Abstract
Description
The invention relates to medicine, more specifically to maxillofacial surgery, and can find application in surgical dentistry for osteotomy of the upper jaw.
It is known that the fixation of the jaws after an osteotomy is difficult in some cases, it is most difficult to do this when combining the deformation of the upper jaw with a cleft. Micrognathia of the upper jaw requires a multi-stage treatment consisting of orthopedic and surgical treatment. Micrognathia is eliminated by orthodontic treatment with preliminary compact osteotomy. Treatment is carried out for 2-3 years. At the end of orthodontic treatment, an osteotomy of the upper jaw is performed for retrognathy.
To fix the jaws in the right position in this category of patients using special tooth devices. These devices consist of orthodontic crowns welded to a support beam, to which hooks for intermaxillary traction are welded. These devices allow reliable fixation of ostomy fragments in the desired position after extension of the upper jaw in both early and late postoperative periods. However, the devices used do not allow to breed fragments of the upper jaw, which must be done with micrognathia and retrognathia of the upper jaw, combined with a cleft, since they are a single device.
The aim of the invention is to provide the possibility of extension and extension of the maxillary bones in osteotomy without prior orthodontic treatment and their rigid fixation in the postoperative period.
This goal is achieved in that the device for fixing the maxillary bones, containing rigidly welded metal crowns with an arc welded to them with hook loops for intermaxillary traction, has a free medial end, which is made in the form of bends.
For the treatment of this group of patients, it is necessary to fix two devices on the maxillary bones, since osteotomy, and then extension and extension, is performed on the jaw bones sequentially, i.e. first on one maxillary bone, and then on the other. Osteotomy of the upper jaw for patients with micro- and retrognatia, combined with a cleft, is carried out only after prolonged orthodontic treatment, since the devices used are fixed simultaneously on both maxillary bones and during surgery only extension is possible. The proposed device allows you to push the maxillary bones without prior orthodontic treatment, since before surgery it is fixed on one of the maxillary bones, and after osteotomy, when the maxillary bones are divorced and extended, a similar device is applied to the other maxillary bone. Then they are fastened together, which allows for reliable fixation of the maxillary bones. In the late postoperative period, the proposed device is used as stabilizing.
Figure 1 shows the device after surgery, a General view; figure 2 is the stage of application of the device to one of the maxillary bones; figure 3 is a stage when the device is fixed on both jaws.
5. Device I consists of
Fixation of fragments of the upper jaw using the claimed device is as follows. In the preoperative period,
