Claims (1)

Способ повышения точности диагностики и качества лечения патологий височно-нижнечелюстного сустава (ВНЧС), содержащий визуализацию элементов ВНЧС, которую осуществляют методом прицельной линейной томографии в боковой проекции, при этом глубину среза определяют по пальпируемому латеральному краю глазницы, который определяют в качестве цефалометрической точки и используют как индивидуальный анатомический ориентир уровня томографического среза, голову больного укладывают, по отношению к плоскости стола, параллельно своей срединно-сагиттальной плоскости, а плоскость физиологической горизонтали - перпендикулярно, центральный луч направляют на 1,0 см выше и кпереди от наружного слухового прохода, затем производят два снимка: в положении «рот закрыт» и «рот открыт», проводят построение цефалометрических плоскостей на томограммах ВНЧС по краниометрическим точкам: α2 - показатель функциональной активности сустава; α1 - амплитуда движения головки по суставному бугорку; γ1 - показатель дистализации головки в межбугорковом положении зубных рядов; γ2 - показатель степени мобильности головки и стабильности связок при максимальном отведении нижней челюсти; Б; Г1; Г2: О - краниометрические точки на суставном бугорке височной кости - Б, головке мыщелкового отростка нижней челюсти - Г1; Г2 слуховом проходе височной кости - О, определяют по краниометрическим точкам значения следующих показателей: α1 - амплитуда движения головки по суставному бугорку; α2 - показатель функциональной активности сустава; γ1 - показатель дистализации головки в межбугорковом положении зубных рядов; γ2 - показатель степени мобильности головки и стабильности связок при максимальном отведении нижней челюсти; F - коэффициент заполненности суставной ямки головкой; A - коэффициент аппроксимации; k - линейный коэффициент перемещения головки нижней челюсти; E - площадь томографического сечения свободного рентгенологического пространства ВНЧС, сравнивают значения показателей с нормой, по отклонениям от нормы устанавливают характер патологии ВНЧС, ставят диагноз, планируют и назначают лечение, изготавливают зубные протезы в соответствии с полученными угловыми показателями.A method for improving the accuracy of diagnosis and the quality of treatment of pathologies of the temporomandibular joint (TMJ), containing visualization of the elements of the TMJ, which is carried out by the method of direct linear tomography in the lateral projection, while the cut depth is determined by the palpable lateral edge of the eye socket, which is determined as a cephalometric point and used as an individual anatomical landmark of the level of the tomographic slice, the patient’s head is laid, in relation to the plane of the table, parallel to its median the gittal plane, and the physiological horizontal plane is perpendicular, the central beam is directed 1.0 cm above and anterior to the external auditory meatus, then two pictures are taken: in the “mouth closed” and “mouth open” position, cephalometric planes are constructed on the TMJ tomograms by craniometric points: α 2 - an indicator of the functional activity of the joint; α 1 - the amplitude of the movement of the head along the articular tubercle; γ 1 - an indicator of the distalization of the head in the inter-tubercular position of the dentition; γ 2 - an indicator of the degree of mobility of the head and stability of the ligaments at the maximum abduction of the lower jaw; B; G 1 ; G 2 : O - craniometric points on the articular tubercle of the temporal bone - B, the head of the condylar process of the lower jaw - G 1 ; G 2 auditory meatus of the temporal bone - O, determined by craniometric points, the values of the following indicators: α 1 - the amplitude of the movement of the head along the articular tubercle; α 2 - an indicator of the functional activity of the joint; γ 1 - an indicator of the distalization of the head in the inter-tubercular position of the dentition; γ 2 - an indicator of the degree of mobility of the head and stability of the ligaments at the maximum abduction of the lower jaw; F is the fill factor of the articular fossa with the head; A is the approximation coefficient; k is the linear coefficient of movement of the head of the lower jaw; E is the area of the tomographic section of the free X-ray space of the TMJ, the values of the indicators are compared with the norm, the deviations from the norm establish the nature of the pathology of the TMJ, make the diagnosis, plan and prescribe the treatment, make dentures in accordance with the obtained angular indicators.
