Claims (28)

1. Протез (1) для остеосинтеза сегмента кости, в частности плечевой кости (О), содержащий по меньшей мере один закрепляющий элемент (2), имеющий внутреннюю полость (24), опорные средства (3), соединяемые с костной тканью сегмента плечевой кости (О), и средства (4) позиционирования опорных средств (3) относительно указанного закрепляющего элемента (2), содержащие один или более резьбовых винтов (41), причем указанный по меньшей мере один закрепляющий элемент (2) имеет форму многогранника с двумя треугольными основаниями (22),
отличающийся тем, что указанный по меньшей мере один закрепляющий элемент (2) имеет одно или более выполненных на поверхности проходов или отверстий (21), сообщающихся с указанной полостью (24), и
указанные опорные средства (3) содержат пластину (31), имеющую по существу Y-образную форму или форму крыльев летучей мыши, с двумя верхними концами, имеющую резьбовые отверстия (312) и выполненную с возможностью расположения на боковой поверхности указанного костного сегмента (О),
причем указанный один или более резьбовых винтов (41) выполнены с возможностью их размещения в отверстиях (312) указанной пластины (31) и взаимодействия с указанными отверстиями (312) с помощью соответствующей резьбы или крепления, причем по меньшей мере один из указанных резьбовых винтов (41) при вводе в указанную внутреннюю полость (24) вставляется через указанные проходные отверстия (21), при этом пластина (31) содержит один или более закрепляющих крючков (311), расположенных на каждом из указанных верхних концов пластины.
2. Протез по п. 1, отличающийся тем, что основания (22) имеют форму равнобедренного треугольника с углом (α) при вершине и парой равных углов (β), причем указанные основания (22) имеют овальные отверстия (21''), причем указанный многогранник имеет два боковых ребра (23'), соответствующих указанным равным углам (β) оснований (22), ребро (23'') при вершине, которое соответствует указанному углу (α) при вершине и длина которого меньше длины указанных двух боковых ребер (23'), две боковые поверхности (25), каждая из которых ограничена указанным ребром (23'') при вершине и соответствующим боковым ребром (23') и имеет по меньшей мере одно проходное отверстие (21') предпочтительно овальной формы, и боковую поверхность (26), соответствующую стороне основания (22), противолежащей указанному углу (α) при вершине, ограниченную указанными боковыми ребрами (23') и являющуюся открытой.
3. Протез по п. 1 или 2, отличающийся тем, что основания (22) являются одинаковыми и/или указанный угол (α) при вершине превышает 90°.
4. Протез по любому пп. 1-3, отличающийся тем, что закрепляющий элемент (2) выполнен из титана, и/или рассасывающегося материала, и/или инертного материала, и/или ткани, выращенной из обработанной кости.
5. Протез по любому из пп. 1-4, отличающийся тем, что указанная пластина (31) выполнена из деформируемого материала, например из пластичного титана или аналогичного материала, так что она может быть подогнана к поверхности костного сегмента (О) даже на этапе установки.
6. Протез по любому из пп. 1-4, отличающийся тем, что указанные опорные средства (3) содержат
стержень (32), вставляемый в костный сегмент (О) для расположения с помощью закрепляющего элемента (2) внутри полости (24) и имеющий одну или более канавок (321), выполненных на его боковой поверхности, и фиксирующее резьбовое отверстие (322), расположенное на одном конце стержня, и
съемную фиксирующую конструкцию (33), содержащую элемент в виде перевернутой буквы L, имеющий первую часть и вторую часть, расположенные примерно под углом 90°, причем указанная первая часть имеет боковые направляющие сквозные отверстия (331) и верхнее сквозное отверстие (332), при этом указанная фиксирующая конструкция (33) выполнена с возможностью ее расположения таким образом, что указанная первая часть расположена параллельно указанному сегменту кости,
причем указанные средства (4) фиксации содержат один или более резьбовых винтов (41), вставляемых через направляющие боковые отверстия (331), так что указанные винты соответствующим образом ориентированы в указанных канавках (321), а также дополнительный винт (41), вставляемый в указанное верхнее отверстие (332) с обеспечением взаимодействия с указанным отверстием (322) стержня (32).
