Abstract

Способ удаления юношеской ангиофибромы основания черепа, локализованной в полости носа, носоглотке, клиновидной пазухе, крылонебной ямке и достигающей подвисочной ямки, включающий трансназальный эндоскопический доступ, выполнение антростомии для удаления опухоли из крылонебной ямки, отличающийся тем, что опухоль удаляют из полости носа центрипетально с частичной резекцией нижнего края средней носовой раковины, вскрытием медиальной стенки верхнечелюстной пазухи на уровне естественного соустья при выраженном распространении опухоли в крылонебную и подвисочную ямки, а при незначительном распространении опухоли в крылонебную ямку антростомию выполняют на уровне заднего конца средней носовой раковины, при этом удаление опухоли из просвета клиновидной пазухи осуществляют прямым трансназальным доступом с резекцией верхней носовой раковины и удалением передней стенки пазухи с помощью костных выкусывателей, выделение части опухоли, расположенной в латеральных отделах носоглотки противоположной стороны и от задних отделов перегородки носа, осуществляют бипортально центрипетально через обе половины полости носа, сохраняя задние отделы перегородки носа.A method for removing juvenile angiofibroma of the base of the skull localized in the nasal cavity, nasopharynx, sphenoid sinus, pterygopalatine fossa and reaching the infratemporal fossa, including transnasal endoscopic access, performing an anthrostomy to remove the tumor from the pterygopalatine fossa, characterized in that the tumor is removed from the nasal cavity from the center resection of the lower edge of the middle turbinate, opening the medial wall of the maxillary sinus at the level of the natural anastomosis with a pronounced tumor spread into the pterygopalatine and infratemporal fossa, and with a slight spread of the tumor into the pterygopalatine fossa, an anthrostomy is performed at the level of the posterior end of the middle nasal concha, while the tumor is removed from the lumen of the sphenoid sinus by direct transnasal access with resection of the superior nasal concha and removal of the anterior sinus wall using bone cutters , the selection of the part of the tumor located in the lateral sections of the nasopharynx of the opposite side and from the posterior sections of the nasal septum is carried out biportally centripetally through both halves of the nasal cavity, preserving the posterior sections of the nasal septum.
