Description
Proteseanordning.
Foreliggende oppfinnelse angår ledd-proteser bestemt for bruk i ortopedi.
Bruken av proteser for å utbedre eller erstatte benstruktur i en levende kropp er vel kjent. Vanlige proteser har vært fremstilt av metaller, keramikk og plastmaterialer, avhengig av deres påtenkte anvendelse.
Et krav til en tilfredsstillende protese som skal permanent innplanteres, er at den må være fysiologisk inert i ubegrenset tid, og en vanskelighet med vanlige proteser er at materialene hvorav de fremstilles, kan bli fysiologisk avvist, kan foranledige betennelse i kroppsvey eller kan nedbrytes, ved legemlige prosesser.
Ennvidere foranlediger påkjenninger fremkalt i en levende kropps skjelett, som er naturlig utformet til å oppfylle en lastbærende funksjon, betraktelige vanskeligheter av mekanisk og konstruksjonsr messig art. Dessuten er det funksjonelle gjensidighetsforhold mellom skjelettet og resten av kroppen av en slik biologisk innviklet art at ennu storre kompliserte vanskeligheter moter utviklingen av egnede proteser.
Spesielt akutt er de skjelett-områder som normalt utsettes for betraktelige påkjenninger. Eksempelvis kan hofteleddproteser synes å funksjonere tilfredsstillende i adskillige år, men slike proteser har imidlertid tendens til å foranledige fortykninger, betennelser <p>g smerte i leddets synoviale foring og kapsel.
Når et bensegment mangler på grunn av en ulykke eller
operasjon, har det hittil ikke vært noen tilfredsstillende metode å ty til enn en benhomopodning for å erstatte det manglende bensegment og for å skjote den gjenværende del av benet. Det har vært gjort forsok på å bruke poros keramikk, men dette har vært generelt mislykket, ikke bare på grunn av at slike materialer ofte ikke aksepteres av kroppen, men også på grunn av at stimulering av natrulig benutvikling inne i keramikken ikke har kunnet oppnås med hell.
Det er derfor et formål med foreliggende oppfinnelse å tilveiebringe forbedrede proteser som oppforer seg usedvanlig vel i det krevende mekaniske miljo av skjelettstrukturen i en levende kropp og som er i stand til å etablere en fast forbindelse dermed. Et annet formål med- oppfinnelsen er å tilveiebringe proteser som er usedvanlig godt egnet for utstrakt funksjonell ytelse, som for eksempel- ledd-proteser, dentalproteser, proteser for hel eller delvis erstatning av ben og gitterverk-lignende proteser inn i og gjennom hvilke-naturlig, levende benvev kan gro slik at utbedring eller erstatning av benstrukturen delvis kan gjennomføres ved naturlige prosesser. Et ytterligere formål er å tilveiebringe forbedrede proteser som har en elastisitetsmodul tilnærmet lik den til naturlig, levende ben og som ikke konsentrerer påkjenninger ved protese/ben-grensesjiktetjmen istedenfor boyer seg i harmoni med det naturlige ben under påkjenninger.
Det er funnet at proteser med forbedrede egenskaper kan fremstilles ved å belegge egnede substrater av foronsket fasong og storrelse med pyrolytisk karbon. Bruken av pyrolytisk karbon som et belegg for proteser er generelt beskrevet i US patent nr. 3.526.005. Pyrolytisk karbon er istand til ikke bare merkbart å oke styrken
av substratet hvorpå det er belagt, men det er også istand til å motstå slitasje og nedbrytning selv når det er innplantert i en levende kropp for lengre tid. Mens det heretter generelt er referert til bruken av proteser for utbedring eller erstatning av benstruktur i en levende menneskekropp, skal det også gjores oppmerkson på at de forbedrede ortopediske anordninger også har veterinær anvendelse.

Generelt påfbres det pyrolytiske karbcnbelegg på et egnet substratmaterial slik at det dekker i det minste den vensentlige del av substratets overflate. Tykkelsen av det pyrolytiske karbonbelegg bor være tilstrekkelig til å tilveiebringe den nødvendige styrke for den påtenkte anvendelse og ofte er det onskelig å anvende belegget for å meddele ytterligere styrke til det spesielle substrat som belegges. Belegget vil i det minste være 25 mikron tykt og vanligvis minst ca. 50 mikron tykt. Hvis det anvendes et ganske svakt substrat, eksempelvis et laget av grafitt, kan det være onskelig å tilviebringe et tykkere belegg av pyrolytisk karbon for å styrke den sammensatte proteseanordning.
