Abstract

Способ лечения «трехлодыжечного» перелома, включающий формирование доступа к области переломов и их остеосинтез, отличающийся тем, что во время операции конечность сгибают в коленном суставе до 90° и в тазобедренном до 45°, доступ начинают от точки, расположенной на 1/3-1/4 расстояния кнаружи от наружной лодыжки, до пяточного бугра и продлевают краниально параллельно оси малоберцовой кости, доступ углубляют до малоберцовых мышц и отводят их медиально, репонируют и временно фиксируют фрагменты малоберцовой кости, далее доступ расширяют по заднемедиальной поверхности короткой малоберцовой мышцы и отводят медиально одним комплексом ткани, включая икроножный нерв, малую подкожную вену и длинный сгибатель большого пальца, репонируют и окончательно фиксируют задний край большеберцовой кости, после чего окончательно репонируют и фиксируют малоберцовую кость и синдесмоз, выполняют рентгенографию и в случае остаточного смещения внутренней лодыжки выполняют ее репозицию и остеосинтез.A method of treating a “three-ankle” fracture, including forming access to the fracture area and their osteosynthesis, characterized in that during the operation the limb is bent in the knee joint to 90 ° and in the hip to 45 °, access starts from a point located at 1 / 3- 1/4 of the distance outside the outer ankle to the calcaneal tuber and extend cranially parallel to the axis of the fibula, deepen the access to the fibula and take them medially, repair and temporarily fix fragments of the fibula, then expand the access non-medial surface of the short fibular muscle and medially removed with one complex of tissue, including the gastrocnemius nerve, small saphenous vein and the long flexor of the thumb, they repair and fix the posterior edge of the tibia, and then completely repair and fix the fibula and syndesmosis, and, in case of radiography residual displacement of the inner ankle perform its reposition and osteosynthesis.
