Description
The proposed utility model relates to medical equipment and can be used in surgery, purulent surgery and related specialties for drainage of cavity formations of various localization.
A large number of options for surgical drainage tubes are known: perforated, single-lumen tubular drainage. Such tubes are made of various materials: red or latex rubber, polymer films, polyethylene, silicone, polyvinyl chloride (R. Konden, L. Naihus. Clinical Surgery. M., Practice, 1998. - 716 p.). The described drainage tubes are usually installed during "open" operations directly under visual control. For percutaneous drainage, these tubes are inconvenient, since it is almost impossible to control the position of the working end of the drainage tube during the operation.
Also known is a drainage device configured to simulate purulent formations on the surface (patent RU No. 2381816, A61M 27/00, published 02.20.2010). The device contains perforated drainage tubes (catheters) connected through couplings with means for supplying an antiseptic solution and means for aspirating purulent exudate. Flexible wires are fixed inside the catheters, allowing you to simulate catheters along the surface of purulent formations.
The disadvantage of this device is the complexity of its design. It is used to drain purulent cavities of various localization and allows you to simulate a drainage tube along the topography of the purulent cavity only with direct visual inspection. These drainage tubes cannot be used for percutaneous drainage of cavity formations.
The closest device to the proposed (prototype) is a device for percutaneous drainage of the biliary tract, made in the form of a hollow elastic tube with a number of lateral drainage holes, equipped with a thread located in the lumen of the tube to bend the working end of the tube (patent RU No. 20088033, A61M 27/00, published February 28, 1994). The thread is brought out through the proximal opening and is fixed externally on the working part of the tube. When pulling the thread, the working end of the tube is folded and fixed in the bile duct.
The disadvantage of this device is the design complexity, as well as the likelihood of tissue trauma during translation of the drainage tube into working condition and vice versa.
The objective of the claimed utility model is to develop a simple and affordable to manufacture a controlled device for percutaneous drainage of cavity formations, which allows it to be placed optimally in a drained cavity without injuring soft tissues and walls in the area of the tube.
The technical result of the claimed utility model is the availability of its manufacture, simplicity of design, reduction of trauma to the soft tissues of the walls of the drained cavity when it is introduced.
This is achieved by the fact that in a controlled device for percutaneous drainage of cavity formations made in the form of a hollow elastic tube with rows of lateral drainage holes, provided with a thread partially placed in the lumen of the tube to change the bending angle of the tube working segment by pulling the thread, the thread is sequentially threaded through paired holes of small diameter, sufficient for free sliding of the thread, in the proximal part inside the working part of the tube, and in the distal part of the working hour These tubes - outside with the possibility of its movement and subsequent removal.
Figure 1 shows a controllable device for percutaneous drainage of cavity formations in its original form, figure 2 - the device in the working position during insertion into the cavity.
A device for percutaneous drainage of cavity formations contains a hollow elastic tube 1, the working section 2 of which is provided with rows of lateral drainage holes 3. The distal section of the working section 2 of the tube 1 has paired holes of small diameter 4, sufficient for the thread to slide freely. The proximal section of the working section of the tube also has a pair of holes of small diameter 5. The tube is equipped with a thread 6 located in the lumen and along the outer side surface of the tube, for bending the working section of the tube by pulling the thread. Thread 6 acts as a traction. The thread 6 is carried out in the tube 1 in the form of a U-shaped loop as follows: the thread is inserted into the lumen of the tube, in the proximal section through the hole 5 is brought out, then in the distal section through the pair of holes 4 it is brought inside the tube and then out again. Forming a loop, the thread through the pair of holes 5 is held in the lumen of the tube and then outside the tube.
A controlled device for percutaneous drainage of cavity formations is used as follows. The cavity formation, which is supposed to be drained, is examined using any visualization device, for example, an ultrasound scanner, determining the point of introduction of the drainage tube the place inside the cavity to which it must be brought. The cavity formation is punctured under the control of an ultrasound scanner using a trocar or other special devices, after which a controlled drainage tube 1 is inserted into it. Inside the cavity formation, the position of the working segment 2 of the tube 1 is controlled by an ultrasound scanner. The drainage cabin is placed in the necessary place of the cavity formation and, pulling the thread 6, bend the working segment 2 of the tube 1. Rotating the tube around the axis and adjusting the bending of the working segment of the tube by tensioning the thread 6, the distal section of the working segment is brought to the desired area of the drained cavity, bypassing the existing internal protrusions. After installing the drainage tube in the desired position, the thread 6 is removed from the tube by pulling on one of the ends. The tube is fixed to the skin using one of the known methods. The secreted fluid from the strip formation enters the working section 2 of the tube 1 through the drainage holes 3 and is discharged out.
The use in surgery, purulent surgery and related specialties of the proposed device for percutaneous drainage of cavity formations of different localization will reduce the trauma of soft tissues of the walls of the drained cavity and reduce the risk of complications.