Skjont et ytre belegg som i det vesentlige er av tilnærmet fullstendig ren, tett pyrolytisk karbon, har tilstrekkelig strukturell styrke, kan det dessuten anvendes en sampåfbring av silisium eller en eller annen karbiddannende tilsetning for å forbedre styrken og slitasjemotstandsevnen til karbonbelegget. Som mer detaljert beskrevet senere, kan silisium i en mengde av opp til ca. 200 vektprosent dispergeres som SiC i det pyrolytiske karbon uten å redusere det pyrolytiske karbons biologisk forenlige egenskaper.
Fortrinnsvis brukes isotropt pyrolytisk karbon som har en BAF-verdi på ca. 1,3 eller mindre, som definert i "Journal of Applied Chemistry", Vol. 6, p.<*>+77, (1956). Som forklaring bemerkes at
1 ,0 (d,et laveste punkt på BAF-skalaen) betegner fullstendig
isotropt karbon, mens hoyere verdier antyder oket grad av anisotropi.
Det pyrolytiske karbons tetthet (densitet) betraktes som et viktig trekk ved bestemmelse av den ytterligere styrke som det pyrolytiske karbonbelegg vil gi substratet. Tettheten er dessuten viktig når det gjelder å sikre vev-forenlighet og tilstrekkelig slitasjemotstandsevne av protesen hvor dette bestemmes. Det antas at det pyrolytiske karbon minst bor ha en tetthet på 1,5 g/cm^, og fortrinnsvis bor tettheten ligge mellom ca. 1,9 og 2,2 g/cm^.
Da protesens substratmaterial fortrinnsvis vil være fullstendig
innkapslet i pyrolytfsk karbon, er valg av material for substratet ikke det aller viktigste i og for seg. Substrat-materialet bor imidlertid ha den nbdvendige mekaniske styrke og strukturelle egenskaper for den spesielle proteseanvendelse hvortil den skal brukes. Hvis imidlertid protesen under bruk skal ha deler av substratet blottlagt for kroppsvev, slik det for eksempel kan forekomme fra maskinering av protesen til den endelige form etter at basisfasongen er blitt belagt med pyrolytisk karbon, bor substratet velges fra materialer som er relativt biologisk inert, fortrinnsvis kunstgrafitt. Det er også viktig at substratmaterialet er forenlig med pyrolytisk karbon, og mer spesielt at det er egnet for bruk i prosessbetingelsene for beleggning med pyrolytisk karbon. Eksempler på egnede substratmaterialer omfatter kunstgrafitt,
borkarbid, silisiumkarbid, tantal, molybden, wolfram og forskjellige typet keramikk, slik som mullitt.
I proteser som er beregnet på å erstatte en merkbar mengde partiell utskifting ved hjelp av naturlig benvekst, anvendes fortrinnsvis kunstgrafitt som substratmaterial på grunn av at dette material har en elastisitetsmodul på fra ca. 0,1^- til ca. 0,28 x 10 kg/cm , hvilket ordinært er i området for elastisitetsmodulen til naturlig levende ben. Eksempelvis er der en spesiell foretrukken form av grafitt for bruk som substratmaterial,polykrystallinsk grafitt.
Et eksempel på slik grafitt er den polykrystallinske grafitt som selges under handelsbetegnelsen "POCO AXF Graphite" som har en tetthet på ca. 9 g/cm^ og en gjennomsnittlig.krystallittstorrelse (L ) på ca. 300 Å, en isotrop! på nær 1,0 på Bacon-skalaen og en elastisitetmodul på ca. 0,119 x 10 6 kg/cm 2.
Når det er onskelig å tilveiebringe et gitterverk inn i og gjennom hvilket nytt benvev kan gro, eksempelvis for fast befestigelse av en protese til et ben, eller for å erstatte eller utbedre benstruktur partielt gjennom erstatning med nytt, naturlig benvev, som senere beskrevet, kan det anvendes substrater som passende metall-eller karbonnettinger, metalltråder eller fibre som substratmaterial. Eksempelvis kan det brukes en netting fremstilt fra en legering bestående av $ 0% rhenium og 50% molybden. Nettinger av tantal, wolfram, molybden eller legeringer derav, som fortrinnsvis er belagt med wolfram for å hindre sprbhet under beleggningen med pyrolytisk karbon, kan også brukes. Det kan også brukes fiber-aggregater i tillegg til nettinger, slik som ikke-vevede strukturer N og filamentomvundne strukturer som substratmaterialer.
