Формирование неотимпанальной мембраны при реконструктивной слухоулучшающей операции является одним из основных этапов, непосредственно влияющих на результаты операции. При реконструктивной слухоулучшающей операции, проводимой больным, перенесшим общеполостную радикальную операцию, неотимпанальная мембрана (барабанная перепонка) в настоящее время формируется так же, как и при перфоративных отитах, т.е. с использованием преимущественно фасции, а укладку трансплантата производят по одной из методик: на деэпителизированную наружную сторону барабанной перепонки или ее остатков; между эпидермальным и фиброзным слоями барабанной перепонки; на внутреннюю поверхность барабанной перепонки, при этом фасциальный лоскут укладывается так, чтобы на 1-2 мм был больше перфорации и прикрывал ее. Однако больным, перенесшим общеполостную радикальную операцию при формировании неотимпанальной мембраны, технические приемы, используемые для мирингопластики при перфоративных отитах мало эффективны. Это связано с тем, что у больных, перенесших радикальную операцию, как правило нет барабанной перепонки, а тимпанальное фиброзное кольцо сохраняется только в переднем и нижнем отделах, в этих же местах иногда сохраняются остатки барабанной перепонки; отсутствует задняя стенка наружного слухового прохода; рубцово измененные ткани барабанной и трепанационной полостей имеют недостаточное кровоснабжение, что ухудшает условия приживления неотимпанального лоскута. Именно поэтому предпочитают формировать малую тампанальную полость, укладывая неотимпанальный лоскут над каналом лицевого нерва. Данная методика так же имеет свои недостатки, так как возможны приращения лоскута к медиальной стенке с последующей облитерацией вновь созданной полости. Наиболее частой причиной неудач формирования неотимпанальной мембраны при реконструктивных слухоулучшающих операциях являются неполное приращение лоскута и образование в лоскуте перфорации; западение лоскута с последующим приращением к медиальной стенке и облитерацией вновь созданной барабанной полости. Повышение результатов формирования неотимпанальной мембраны, создание условий для лучшего приживления трансплантанта при реконструктивной слухоулучшающей операции после общеполостной радикальной операции остается актуальной проблемой. В качестве прототипа взят способ формирования неотимпанальной мембраны, при котором фасциальный трансплантант укладывается в передних отделах на деэпителизированный фиброзный слой остатков барабанной перепонки, а в задних отделах (в зависимости от варианта проводимой реконструктивной операции трансплантат укладывается на вновь сформированную (кортикальная кость, хрящ) заднюю стенку слухового прохода или покрывает хрящ, уложенный над каналом лицевого нерва. Как указывают авторы, необходимые местные условия для проведения этой операции, особенно по первому варианту встречаются редко. У больных, перенесших общеполостную радикальную операцию, барабанная перепонка часто отсутствует полностью, рубцовые изменения тканей барабанной и трепанационной полостей нарушают кровоснабжение, что ухудшает условия приживления неотимпа- нального лоскута. При формировании барабанной перепонки над каналом лицевого нерва возможны приращения лоскута к медиальной стенке с последующей облитерацией вновь созданной полости. Трудности, возникающие при формировании неотимпанальной мембраны указывают, что обычно технические примы мирингопластика, используемые как правило при перфоративных отитах при реконструктивных слухоулучшающих операциях недостаточны и требуют совершенствования. In reconstructive auditory-improving surgery performed by a patient who underwent general cavity radical surgery, the neotympanic membrane (tympanic membrane) is currently formed in the same way as with perforated otitis media, i.e. using mainly the fascia, and the graft is laid according to one of the methods: on the de-epithelized outer side of the eardrum or its remains; between the epidermal and fibrous layers of the tympanic membrane; on the inner surface of the eardrum, while the fascial flap is laid so that 1-2 mm was more perforation and covered it. However, in patients who underwent general cavity radical surgery during the formation of a non-tympanic membrane, the techniques used for myringoplasty for perforated otitis media are not very effective. This is due to the fact that in patients who underwent radical surgery, as a rule, there is no tympanic membrane, and the tympanic fibrous ring is preserved only in the anterior and lower sections, the remains of the tympanic membrane are sometimes preserved in these places; there is no back wall of the external auditory meatus; cicatricially altered tissues of the tympanic and trepan cavities have insufficient blood supply, which worsens the engraftment conditions of the neotimpanal flap. That is why they prefer to form a small tampanal cavity by laying a neotimpanal flap over the channel of the facial nerve. This technique also has its drawbacks, since flap increments to the medial wall are possible with subsequent obliteration of the newly created cavity. The most common cause of failure to form a non-tympanic membrane during reconstructive hearing-improving