Abstract
Description
The invention relates to dentistry and can be used to monitor the patient’s adaptive state to dentures.
Dental interventions in the form of the introduction of dental and dentofacial prostheses, orthopedic devices, diagnostic and treatment devices, etc. cause discomfort in patients due to adaptation processes. Monitoring the adaptation process allows you to evaluate the correctness of the constructive decision of the dentist and take timely measures to exclude the pathological consequences of dental intervention.
To date, adaptation has been evaluated by the subjective sensations of the patient, which did not always correspond to the true state, because various psychoemotional factors are superimposed on this assessment.
There is an objective way to assess the neuromuscular system is to control biopotentials. Biopotentials accompany all major life processes and are universal indicators of the functional state of any tissues and organs. Even minor disturbances cause changes in their electrical activity.
In dentistry, the monitoring of the biopotentials of the masticatory muscles through
removal of electromyographic impulses (EMG). A similar procedure is described in an article by V.V. Parilova. V.V. Ponomareva, L.S. Persina, published in the journal "Dentistry" 1986, vol. 65 N 2, p. 80 82. The article presents the results of a study of the bioelectric activity of the masticatory muscles, but there is no consideration of the results of such studies to evaluate the adaptation process.
The objective of the proposed solution is the development of a method for an objective assessment of the state of the adaptation process to dental intervention.
To solve the problem in a method for determining the adaptation process, which includes the registration of electromyographic impulses (EMG) from the masticatory muscles during chewing. EMG pulses are recorded before and after dental intervention. The data obtained are compared and judged on the state of the adaptation process by the intensity of the appearance of pulses during chewing for equal periods of time. If the intensity of the pulses is equal before and after the dental intervention, good adaptation is judged, with a lower intensity of pulses, the poor state of adaptation is judged.
The introduction of new operations makes it possible to more reliably judge the quality of dental devices on the basis of objective evidence, to make corrective amendments to their design at the time, to ensure patient comfort and to exclude complications from the use of incorrectly selected designs.
For electromyographic examination using a four-channel electromyograph "Medicor", Hungary. At the same time, the patient should sit in a comfortable chair at rest. Standard electrodes are attached to the patient's chewing muscles, for example, using an adhesive plaster. Then the patient is asked to close his teeth as much as possible by volitional force, while the maximum value of EMG pulses is fixed.
The device is configured to register the amplitude of the pulses equal to or greater than the previously obtained value. This is done in order to facilitate the doctor's analysis of the intensity of EMG pulses. The analysis will be reduced to counting only the maximum pulses.
The patient is offered to chew, for example, 1/4 of the walnut kernel. In this case, the instrument records the frequency of occurrence and the number of maximum pulses, as well as the time during which they were recorded. Then a dental intervention was performed and EMG impulses were again measured during chewing of a nut for a given period of time. The results are compared. Repeated examination is repeated, for example, every week. If the number of pulses after a dental intervention is equal to or gradually from week to week approaches the number of pulses before a dental intervention, then a good adaptation and the correctness of the chosen solution are judged. If the number of pulses after the dental intervention is less and does not increase over time to the value recorded before the start of treatment, then poor adaptation is judged and the treatment is changed.
The method is simple, efficient and harmless to the patient. To implement the method, the equipment that most clinics have is used. The method gives objective indications that exclude subjective reactions of both the patient and the doctor.
Example 1. Patient B. was examined during prosthetics. Before prosthetics, when chewing a walnut kernel, 12 pulses were recorded for 30 s. Immediately after prosthetics, 6 pulses, after a week, 10 pulses, and after 2 weeks, 15 pulses, which indicates a good adaptation to prostheses.
Example 2. Patient T. was examined for adaptation to new dentures. Before prosthetics, according to the above methodology, 14 pulses were recorded in 30 s, after prosthetics 4 pulses were observed at the same time, and after 6 weeks, 6 pulses. Upon further examination, the number of pulses did not exceed 6, which indicates poor adaptation.
The proposed method allowed to make more reliable the results of studies of the adaptive state during dental intervention, since the results are based on objective indications of devices. Research data allow timely amendments to the design of dentures or the procedure for dental treatment. This speeds up the treatment process, allows you to achieve better results in treatment and to exclude complications from erroneous interventions.