Foreliggende oppfinnelse går nærmere bestemt ut på en sammensatt ortopedisk leddproteseanordning for reparasjon eller erstatning av struktur i en levende kropp, omfattende et ildfast substrat med passende forutbestemt form og med et damp-påfbrt pyrolytisk karbonbelegg som er biologisk forenlig med levende vev og har en tetthet (densitet) på minst 1_,5 g/crn-^ og en tykkelse på minst 25 mikron, og det særegne ved denne proteseanordning består i at
ft 6 ? substratet har en elastisitetsmodul på 0.12x10 - 0.28x10 kg/cm og at proteseanordningen har en flate innrettet til å forbindes med benvev og en sliteflate, hvor det pyrolytiske karbonbelegg på sliteflaten er polert og hvor det pyrolyttiske karbonbelegg på i det minste en del av benvev-forbindelsesflaten hår en overflateporositet
som fremkommer ved pådampingen og uten ytterligere bearbeiding.
Ytterligere særtrekk ved oppfinnelsen fremgår av kravene.
Det pyrolytiske karbonbelegg påfores substratet under anvendelse
av et egnet apparat for dette formål. Fortrinnsvis brukes et apparat, som holder et substrat i bevegelse mens belegningsprosessen utfores for å sikre at belegget blir jevnt fordelt på de foronskede overflater av substratet. Et roterende trommel-beleggningsapparat eller et vibrerende bor kan brukes. Når substratet som skal belegges er lite nok til å kunne sveve i en oppoverrettet gasstrom, brukes fortrinnsvis4et beleggningsapparat basert på fluidisert sjikt.Når det anvendes storre substrater eller hvor det er onskelig å variere tykkelsen eller andre egenskaper av det pyrolytiske karbonbelegg over forskjellige deler av substratet, kan det anvendes forskjellige belegningsmetoder, slik som å bære substratet på en roterende eller stasjonær dorr inne i et stort fluidisert sjikt.
Egenskapene til det karbon som skal påfores, kan varieres ved å variere betingelsene hvorunder pyrolysen utfores. For eksempel
i en fluidsert sjikt-belegningsprosess, hvori brukes en blanding av en hydrokarbongass, slik som metan, og en inert gass, slik som helium eller argon, påvirker variasjoner i hydrokarbongassens volumprosent, den fluidiserende gasstroms totale stromnings-hastighet og temperaturen hvorved pyrolysen utfores, egenskapene til det pyrolytiske karbon som påfores.
Som tidligere nevnt kan belegget være av tilnærmet rent pyrolytisk karbon, eller det kan inneholde en karbiddannende' tilsetning, slik som silisium, som er funnet å oke beleggets totale mekaniske egenskaper. Silisium i en mengde av opp til 20 vektprosent på basis av den totale vekt av silisium pluss pyrolytisk karbon kan innblandes uten å redusere det pyrolytiske karbons foronskede fysiologiske egenskaper, og når silisium brukes som tilsetning, anvendes det generelt i en mengde på mellom 10 og 20 vektprosent. Eksempler på andre karbid-dannende elementer som kan brukes som tilsetning i ekvivalente vektprosenter, omfatter bor, wolfram, tantal, niob, vanadium, molybden, aluminium, zirkonium, titan og hafnium. Generelt vil slike elementer ikke bli brukt i en mengde stbrre enn 10 atomprosent"på basis av de. totale atomer av pyrolytisk karbon pluss elementet.