operations is incomplete flap increment and formation of perforation in the flap; retraction of the flap, followed by an increment to the medial wall and obliteration of the newly created tympanic cavity. Improving the results of the formation of a non-tympanic membrane, creating conditions for better engraftment of the transplant during reconstructive hearing-improving surgery after general cavity radical surgery remains an urgent problem. As a prototype, the method of forming a non-tympanic membrane is taken, in which the fascial graft is placed in the front sections on the de-epithelialized fibrous layer of the remnants of the tympanic membrane, and in the posterior sections (depending on the type of reconstructive surgery performed, the graft is placed on the newly formed (cortical bone, cartilage) posterior wall of the ear canal or covers the cartilage laid over the canal of the facial nerve.As the authors indicate, the necessary local conditions for this opera , especially in the first variant, are rare.In patients who underwent general cavity radical surgery, the eardrum is often completely absent, cicatricial changes in the tissues of the tympanic and trepanal cavities disrupt the blood supply, which worsens the healing conditions of the neotampal flap.When the tympanic membrane is formed above the canal of the facial nerve increments of the flap to the medial wall are possible, followed by obliteration of the newly created cavity. Difficulties arising in the formation of a non-tympanic membrane indicate that usually the technical priming of myringoplasty, used as a rule for perforated otitis media during reconstructive hearing-improving operations, is insufficient and requires improvement.

Данный способ осуществляется следующим способом. Под общим обезболиванием через заушный разрез вскрывается трепанационная и барабанная полости, тщательно отсепаровывается эпидермальная выстилка и при наличии патологически измененных тканей последние удаляются. Открывается барабанное устье евстахиевой трубы (часто закрытое соединительнотканной мембраной) и проверяется проходимость трубы. Все участки метапла- зированной ткани выстилки барабанной полости удаляются. В зависимости от наличия или отсутствия стремени проводится один из вариантов оссикулопластики stapes to drum или plate to drum. Показания к формированию неотимпанальной мембраны по предлагаемому способу аналогичны показаниям для реконструктивной слухоулучшающей операции, выполняемой в полном объеме (местоидопластика, оссикулопластика, мирингопластика) и сводятся к отсутствию острого воспалительного процесса, подвижности структур окон лабиринта, сохранности улитковой и тубарной функций, наличие слизистой оболочки в барабанной полости (хотя бы в устье слуховой трубы). Наличие остатков барабанной перепонки в передних отделах облегчает формирование неотимпанальной мембраны, однако их отсутствие не является противопоказанием к операции, так как у больных, перенесших радикальную операцию в передне-нижних отделах, как правило сохраняется фиброзное кольцо или его остатки. The presence of remnants of the tympanic membrane in the anterior sections facilitates the formation of a non-tympanic membrane, however, their absence is not a contraindication to surgery, since patients who undergo radical surgery in the anterior lower sections usually retain a fibrous ring or its remnants.
При подготовке ложа для неотимпанального лоскута эпидермис отсепаровывается наличие от тимпанального фиброзного или костного кольца и затем кожа верхней, передней и нижней стенок слухового прохода на протяжении 1 см кнаружи циркулярно. Трансплантат готовится в процессе операции из фасции височной мышцы размером 6-8 см2. После проведения мастоидопластики с формированием задней стенки слухового прохода кортикальной костью и одного из видов оссикулопластики фасциальный лоскут предварительно истонченный и подсушенный укладывается на обнаженную кость верхней, передней, нижней стенок слухового прохода от фасциального или костного кольца на глубину 1 см (под кожу), а в задних отделах лоскут полностью покрывает вновь созданную заднюю стенку слухового прохода, после чего предварительно отсепарованная кожа слухового прохода укладывается поверх фасции на место. Натянутая часть лоскута (собственно новая барабанная перепонка) укладывается на шляпку протеза, что предупреждает ее от западания. По предложенному способу оперировано 46 больных после перенесенной ранее радикальной операции. Послеоперационный период протекал без осложнений. Практически у всех больных в послеоперационном периоде отмечались признаки болезни лоскута различной степени. Первичное приживление трансплантата было во всех случаях, однако у 4 больных возникли небольшие перфорации в передних или верхних отделах, которые были связаны с дисфункцией слуховой трубы. Удовлетворительный результат был получен у 91,3% больных при исследовании через 6 месяцев и 1 год. The proposed method operated on 46 patients after undergoing a previous radical operation. Использование предлагаемого способа позволяет повысить приживляемость неотимпанального лоскута, улучшить результаты мирингопластики и в целом реконструктивной слухоулучшающей операции.
