Способ позадилонной радикальной простатэктомии при раке предстательной железы, включающий надлобковый доступ, тазовую лимфаденэктомию из области бифуркации повздошных сосудов и запирательного нерва, пересечение пубопростатических связок, перевязку и пересечение дорсального венозного комплекса, пересечение уретры, мобилизацию семенных пузырьков, пересечение семявыносящих протоков, отсечение предстательной железы от мочевого пузыря, формирование шейки мочевого пузыря и наложение везикоуретрального анастомоза, отличающийся тем, что надлобковый доступ производят в виде разреза длиной 3-4 см, при проведении лимфаденэктомии область бифуркации повздошных сосудов и запирательного нерва выделяют с помощью инструмента ретроперитонеоскопа, а для выделения дорсального венозного комплекса выполняют дополнительный продольный разрез длиной 1-2 см на тыльной поверхности полового члена у его основания, через этот разрез производят перевязку дистального конца дорсального венозного комплекса, а перевязку проксимального конца производят со стороны позадилонного доступа, при этом для пересечения пубопростатических связок, перевязки и пересечения дорсального венозного комплекса, пересечения уретры, мобилизации семенных пузырьков, пересечения семявыносящих протоков, отсечения предстательной железы от мочевого пузыря, формирования шейки мочевого пузыря и наложения везикоуретрального анастомоза в рану устанавливают, закрепленные на кольце, шарнирные ретракторы, с помощью которых отодвигают стенку мочевого пузыря и переходную складку брюшины.A method of positon radical prostatectomy for prostate cancer, including suprapubic access, pelvic lymphadenectomy from the area of bifurcation of the iliac vessels and obturator nerve, intersection of the puboprostatic ligaments, ligation and intersection of the dorsal venous complex, intersection of the urethra, mobilization of seminal vesicles, intersection of the seminal vesicles, intersection of the seminal vesicles, intersection the bladder, the formation of the neck of the bladder and the imposition of a vesicourethral anastomosis, characterized in that o suprapubic access is made in the form of a section 3-4 cm long, during lymphadenectomy, the area of bifurcation of the iliac vessels and obturator nerve is isolated using a retroperitoneoscope tool, and an additional longitudinal section 1-2 cm long is performed on the dorsal surface of the penis to isolate the dorsal venous complex its base, through this incision, the distal end of the dorsal venous complex is ligated, and the proximal end is ligated from the side of the posterior approach, with for crossing the puboprostatic ligaments, ligation and crossing of the dorsal venous complex, crossing the urethra, mobilizing the seminal vesicles, crossing the vas deferens, cutting off the prostate from the bladder, forming the bladder neck and applying a vesicourethral anastomosis, they are mounted on the hinge ring with which they move the wall of the bladder and the transitional fold of the peritoneum.
