Description
"Anestetisk preparat"
Denne oppfinnelse angår et nytt preparat som er i be-sittelse av anestetisk virkning og som er tilnærmet uten uønskede bivirkninger ved administrering til varmblodige dyr ved inhalering.
I henhold til oppfinnelsen tilveiebringes et anestetisk preparat som omfatter som anestetisk middel 1,2,2-trifluoretyl-difluormetyléter sammen med oksygen og eventuelt sammen med ett eller flere andre fysiologisk godtagbare materiale(r), idet andelen av anestetisk middel i preparatet er slik at når preparatet administreres ved inhalering til et varmblodig dyr frembringes og/eller opprettholdes anestesi, og andelen av oksygen i preparatet er slik at når preparatet administreres ved inhalering til et varmblodig dyr, opprettholdes respirasjonen.
Forbindelsen 1,2,2-trifluoretyl-difluormetyleter er en kjent forbindelse (Tetrahedron, 1971, 21_, 4533 til 4551) , men den kan mest hensiktsmessig fremstilles ved fremgangsmåten beskrevet i vår britiske ansøkning 50040/75. Det skal forstås at 1,2,2-trifluoretyl-dif luormetyleteren må være fri for toksiske forurensninger når den anvendes i preparatet ifølge oppfinnelsen.
Oksygenet som er tilstede i preparatet ifølge oppfinnelsen, kan være rent oksygen, eller det kan være i form av luft, dvs. i en blanding med nitrogen og mindre mengder av andre gasser.
Det (de) andre fysiologisk godtagbare materiale(r) som eventuelt kan være tilstede i preparatet ifølge oppfinnelsen, kan f.eks. være ett eller flere stoffer valgt fra andre, anestetiske midler for inhalering, f.eks. halotan, dinitrogenoksyd, dietyleter, divinyleter, trifluorety1-vinyleter, cyklopropan, trikloretylen, kloroform, enfluran, fluroxen, metoksyfluran, tefluran og 1-klor-2,2,2-trifluoretyl-difluormetyleter; farmasøytisk inerte gasser, f.eks. nitrogen, kjemisk inerte gasser som er tilstede i luften, f.eks. neon, argon og xenon, og karbondioksyd og vanndamp; og farmasøytisk godtagbare stabilisatorer som kan være tilstede for å beskytte én eller flere av de andre komponenter i preparatet fra innvirkning av lys, oksydasjon og/eller angrep av syre eller base. Som egnet stabilisator kan f.eks. anvendes et flyktig stabiliseringsmiddel som er fysiologisk godtagbart, f.eks. etanol, eller et ikke-flyktig stabiliseringsmiddel som ikke medføres vesentlig under fordampning, f.eks. tymol.
Preparatet ifølge oppfinnelsen vil vanligvis inneholde
mellom 0,25 og 10 volum% og særlig mellom 0,5 og 6 volum% basert på volumet av 1,2,2-trifluoretyl-difluormetyleteren ved anvendelse for frembringelse av anestesi, og vil vanligvis inneholde mellom 0,25
og 3%, og særlig ca. 2%, ved anvendelse for opprettholdelse av anestesi.
Preparatet ifølge oppfinnelsen kan administreres til varmblodige dyr, innbefattet mennesker, for å oppnå anestesi ved vanlige metoder. Preparatet kan gjøres ferdig på forhånd og administreres som sådant, eller alternativt kan 1,2,2-trifluoretyl-difluormetyleteren og oksygen, hvorav én eller begge kan ha andre fysiologisk godtagbare materialer tilstede, administreres separat, idet preparatet ifølge oppfinnelsen dannes enten umiddelbart før eller under administreringsforløpet. F.eks. kan preparatet anvendes i apparater eller maskiner som er konstruert for fordampning av flytende anestetiske midler og blandingen derav med oksygen eller med luft eller andre gassformige blandinger inneholdende oksygen i mengder som kan understøtte respirasjonen.
I henhold til et ytterligere trekk ved oppfinnelsen tilveiebringes et inhaleringsanestetisk apparat inneholdende 1,2,2-trifluoretyl-dif luormetyleter .
I henhold til et ytterligere trekk ved oppfinnelsen tilveiebringes en fremgangsmåte for frembringelse av anestesi i et varmblodig dyr, omfattende administrering til nevnte dyr en anestetisk effektiv mengde av 1,2,2-trifluoretyl-difluormetyleter sammen med tilstrekkelig oksygen til å opprettholde respirasjonen.
De følgende eksempler.skal tjene til å illustrere oppfinnelsen ytterligere.
Eksempel 1
En gruppe på 6 mus anbringes i et kammer på 10 liter, som inneholder en fast blanding av natriumhydroksyd og kalsiumoksyd, og en blanding av difluormetyl-1,2,2-trifluoretyleter og oksygen, i kjent prosentmengde frigjøres inn i kammeret. En reservoirsekk inneholdende blanding med kjent prosentmengde anvendes for å opprettholde atmosfærisk trykk eftersom blandingen inhaleres av musene og eftersom utåndet karbondioksyd absorberes av natriumhydroksyd-kalsiumoksyd-blandingen. Efter 30 minutter bestemmes konsentrasjonen av difluormetyl-1,2,2-trifluoretyleter i kammeret ved gass-kromatografi.
