studied the anti-ischemic properties of the drug, allowing it to be used for a new purpose.
Currently, there are many different drugs used for the prevention and treatment of coronary heart disease.
However, they all have a significant amount of side effects.
The purpose of the invention is the prevention of complications and consequences of ischemia and reperfusion of the heart without side effects.
To achieve this goal, the drug thymogen was used in a dose of 7.15 μg / kg. This dose was most effective in treating cardiac arrhythmias, and, as you know, arrhythmias are one of the most formidable complications of coronary heart disease.
1-6. The invention is illustrated in FIG. 1-6.
Example 1. Control series of 8 animals.
Hears D.Y et al. Protection of the ischemic myocardium; cardioplegia.-N.Y, Raien Press. -1981. P. 47-49).The rat was killed under light ether anesthesia. Heart perfusion was performed through the aorta according to the Langendorff method with a solution of the following composition: 150 mM NaCl, 5 mM KCl, 2 mM NaHCO 3 , 1 mM MgCl 2 , 2.5 mM CaCl 2 , 2 mM Tris-HCl (pH 7.4), 11 mM glucose. The solution was saturated with oxygen and heated to 37 ° C. To simulate ischemia and reperfusion, the stock solution was turned off for 20 minutes and then the heart was perfused with normal solution Hears DY et al. Protection of the ischemic myocardium; cardioplegia.-NY, Raien Press. -1981. P. 47-49).
(NADH+NADPH). The degree of ischemic and reperfusion damage to the heart was assessed by the development of ischemic contracture (for this purpose, diastolic pressure in the left ventricle was recorded), during the reperfusion period by the development of reperfusion contracture, restoration of the amplitude of contractions (considering such an amplitude as not less than 50% of the initial one), output of myoglobin, adenine nucleotides (adenosine + inosine + hypoxanthia), pyridine nucleotides (NADH + NADPH).
2. In the control, reperfusion arrhythmias were observed in 100% of cases, including ventricular fibrillation in 37.5%. The amplitude of heart contractions was restored in 12.5% (Fig. 1). A general picture of the dynamics of the amplitude of contractions is shown in FIG. 2.
Example 2. A series with the thymogen of 8 animals.
The experimental conditions differed in that 5 minutes before conducting ischemia, the hearts were perfused with a solution containing thymogen at a concentration of 0.3 μM, which corresponds to a dose of 7.15 μg / kg.
On the model of ischemia and reperfusion, the drug significantly reduced the number of reperfusion arrhythmias (from 100% to 16.6%), completely prevented the development of ventricular fibrillation (Fig. 1); helped restore the strength of myocardial contractions (in the control 12.5% in the
Example 3. A series of
N.Y. 1981, P. 73-79). The experimental conditions differed from the control in that 5 minutes before the reproduction of ischemia, the hearts were perfused with a solution containing 0.2 μM verapamil (Hears D.Y. et al. Protection of ischemic myocardium: cardioplegia. N.Y. 1981, P. 73-79).
2). Reperfusion arrhythmias were observed in 87.5% of cases; in the remaining 12.5%, the heart did not restore contractile activity. Verapamil completely prevented fibrillation. Of the developed arrhythmias, 12.5% ended with asystole, i.e., the total number of hearts with a zero amplitude of contractions was 25% (Fig. 1). The amplitude of contractions was not restored to the initial level (Fig. 2).
3). Verapamil reduced the degree of ischemic contracture, but had little effect on reperfusion (Fig. 3).
The drug reduced the yield of pyridine nucleotides (Fig. 5), but increased the adenyl output (Fig. 4) and significantly increased the yield of myoglobin (Fig. 6) compared with the control, i.e. he increased the depth of cytolysis.
Thus, thymogen limited the severity of the complications and consequences of induced ischemia and reperfusion of the heart, making it more effective than verapamil.
Using this method can prevent arrhythmias without the development of side effects.
