Abstract
Description
The invention relates to medicine, namely to psychiatry.
97 101). A known method for predicting the effectiveness of plasmapheresis in endogenous psychoses with effective disorders by clinical research (Social and clinical psychiatry, 1993, N 1, p. 97 101).
The disadvantage of this method is the subjectivity of the clinical assessment, the dependence on the skill level of the doctor, as well as the duration of observation of 2-3 plasmapheresis operations.
The purpose of the invention to improve the accuracy of the forecast.
This goal is achieved by the fact that additionally investigate the level of molecules of the average mass of blood plasma and after the first operation of plasmapheresis with a decrease in the level of molecules of average mass of at least 10% predict the positive effect of plasmapheresis.
The method is as follows. Before a plasmapheresis operation, a patient with endogenous psychosis with affective disorders is examined the level of molecules of the average mass of blood plasma by the method of V.V. Nikolaychik and co-authors (Laboratory work, 1991, N 10, p. 13 18). After the operation, plasmapheresis re-examine the level of plasma molecules of medium weight. With a decrease of at least 10%, a positive effect of plasmapheresis is predicted.
р. Example. Patient G., born in 1967 medical history N 871. Diagnosis: schizophrenia, paranoid form. Mentally ill since 1989, when the ideas of persecution appeared, voices sounding inside the head of a menacing and imperative nature, accompanied by fear, anxiety, internal tension. Repeatedly hospitalized in psychiatric hospitals. 04/26/93 entered the Moscow Research Institute of Psychiatry of the Ministry of Health of the Russian Federation, intensive care unit, with complaints of fear, anxiety, discomfort in the head. He expressed the ideas of persecution, experienced auditory deceptions of perception in the form of voices. From the moment of admission, the patient was prescribed therapy with haloperidol 30 mg per day, amitriptyline 150 mg per day, cyclodol 6 mg per day. The therapy was carried out for 2 weeks and had no effect. 05/12/93 a patient undergoing ongoing treatment underwent the 1st operation of plasmapheresis with the extraction of 1100 ml of plasma. Before plasmapheresis, the level of medium-weight molecules was determined (05/11/93). He amounted to 0.334 units. optical density. After plasmapheresis, the level of medium-weight molecules decreased to 0.167 units. optical density. In the patient's condition, there was a significant positive dynamics. The mood improved, delusional and hallucinatory experiences were reduced, and behavior was streamlined. In a state of remission, against the background of ongoing psychopharmacotherapy, he was discharged home. Catamnesis. Over the next 3 months, the patient's condition is satisfactory.
Thus, in this case, there is a coincidence of the predicted and real results of treatment to reduce the level of molecules of average weight.
32,8% The specified method was applied to 18 patients with endogenous psychoses with affective disorders. The coincidence of the predicted and real results occurred in 16 of 18 patients (89%). In 14 patients with a positive therapeutic result, the use of plasmapheresis was accompanied by a decrease in the level of molecules of average weight from 10.6 to 58.5% compared with the initial data. In one patient with a lack of positive treatment results, the average molecular weight level increased by 32.8%