Forsøket gjentas under anvendelse av forskjellige blandinger av difluormetyl-1,2,2-trifluoretyleter og oksygen, og AC^0, dvs.
den volumkonsentrasjon av difluormetyl-1,2,2-trifluoretyleter som bedøver 3 mus av 6 efter 30 minutters behandling, blir funnet å
være 1,35%. ^C^, ^vs. den volumkonsentras jon av dif luormetyl^-1, 2 , 2-trifluoretyleter som dreper 3 mus av 6 efter 30 minutters behandling, blir funnet å være 7%. Forbindelsens terapeutiske forhold er derfor 7,0/1,35, dvs. 5,2. Under tilsvarende betingelser er AC5Q, LC5Q og
terapeutisk forhold for halotan henholdsvis 0,85%, 3,4% og 4,0.
Eksempel 2
En katt bedøves med tiopenton-hatrium, og en polyetylenkanyle innføres i en femoralarterie for å gjøre det mulig at arterieblodtrykket (b.p.) kan måles og prøver av arterieblod oppsamles for måling av partialtrykket av karbondioksyd (pC^) deri. Forskjellige blandinger av difluormetyl-1,2,2-trifluoretyleter og oksygen føres til katten ved hjelp av et Ayres T-stykke og et endotrakialt rør,
og arterieblodtrykket og pC02måles for hver av blandingene. Forsøket gjentas derefter i hver av tre ytterligere katter, og gjennomsnittlig b.p. ogPCO2beregnes for hver blanding. Disse gjennomsnittlige verdier er vist i den følgende tabell. Det er funnet at minimum konsentrasjon av eteren i oksygen som opprettholder anestesi i katten, er 2 volum%.
Under tilsvarende betingelser er minimum konsentrasjon av halotan i oksygen som opprettholder anestesi, 1 volum%, og de følgende gjennomsnittlige verdier for arterielt b.p. og pC02finnes med varierende konsentrasjoner av halotan i oksygen.
Det vil sees at difluormetyl-1,2,2-trifluoretyleter er mindre hypotensiv enn halotan ved sammenlignbare anestetiske konsentrasjoner, og at pCC^-målinger viser at det ikke er noen betydelig forskjell mellom de respirasjonsnedsettende virkninger av difluormetyl-1,2,2-trifluoretyleter og halotan.
Eksempel 3
En katt prepareres som beskrevet i eksempel 2, og en ytterligere polyetylenkanyle innføres i en femoralvene for å mulig-gjøre infusjon av adrenalin, og kattens hjerte kontrolleres ved elektrokardiogram. Katten holdes derefter i anestesi med en 2% volum/volum blanding av difluormetyl-1,2,2-trifluoretyleter og oksygen, og økende mengder av adrenalin infuseres inntil ventrikkel-arrhytmi opptrer. Forsøket gjentas med hver av tre ytterligere katter, og den gjennomsnittlige arrhytmiske dose av adrenalin blir funnet å.være 8,07 yg/kg kroppsvekt.
Under tilsvarende betingelser blir den gjennomsnittlige minimale arrhytmiske dose av adrenalin når katter er bedøvet med 1% volum/volum halotan og oksygen, funnet å være 2,83 yg/kg kroppsvekt. Det vil derfor sees at difluormetyl-1,2,2-trifluoretyleter forårsaker mindre følsomhet for hjertet overfor adrenalin enn halotan gjør.
Claims (10)
1. Anestetisk preparat, karakterisert ved at det som anestetisk middel inneholder 1,2,2-trifluoretyl-difluormetyleter sammen med oksygen og eventuelt sammen med ett eller flere andre fysiologisk godtagbare materiale(r), idet andelen av anestetisk middel i preparatet er slik at når preparatet administreres ved inhalering til et varmblodig dyr, frembringes og/eller opprettholdes anestesi, og andelen av oksygen i preparatet er slik at når preparatet administreres ved inhalering til et varmblodig dyr, opprettholdes respirasjonen.
2. Preparat som angitt i krav 1, karakterisert ved at det (de) andre fysiologisk godtagbare materiale(r) omfatter ett eller flere stoffer valgt fra andre anestetiske midler for inhalering, farmasøytisk inerte gasser, kjemisk inerte gasser som er tilstede i luften, karbondioksyd, vanndamp; og farmasøytisk godtagbare stabilisatorer.
3. Preparat som angitt i krav 1 eller 2,
karakterisert ved at det inneholder mellom 0,25 bg 10 volum% av 1,2,2-trifluoretyl-difluormetyleteren.
4. Preparat som angitt i krav 3, karakterisert ved at det inneholder mellom 0,5 og 6 volum% av 1,2,2-trifluoretyl-dif luormetyleteren.
5. Preparat som angitt i krav 1 eller 2,
karakterisert ved at det inneholder mellom 0,25 og 3 volum% av difluormety1-1,2,2-trifluoretyleter.
6. Preparat som angitt i 'krav 5, karakterisert ved at det inneholder 2 voluml difluormetyl-1,2,2-trifluoretyleter.
7. Inhaleringsanestetisk apparat inneholdende 1,2,2-trifluoretyl-difluormetyleter.
8. Fremgangsmåte for å frembringe anestesi i et varmblodig dyr, karakterisert ved at det til nevnte dyr administreres en anestetisk effektiv mengde av 1,2,2-trifluoretyl-dif luormetyleter sammen med tilstrekkelig oksygen til å opprettholde respirasjon.
