Description
Foreliggende oppfinnelse vedrører fremstilling av 7-[2-karboksy-2-(2-tienyl)acetamido]cefalosporansyre og salter derav som har høy antibakteriell virkning.
Cefalosporiner adskiller seg fra hverandre ved substitu-enten R i den generelle formel angitt nedenfor hvor acylgruppen i 7-aminocefalosporansyren er avledet fra en karboksylsyre eller et funksjonelt derivat derav, slik som et acylhalogenid eller anhydrid.
De farmakodynamiske og antibakterielle egenskaper av et gitt cefalosporin er i stor utstrekning gitt av betydningen av gruppen R.
a-karboksy-3-tienylmetylcefalosporin er angitt i syd-afrikansk patent nr. 68/1208 som et antimikrobielt middel.
I tysk off.skrift 1.959.569 er det bl.a. angitt cefalosporin-forbindelser som har en forestret karboksygruppe og en tienylgruppe i 2-stilling på 7-acetamidosubstituenten, men det nevnes intet om noen forbindelse som har en fri karboksygruppe i denne stilling, og det er ingen spesifikk omtale av 2-tienyl-gruppen eller antydning av at denne skulle være en særlig fordelaktig substituent i 7-sidekjeden.
Det har nå vist seg at cefalosporiner med formelen:
og farmasøytisk aksepterbare salter derav, er karakteriserbare forbindelser, fremstillbare i gode utbytter og er meget aktive mot både gram-positive og gram-negative mikroorganismer.
Fra britisk patent 1.193.302 er bl.a. kjent 7-[2-karboksy-2-(3-tienyl)acetamido]cefalosporansyre. Denne forbindelse er imidlertid mindre effektiv enn den ifølge oppfinnelsen frem-stilte 2-tienyl-isomer som vist ved sammenligningsdata i tabell I, II og IIA. På grunnlag av all den litteratur som finnes på dette område, kan foreliggende oppfinnelse ansees som en ut-velgelsesoppfinnelse.
Ifølge oppfinnelsen tilveiebringes en fremgangsmåte for fremstilling av en forbindelse med formelen: og farmasøytisk aksepterbare salter derav. Fremgangsmåten karakteriseres ved at man omsetter en forbindelse med formelen med 2-tienylmalonsyre i nærvær av et vannløselig karbodiimid ved en pH på ca. 6,0 i et reaksjonsinert løsningsmiddel ved omgivelsestemperatur og, om ønsket, fremstilles de farmasøytisk aksepterbare salter derav.
Det følgende skjema illustrerer fremgangsmåten som anvendes ifølge foreliggende oppfinnelse^
I praksis anvendes vanligvis ekvimolare mengder av reaksjonskomponentene, idet så meget som 10% overskudd av amino-forbindelsen og karbodiimid gir utmerkede utbytter av det ønskede produkt. For å oppnå optimale resultater er det foretrukket at pH-verdien av reaksjonsblandingen holdes ved ca. 6,0 ved til-setning av enten 6N saltsyre eller en mettet løsning av natrium-bikarbonat.
Det reaksjonsinerte løsningsmiddel som utgjør væskefasen av reaksjonsblandingen kan være vandig eller vann pluss et vann-blandbart reaksjonsinert løsningsmiddel slik som aceton, tetra-hydrofuran, dioksan eller dimetylformamid.
Karbodiimid-koblingsreagensene som er mest egnet for an-vendelse ved fremgangsmåten ifølge foreliggende oppfinnelse, er de vannløselige karbodiimider som inneholder i sin kjemiske struktur en basisk aminogruppe som er i stand til å danne et syreaddisjonssalt og således gir vannløselighet. Det foretrukne karbodiimid for syntesen av den beskrevne forbindelse er N-etyl-N-3-(dimetylamino)propylkarbodiimid.
Ved avslutningen av den ovenfor nevnte reaksjon heves pH-verdien til ca. 8, og eventuelt ikke sure biprodukter ekstraheres med et løsningsmiddel som ikke er blandbart med vann. Vannfasen gjøres derefter sur og produktet ekstraheres og over-føres til en lettere håndterbar form slik som et salt.