7. Протез по любому из пп. 1-4, отличающийся тем, что указанные опорные средства (3) содержат по меньшей мере одну жесткую направляющую (34, 34'), имеющую первый и второй конец и вставляемую через боковую поверхность указанного костного сегмента (О), и фланец (343), закрепленный на указанном первом конце, при этом указанная по меньшей мере одна жесткая направляющая (34, 34') может быть вставлена в костную ткань таким образом, что указанный второй конец вводится по меньшей мере через одно из указанных проходных отверстий (21) закрепляющего элемента (2), так что по меньшей мере одна часть указанной по меньшей мере одной жесткой направляющей (34, 34') расположена в указанной внутренней полости (24),
причем указанные средства (4) позиционирования содержат по меньшей мере один резьбовой винт (41), выполненный с возможностью взаимодействия с помощью резьбы с указанным фланцем (343) и с костной тканью костного сегмента (О).
8. Протез по п. 7, отличающийся тем, что указанная по меньшей мере одна жесткая направляющая (34, 34') имеет изогнутую форму.
9. Протез по п. 7 или 8, отличающийся тем, что указанная по меньшей мере одна жесткая направляющая (34, 34') имеет внутренний канал, выполненный с впускным отверстием (341) и выпускным отверстием (342).
10. Протез по п. 9, отличающийся тем, что указанная по меньшей мере одна жесткая направляющая (34') содержит клапан.
11. Протез по любому из пп. 7-10, отличающийся тем, что указанная жесткая направляющая (34') имеет открытую боковую часть.
12. Протез по любому из пп. 1-4, отличающийся тем, что указанные опорные средства (3) содержат стержень (35), выполненный с возможностью размещения в канале указанного диафизарного сегмента (О) кости и с помощью закрепляющего элемента (2) вводимый и располагаемый внутри полости (24), причем указанные средства (4) позиционирования содержат по меньшей мере один крючок (42), расположенный на одном конце указанного стержня (35) и предназначенный для соединения с закрепляющим элементом (2).
13. Протез по п. 12, отличающийся тем, что указанный по меньшей мере один крючок (42) соединен со стержнем (35) с возможностью разъединения.
14. Протез по п. 12 или 13, отличающийся тем, что он содержит направляющую проволоку, выполненную из металла или деформируемого материала с возможностью ее проведения через указанный стержень (35) и закрепляющий элемент (2) для приближения указанного стержня к указанному элементу.
15. Протез по любому из пп. 1-14, отличающийся тем, что содержит протезную конструкцию (5), которая в свою очередь содержит
дополнительный стержень (51), вставляемый в указанный костный сегмент (О), в частности, внутрь диафиза плечевой кости (О), имеющий верхний крепежный штифт (511) и блокирующую или резьбовую полость (513), а также выполненные на боковой поверхности указанного стержня продольные выступы (512), вставляемые в костную ткань указанного костного сегмента (О),
колпачок или головку (52), выполненную с возможностью размещения на проксимальном эпифизе (Р) указанного костного сегмента (О) и содержащую соединительное кольцо (521), имеющее гнездо (522), регулируемое поворотным образом, и
деталь (53) для соединения указанного дополнительного стержня (51) и указанного колпачка или головки (52), содержащую верхнюю часть (531), выполненную с возможностью соединения с гнездом (522) указанного соединительного кольца (521) колпачка или головки (52), и нижнюю часть (532), ориентированную под углом (у) относительно указанной верхней части
(531) и образующую вместе с указанной частью фигурный профиль, имеющий выемку (533), выполненную с возможностью размещения в ней закрепляющего элемента (2), причем нижний конец указанной нижней части
(532) имеет соединительный элемент (534), выполненный с возможностью размещения в нем и разъемного соединения с ним указанного крепежного штифта (511) указанного стержня (51),
при этом указанные опорные средства (3) с возможностью разъединения соединены с указанной блокирующей или резьбовой полостью (513).
