Description
The useful model relates to the field of medicine, more precisely to orthodontics, and can be used to treat pathologies of the temporomandibular joint.
An occlusal splint is an orthodontic appliance used in the treatment of pathologies of the temporomandibular joint (TMJ), the so-called splint therapy. An occlusal splint is a mouthguard made of a biocompatible rigid material or a combination of biocompatible materials, which is installed on the upper or lower dentition. The surface of the occlusal splint can be both flat and with a certain geometry, due to the peculiarities of the performed splint therapy in each individual clinical case.
The patient wears the occlusal splint all the time, removing it only to brush the teeth, eating is also done in the occlusal splint, the patient also sleeps in the occlusal splint. The duration of splint therapy can be from 3 months or more - in each clinical case in different ways.
To support the occlusal splint on the teeth, fasteners are used, or due to frictional forces, when the occlusal splint is made as a casting according to the dentition model. Such options are proposed, for example, in US patent 20170319367 (prior 26.07.2017). The disadvantage of using fasteners is the inconvenience associated with the process of removing and putting on the occlusal splint, for example, for cleaning teeth.
The occlusal splint, made as a casting according to the model of the dentition, is held in place by frictional forces, but during use it loses its shape, wears out and loses its ability to stay on the teeth much faster than the total duration of split therapy.
Most often, clasps are used to fix the occlusal splint, which are a metal wire installed in the body of the occlusal splint and entering the interdental spaces from the outer or inner side of the teeth. When the occlusal splint wears and moves on the teeth, the clasps are compressed for better fixation in the interdental spaces. The clasps are visible, which impairs the aesthetics, and also require regular visits to the doctor by the patient.
The closest to the claimed device in technical essence and the achieved result is the Korean patent KR20170106750 (prior 14.03.2016) "Mouthguard with improved retention on the teeth and the method of its manufacture", which describes a mouthguard equipped with a frame forming a pocket for receiving mouthguards ... The frame is provided with a retaining member for increasing the retention capacity in the attachment state of the mouthguard by wrapping the posterior teeth located on the outermost side along the dental arch. In this case, the holding element is a metal wire with a shape memory, which can be transformed into a shape to apply a clamping force to the posterior tooth, which requires a holding force due to the temperature in the oral cavity during wear.
The disadvantage of the occlusal splint proposed in the patent KR20170106750 is the complexity and high cost of manufacture associated with the need to form a frame from a shape memory wire, corresponding to the shape of the surface of the teeth on which the occlusal splint is attached.
The technical result, which is achieved in the claimed utility model, is the creation of an occlusal splint attachment structure that allows, at minimal cost, to install an occlusal splint on the teeth with high adhesive properties for wearing throughout the entire duration of the split therapy.
The technical result is achieved by the fact that the structure of fastening an occlusal splint on the teeth for the treatment of pathologies of the temporomandibular joint includes an occlusal splint consisting of a bite part and a burr part. The burr is designed to secure the occlusal splint on the teeth. In this case, the surface of the burr part adjacent to the teeth is provided with recesses of a predominantly spherical shape, made of the same shape and locations with the attachments made of composite material formed on the teeth.
Also, the burl can be made of a translucent material with no adhesion to the composite material from which the attachments are made.
The production of an occlusal splint from two parts, a bite and a burr, allows the use of different materials for each part, making the burl thinner and more elastic. This allows the patient to easily remove the occlusal splint on their own without additional effort, for example, for cleaning teeth.
Recesses made on the surface of the burr part adjacent to the teeth allow the insertion of composite attachments fixed to the teeth. Thus, a tongue-and-groove locking connection is obtained, which allows you to firmly hold the occlusal splint during the entire splint therapy process.
The fact that the recesses are made predominantly of a spherical shape allows the patient to easily remove and put on the occlusal splint independently, avoiding snagging, without damaging the inner surface of the burr part.
The execution of the burr part from a translucent material with no adhesion to the composite material from which the attachments are made allows the physician to place and fix the attachments directly when installing the occlusal splint on the patient's teeth.
In the following, the claimed technical solution is illustrated by a detailed description of a specific, but not limiting present solution, an example of its implementation and the attached drawings, in which:

The
3.In the proposed in the present utility model, the construction of fastening an occlusal splint 1 with high adhesive properties for wearing throughout the entire time of the split therapy,
The
It should be noted that there are at least two ways to create an attachment structure for the occlusal splint 1 with high adhesive properties for the treatment of pathologies of the temporomandibular joint proposed in the present utility model.
2.According to one of them, it is possible to first apply
2.According to another method of creating an attachment structure for an occlusal splint 1, intraoral scanning or impressions of the patient's jaws are performed, on the basis of which
2.After that, the
For the functional part of the occlusal splint 1, the bite part 4, made, for example, of special plastic, is responsible. Known methods can be used to produce the bite portion 4. For example, a cylindrical blank made of orthodontic plastic is applied to the manufactured burl, it is formed by hand, polymerized, and all excess material is removed by grinding. There are other known methods, therefore, in the description in detail, the manufacturing process of the bite part 4 is not given.
2.In the future, when using the occlusal splint 1 by the patient, he can independently put on her teeth 3. When putting on the occlusal splint 1 “snaps into place” on the
In addition to reliable fixation on the teeth 3,
Thus, the proposed utility model makes it possible to achieve the claimed technical result, namely, to create an attachment structure for the occlusal splint 1, which allows, at minimal cost, to install on the teeth 3 an occlusal splint 1 with high adhesive properties for wearing throughout the entire duration of the split therapy.
It should also be noted that the proposed utility model is more aesthetically pleasing and does not require additional manipulations by the doctor during work.