Som tidligere angitt er et karakteristisk trekk ved det sure cefalosporin fremstilt ifølge foreliggende oppfinnelse dets evne til å danne basiske salter. Disse salter dannes ved a-karboksygruppen så vel som ved 3-karboksygruppen og tillater fremstillingen enten av et mono- eller dibasisk salt. Salt-typen, dvs. mono- eller di-, vil i stor utstrekning være bestemt av mengden av basisk reagens som anvendes i forhold til mengden av utgangssyre. Slike basiske reagenser som er egnet ved fremstillingen av de angitte salter, kan være en rekke forskjellige og er ment å omfatte slike baser som organiske aminer, ammoniakk, alkalimetallhydroksyder, karbonater, bikarbonater, hydrider og alkoksyder, så vel som jordalkalimetallhydroksyder, hydrider, alkoksyder og karbonater.
Det må selvfølgelig anvendes farmasøytisk aksepterbare salter. De foretrukne farmasøytisk aksepterbare salter omfatter natrium, aluminium, kalium, kalsium, magnesium, ammonium og substituert ammonium, f.eks. procain, dibenzylamin, N,N<1->dibenzyl-etylendiamin, N,N-bis(dihydroabietyl)-etylendiamin, 1-efenamin, N-etylpiperidin, N-benzyl-p-fenetylamin, trietylamin, så vel som salter med andre aminer som er blitt anvendt for å danne salter med penicilliner og cefalosporiner.
Det angitte cefalosporin og dets salter utviser in vitro virkning mot en rekke mikroorganismer omfattende både gram-positive og gram-negative bakterier. Den nyttige virkning kan lett demonstreres ved in vitro forsøk mot forskjellige organismer i et hjerne-hjerte-infusjonsmedium ved hjelp av den vanlige dobbelte seriefortynningsteknikk. In vitro aktiviteten av de beskrevne forbindelser gjør dem nyttige for topiske anvendelser i form av salver, kremer o.l.
Disse cefalosporiner er effektive antibakterielle midler
in vivo i dyr, deriblant mennesker, spesielt via den parenterale administreringsvei.
Legen vil bestemme dosen som vil være mest egnet for en
gitt individuell person, og den vil variere med alder, vekt og responsen til den spesielle pasient så vel som med typen og symptomene og de farmakodynamiske karakteristika av det spesielle middel som administreres. Det viser seg ofte at når midlet administreres oralt, vil det være nødvendig med større mengder av den aktive ingrediens for å gi det samme nivå som man får med en mindre mengde som administreres parenteralt.
Idet man tar hensyn til de angitte faktorer, anser man at
en effektiv daglig oral dose av forbindelsen fremstilt ifølge foreliggende oppfinnelse på mennesker på ca. 500-2000 mg/kg pr. dag, med et foretrukket område på ca. 750-1500 mg/kg pr. dag i enkle eller oppdelte doser, og en parenteral dose på 20-400 mg/ kg pr. dag, med et foretrukket område på ca. 30-300 mg/kg effektivt vil lindre infeksjons-symptomene. Disse verdier er bare gitt for å illustrere og i individuelle tilfeller kan man gi høyere eller lavere doser.
Det antimikrobielle spektrum av 7-[2-karboksy-2-(2-tienyl)-acetamido]cefalosporansyre og 7-[2-karboksy-2-(3-tienyl)-acetamido]cefalosporansyre er angitt i tabell I. Forsøkene er utført under standard betingelser hvor næringsmediet, inne-holdende forskjellige konsentrasjoner av forsøksmaterialet, tilsettes den spesielle organisme, og minimumskonsentrasjonen (MIC) hvor det ikke finner sted noen vekst av organismen noteres.
Tallene i tabell I indikerer at 2-tienyl-forbindelsen fremstilt ifølge foreliggende oppfinnelse er markant bedre mot gram-positive mikroorganismer enn 3-isomeren, mens mot gram-negative organismer er 3-isomeren noe bedre.
Tabell II og IIA gir in vivo tall for 7-[2-karboksy-2-(2-tienyl)acetamido]cefalosporansyre og 7-[2-karboksy-2-(3-tienyl)acetamido]cefalosporansyre mot visse eksperimentelle in-feksjoner i mus.
Verdiene (overlevende/totalt antall infiserte mus) er opp-nådd under standard betingelser som er kjent for fagmannen på området. Forsøksforbindelsen administreres til den infiserte musen ved hjelp av en multippel doseringsmetode hvor første dose gis 1/2 time efter inokuleringen og gjentas 4 og 25 timer senere.

Cefalosporinet fremstilt ifølge foreliggende oppfinnelse og saltene derav er av verdi som antibakterielle midler og er bemerkelsesverdig effektive ved behandling av en rekke infek-sjoner som skyldes gram-negative og gram-positive bakterier i fjærkre og andre dyr, deriblant mennesker.
Eksempel
7-[ 2- karboksy- 2-( 2- tienyl) acetamido] cefalosporansyre, mononatriumsalt
En løsning av 372 mg (2 mmol) 2-tienylmalonsyre i 10 ml vann ved en pH på 6,0 og avkjølt i et isbad tilsettes 571 mg (2,1 mmol) 7-aminocefalosporansyre og pH-verdien justeres tilbake til 6,0 like efter tilsetningen. N-etyl-N'-3-(dimetyl-amino)-propylkarbodiimid tilsettes, og reaksjonsblandingen står under omrøring i 5 minutter ved isbad-temperatur og 75 minutter ved værelsestemperatur. Saltsyre (6N) tilsettes periodisk for å justere pH mellom 5,8 og 6,1 under reaksjonsperioden ved værelsestemperatur. pH-verdien heves til slutt til 7,7 og reaksjonsblandingen ekstraheres med etylacetat. Saltsyre til-
settes til den adskilte vandige fase inntil man får en pH på
2,6, og den resulterende surgjorte fase ekstraheres en rekke ganger med etylacetat. De kombinerte organiske ekstrakter vaskes i motstrøm med vann og en mettet saltløsning og tørkes derefter over natriumsulfat. Det filtrerte etylacetat behandles med 2 mmol natriumheksanoat oppløst i det samme løsningsmiddel,
og blandingen får lov til å stå ved værelsestemperatur mens mono-natriumsaltet utfelles. Overliggende etylacetat dekanteres og det faste stoffet som er tilbake tritureres med aceton og filtreres, 384 mg (41,6%).

- Analogifremgangsmåte for fremstilling av en terapeutiskaktiv cefalosporinforbindelse med formelenog farmasøytisk aksepterbare basesalter derav, karakterisert ved at en forbindelse med formelenomsettes med 2-tienylmalonsyre i nærvær av et vannløselig karbodiimid ved en pH på ca. 6,0 i et reaksjonsinert løsningsmiddel ved omgivelsestemperatur, og om ønsket fremstilles de farma-søytisk aksepterbare salter derav.
