Abstract
A device for dynamic immobilization and development of movements in the joints of the fingers of the hand after surgical treatment of injuries of the tendon apparatus, including rubber traction fixed to the plaster cuff on the wrist and to the adhesive tape fixed to the distal phalanges of the corresponding fingers.
Description
The utility model relates to medicine, in particular to traumatology and orthopedics, and can be used in the treatment of injuries of the tendon of the hand.
- С.248-250., Волкова A.M. - 1993. - T.1.). Postoperative treatment of injuries of the tendons of the hand involves on the one hand immobilization for the full regeneration of the damaged tendon, but on the other hand the absence of excursions of the immobilized tendon leads to the formation of adhesions in the region. superimposed tendon suture. To solve this problem, some authors propose dynamic immobilization of a removable plaster cast (Beidik OV et al. "The use of tendon plastics in the treatment of patients with injuries of the flexor tendons of 2-5 fingers in the critical zone" Saratov Journal of Medical Scientific Research 2009 - T.5 , - No. 2. - P.248-250., Volkova AM Hand Surgery. - 1993. - T.1.). Others use elastic traction made of glove rubber, fastened with a seam to the nail plate (Shcherbakov MA Optimization of methods for plastic surgery of the flexor tendons of 2-5 fingers of the hand when they are damaged in the area of the bone-fibrous canal - abstract of dissertation ... kmn. Saratov 2009). Foreign authors mainly use industrial production devices.
The proposed devices, in our opinion, have the following disadvantages.
The use of removable bandages implies the participation of an assistant in the process of removing and re-applying the splint. It is difficult for the patient to dose flexion and extension of the fingers; constant monitoring by the attending physician is required. In this regard, the authors offering this design (Volkova A.M.) indicate the need for a long stay of the patient in the hospital under the supervision of the operated surgeon.
Attaching elastic ties to the nail plate causes pain when developing movements. When teething sutures, repeated nail flashing with the use of anesthesia is required. The use of latex glove rubber as rods is not completely reliable due to breaks in the region. mounts.
The device we offer consists of a gypsum cuff with a proximal loop fastening, elastic rods made of rubber bandage and a distal adhesive bandage fastener. The manufacturing procedure of the device is as follows. A plaster cuff is placed on the wrist. In the projection of the passage of the proximal part of the restored tendon through the carpal tunnel, a wire loop is attached to the cuff with plaster bandages (we use a paper clip). Several rounds of adhesive plaster are applied to the distal phalanx of the corresponding finger. Using a small pin drawn through an adhesive plaster, the finger is fixed with a strip of rubber bandage up to 3-5 mm wide to the wire loop in the position of flexion or extension, depending on the type of damaged tendon.
The advantages of the proposed device are as follows.
1. It is not required to periodically remove and put it on.
2. Intact fingers remain intact, movements in them are preserved in full.
3. No nail plate flashing required. When the adhesive patch slides off the distal phalanx, the mount is easily restored by replacing the adhesive patch.
4. The range of movements is easily dosed by the patient himself on the recommendation of the attending physician, it does not require a long stay of the patient in the hospital.
5. The device is easily made within 5-6 minutes from improvised materials available in any medical institution.
Figure 1 of the application presents a view of a device for treating damage to the extensor extremities. Figure 2 shows a view of a device for treating damage to flexors.
