Известен способ лечения доброкачественной гиперплазии предстательной железы, состоящий в том, что перед хирургическим лечением доброкачественной гиперплазии предстательной железы проводят УЗИ исследования ректальным датчиком и определяют размеры аденоматозных узлов. Далее в надлобковой области послойно вскрывают ткани до мочевого пузыря, а потом вскрывают мочевой пузырь. После этого с помощью специального устройства проводят отделение аденоматозных узлов от уретры, а затем хирургические манипуляции по удалению аденоматозных узлов из полости мочевого пузыря. Ложе аденомы осушивают марлевыми салфетками, а затем проводят все последующие действия хирургической операции (авт.св. СССР N 1572565). Сущность устройства для отделения аденоматозных узлов от уретры состоит в том, что оно содержит полый цилиндрический нож с возможностью его осевого перемещения относительно направителя, причем нож с тыльной стороны выполнен с пазами и установлен на оси, передняя часть которой сконструирована в виде ловителя, а к задней жестко прикреплена рукоятка с подвижным рычагом, нижняя часть которого сконструирована в виде поводка, входящего в паз ножа. Между рукояткой и рычагом установлена плоская пружина. Далее проводят хирургические манипуляции по удалению аденоматозных узлов из полости мочевого пузыря. Осушивают марлевой салфеткой ложе аденомы, а затем кратковременно от 1 до 2 мин тампонируют ложе кровоостанавливающими средствами. После этого расфокусированным лучом лазерной установки "Янтарь", основанный на парах меди, проводят облучение ложа аденомы. В результате внутрисосудистой коагуляции в поврежденных сосудах образуется тромб протяженностью 1-2 мм, прочно сваренный со стенками сосуда. Функциональные клетки при этом не повреждаются. Вместе с тем происходит процесс лазероаблации. Как показали клинические испытания, наиболее оптимальным облучением являются лазерные лучи длиной 0,51-0,58 мкм плотностью мощности 0,04-0,6 Вт/см2 и временем экспозиции всей поверхности ложа аденомы 6-8 мин. Кратковременное тампонирование ложа аденомы кровоостанавливающими средствами, такими, например, как эпсило-аминокапроновая или транексамическая кислоты усиливает эффект коагуляции. При этом изобретение является технически простым и легко выполнимым, доступным хирургу-урологу различной квалификации. Кроме этого, описанный способ лечения формирует защитный биологический барьер в ложе аденомы,что является профилактикой мочевой инфильтрации. При использовании способа полностью исключается некроз тканей и отсутствует воздействие инфицированной мочи на раневую поверхность.