Pyrolytisk karbon som har de .tidligere nevnte egenskaper, betraktes å være spesielt fordelaktig for dannelse av overflaten, på en protese på grunn av at det er inert overfor stoffskifte-prosesser, enzymer og andre væsker (fysiologiske væsker) som finnes i en levende kropp. Dessuten er slik pyrolytisk karbon ikke skadelig for naturlig benvekst. Spesielt foretrukket er pyrolytisk karbon som har en tetthet mellom ca. 1,9 og 2,2 g/cm^, hvori overflateporositeten av slikt karbon letter groing av ben til karbonet, hvilket er spesielt fremmende for naturlig benvekst slik at det etableres en god binding mellom naturlig ben og protesen. For å forbedre befestigelsen av enten ben eller vev kan overflaten oksyderes. Dette kan gjores for å forbedre overflate-ruheten og/eller for å tilveiebringe oksygenbærende polare grupper på overflaten som kan reagere med vev- eller benmolekylene.
Protesens pyrolytiske karbonoverflate kan fremstilles med forskjellige fysikalske egenskaper på forskjellige overflatesteder, eksempelvis kan det anvendes en tett, polert sliteflate ved den
leddforbindende ende av et proteseledd, mens en mer ru,poros
N overflate kan anvendes på overflater hvor protesen skal forbindes med naturlig ben for å lette benveksten dertil.
I tillegg til mekanisk modifisering av den pyrolytiske karbon-overf late, slik som polering, kan det være onskelig å bruke andre fysikalske og kjemiske modifikasjoner av den pyrolytiske karbonoverflate. Eksempelvis kan kjemisoerberte gasser, slik som oksygen, fjernes for tilveiebringelse av en mer hydrofob overflate, eller omvendt kan en overflate som har adsobert gass, slik som oksygen, ellBP som er kjemisk modifisert, for eksempel ved dannelse av overflatehydroksylgrupper, anvendes for tilveiebringelse av mer hydrofile overflater. Det antas at pyrolytiske karbonoverflater som er mer hydrofile, slike som dem som har overflatehydroksylgrupper, er mer tjenlig for etablering av en fast binding til naturlig ben. Dessuten bor protesen være steril for innplantering i en levende kropp. Den steriliseres og kjemisorbert oksygen fjernes ved oppvarmning i vakuum ved en forhoyet temperatur.
Oppfinnelsen skal nærmere beskrives under henvisning til vedfoyde tegning. Figur 1 er et perspektivriss, delvis gjennomskåret, av en kneledd/protese.

Figur 3 er et perspektivriss, delvis gjennomskåret, av en hofteleddprotese. I Figur 1 i den medfblgende tegning er illustrert en kneleddprotese 10 for erstatning av et bdelagt kneledd. Kneleddprotesen 10 er fremstilt fra et substrat 12 på hvilket er påfort et ytre pyrolytisk karbonbelegg 11*. Protesen 10 har en svingbar sliteflate 16 som er buet i en retning mellom en frontflate 26 og en bakflate 28 og som er utformet for svinging mot en annen slik buet sliteflate ved kneleddets funksjonering. Sliteflaten 16 er bare svakt buet i retningen mellom sidekanter 30 for å begrense svinging i den retning og således stabilisere kneleddet mot side-veis bevegelse. Den svingbare sliteflate 16 er polert ved hjelp av polerskive og et diamantstovslipemiddel for å redusere friksjon og slitasje.
Som klarere vist i fig. 2, er flaten 18 som er motsatt sliteflaten 16, forsynt med en serie fremspring eller tapper 20 og innsnitt eller spor 22 for å sikre en sterkt, mekanisk stabil forbindelse mellom protesen 10 og det natulige ben i femur eller tibia. Det pyrolytiske karbonbelegg på den benskjotende side 18 som tapper 20 og spor 22, er ikke polert, men besitter heller den runet som er oppnådd ved karbonets påforing. Dessuten kan det være oksydert for å forbedre befestigelsen. Denne ruhet stimulerer utviklingen av en sterk mekanisk forbindelse mellom protesen 10 og der naturlige ben gjennom groing av naturlig benvev til protesen. Det pyrolytiske karbon påfort front- og bakflatene 26 og 28 og på sideflatene 2h av protesen 10 kan også være polert for å redusere muligheten for irritasjon av vev som kan komme i glidende kontakt med disse flater under leddets bevegelse.
Substratet 12 fremstilles fortrinnsvis fra polykrystallinsk grafitt, for eksempel av typen "POCO AXF", som har en tetthet på ca. 1,9 g/cm0 0 og en gjennomsnittlig krystallittstbrrelse (Lc) på ca. 300 Ao og en isotropi på Bacon-skalaen på nær 1,0.
