Abstract
Description
The invention relates to medicine, in particular to traumatology and orthopedics.
The closest technical solution is a method of treating the acromial end of the clavicle by plastically damaged ligaments with subsequent fixation of the limb.
However, when using the known method, it is extremely difficult to straighten and hold in position the end of the clavicle until the end of treatment.
The aim of the invention is to prevent relapse of dislocation.
The goal is achieved by the fact that in the method of treating dislocation of the acromial end of the clavicle by plastically damaged ligaments with subsequent fixation of the limb in front of the plastic of the ligaments, the clavicle is osteotomy with subsequent fixation of the fragments.
The method is as follows.
An incision of 7-8 cm is made over the distal third clavicle and the area of the acromioclavicular joint. At the border between the distal and middle third of the clavicle, the bone is skeletonized, the soft tissues are removed and a Jigley saw is made with an oblique bone saw / oblique clavicle osteotomy /, at the ends of the sawn bone they are made holes with an awl or a small drill. Through these holes, a silk or catgut thread is drawn and fragments are fixed. Bite sharp edges, seams on the periosteum. After this, the distal third of the clavicle is freely set into its bed, then stitches are placed on the ligamentous apparatus and the capsule of the acromioclavicular joint. The wound is sutured in layers tightly and an aseptic dressing is placed. The shoulder girdle is immobilized with a plaster cast of Smirnov-Weinstein for 3-4 weeks. From the 2nd day UHF is prescribed. Upon removal of the plaster cast, exercise therapy, massage of the muscles of the shoulder girdle are performed. The operation performed in this way allows the free and complete reposition of the dislocated end of the clavicle, the formation of a dense shaft around the damaged acromioclavicular joint, which prevents further dislocation of the clavicle even with heavy physical exertion. The obliquely sawn and fixed by the thread ends of the clavicle easily fuse under a small angular displacement. In addition, the coracoid clavicular ligaments are scarred.
The method can be used in all cases of dislocation of the acromial end of the clavicle with the goal of free and full reposition of the dislocated end of the clavicle and keeping it in the adjusted position until the end of treatment.