Grafitten har en Young-elastisitetsmodul på 0,119 x 10 6 kg/cm 2. Substratet er utformet i den fasong som protesen 10 har, og er belagt med et lag pyrolytisk karbon ca. 500 mikron tykt og som har en tetthet på ca. 1,9 g/cm-' og en elastisitetsmodul på 0,28 x 10 kg/cm p. Den sammensatte protese 10 har en effektiv elastisitetsmodul som ligger i området for naturlig levende ben som har en elastisitetsmodul mellom ca. 0,1^ x 10 6 og 0,28 x 10 6 kg/cm 2.
For å erstatte et odelagt kneledd fjernes operativt det natuiiige benleddparti som svarer til protesen .10, og den gjenværende ende formes for tilveiebringelse av en sammenpassende flate som vil passe.inn i det gjensidig låsende forhold med tapper og spor i flaten 18. Det ikke polerte pyrolytiske karbonbelegg 1<*>+ på flaten 18 er tjenlig for vekst av naturlig benvev og dettes befestigelse
til flaten, og dets nærvær fremmer mottagelse av erstatnings-
N protesen som en funksjonelt permanent del av det naturlige ben.
På grunn av at protesen har en stivhet som ligger nær opp til den for naturlig ben, tilveiebringes en utmerket forankring og protesen vil forbli permanent og oppfore seg snm naturlig ben. Da protesens 10 elastisitetsmodul er meget nær lik den for naturlig ben, vil benet og protesen festet dertil tilformes i elastisk harmoni, slik at protesens tendens til å arbeide seg Ibs reduseres sterkt.
Det er forutsatt at protesen 10 vil bli brukt sammen med en sammenpressende protese og således vil, for den utformning som er vist i fig. 1 og 2, hele kneleddet bli erstattet av proteser i de leddforbindende ender av både femur (lårben) og tibia (skinneben). Under bruk mot en sammenpassende karbonflate som ovenfor beskrevet, har den polerte, buede flate 16 av det pyrolytiske karbonbelegg en ytterst god slitasjemotstandsevne, slik at en ny operasjon på grunn av be'tennelse foranlediget av stov som- resultat av slitasje ikke blir nodvendig. Det er underforstått at hvis det er onskelig eller nodvendig bare å erstatte en del av kneleddet, f.eks. bare den del som tilhorer femur eller tibia, kan den svingbare sliteflate 16 fremstilles med forskjellig fasong for en heldigere funksjonering mot leddenden av den naturlige bendel av leddet. I
et slikt tilfelle vil det forstås at tilpasningen av fysikalske egenskaper til naturlig ben oppnådd gjennom bruken av det pyrolytisk karbonbelagte grafittsubstrat er overlegen overfor sammenpasningen av en naturlig benende (eller brusk) til kontakt med for eksempel et metall som har en elastisitetsmodul som kan være 10 ganger hoyere enn den naturlige benende. Hvor det er nodvendig å utbedre eller erstatte et kneledd som krever ytterligere erstatning av andre deler av naturlig ben, kan den illustrerte protese lett utvides i storrelse etter som det måtte være nodvendig.
I figur 3 er illustrert en stoteabsorberende hofteleddsprotese 80 som har en kuledel 82 fremstilt av polykrystallinsk karbonsubstrat 8<*>+ med et belegg 86 av pyrolytisk karbon og som er utformet slik at den kan være en effektiv erstatning for leddets naturlige bendel. Den ytre del av det pyrolytiske karbonbelegg høyglanspoleres polerskive og diamantstov for tilveiebringelse av en usedvanlig sterk sliteflate og for å redusere friksjon. Kulen 82 festes til et metallskaft eller trykkstang 90 ved hjelp av et egnet elastomermellomlag 92 som er adhesivt bundet til hver av delene.
Et vanlig problem med metalliske hofteleddproteser har vært
deres mangel på stotabsorberende egenskaper som naturlig ben har. Bruken av kulen 82 som er et karbonsubstrat med pyrolytisk karbonbelegg, reduserer denne mangel, men ytterligere stotdempning kan være onskelig i leddet. I den henseende tilveiebringer elastomermellomlaget 92 ikke bare binding mellom kulen og metallskaftet 90, men har også en stotabsorberende funksjon. Det stotabsorberende lag 92 kan fremstilles av polyetylen eller annet egnet gummilignende material og kan beskyttes mot kroppsvæsker,
ved en passende tetning 98 som holdes svakt sammenpresset mot både
kulen og metallskaftet ved fjærende stramning tilveiebragt av elastomerbefestigelsen..
Ved den ende som er motsatt kulen 82, boyer skaftet 90 seg under dannelse av en stang 100 som er innfort i og bundet til en konisk hylse 102 fremstilt av polykrystallinsk karbon som kan ha et ytre belegg av pyrolytisk karbon 10<*>+. En festeanordning 106 kan brukes på enden av stangen 100 for å sikre hylsen dertil. Den koniske hylse 102 med det pyrolytiske karbonbelegg 10^ er utmerket egnet for innfbring i lårbenet under tilveiebringelse av en usedvanlig god binding ved adhesjonen med naturlig benvev.
Alternativt kan hele protesen 80 fremstilles av et karbonsustrat hvorpå er påfort et pyrolytisk karbonbelegg, enten med eller uten det stotabsorberende trekk i den illustrerte utforelsesform.
Eksempel 1.
En. kneleddprotese, slik som den illustrert i fig. 1 og 2, ble maskinert fra polykrystallinsk grafitt ("P0C0 AXF"). Det maskinerte substrat ble utformet til tilstrekkelig storrelse for utbedring av kneleddet til en voksen mann av gjennomsnitts kroppsbygning. Substratet ble plassert i et roterende strengbur i et reaksjonskammer som dessuten inneholdt et partikkelsjikt. Substratet passet lost i strengburet slik at når dette roterte, forandret substratets kontaktpunkt eller -punkter med buret seg under rotasjon, hvorved det pyrolytiske karbonbelegg ble jevnt påfort.
Reaksjonskammeret ble oppvarmet til en temperatur på ca. 1350°C.
Når temperaturen i substratet nådde 1350°C, ble en strom av heliumgass og propan, slik at det var et partialtrykk av propan på 0,^- (totaltrykk 1 atm), innfort i reaksjonskaret og rettet mot substratets overflate som ble rotert i strengburet. Under disse betingelser ble propan spaltet og avsatt som et tett, isotropisk, pyrolytisk karbonbelegg på det polykrystallinske grafittsubstrats hele overflate. Under disse belegningsbetingelser var karbon-påforings-hastigheten ca. 5 mikron pr. minutt og propangasstrommen fortsatte inntil et isotropisk, pyrolytisk karbonbelegg på 500 mikrons tykkelse var påfort substratet. Etter at den foronskede tykkelse av karbonbelegget på substratet var oppnådd, ble propangasstrommen avstengt. Det belagte substrat ble kjblt ganske langsomt i he.liumgasstrommen og ble så fjernet fra reaksjonskammeret.
Protesens sliteflate cg front-, bakre og sideflater ble polert
til hoyglans under anvendelse av det diamantslipemiddel. Det pyrolytiske beleggs overflateruhet ble bevart på den benskjotende side som inneholdt knaster og spor, og dette lettet etableringen
av en sterk forbindelse mellom protesen og det naturlige ben. Målinger viste at det pyrolytiske karbon hadde en tetthet på ca. 1,9 g/cm-3, en LQ på ca. 30 Å og en BAF-verdi på ca. 1,1.
Eksempel II
Det ble fremstilt en kneleddprotese noyaktig som i eksempel I med unntagelse av at det ble tilveibragt en forskjellig struktur for oppnåelse av en fast forbindelse mellom protesen og det naturlige ben hvortil den skulle festes. I stedet for en serie tapper og spor som i protesene vist i fig. 1 og 2, ble substratet forsynt med bare en tapp plassert sentralt i den benskjotende side.
Imidlertid ble en strimmel av metallnetting (90% wolfram/10$ tantal-legering) viklet rundt og bundet til omkretsen av substratet slik at en side av nettingen ikke strakk seg helt opp til sliteflaten, men på den annen side overlappet utenfor den benskjotende side en distanse på ca. 12,7 mm. Påforing av det pyrolytiske karbonbelegg som i eksempel I tjente til ytterligere å binde nettingen til substratet og til å sveise hele protesen sammen som en enhet. Etter påforing av det pyrolytiske karbonbelegg som i eksempel I ble sliteflaten hbypolert under anvendelse av et diamantstbvslipemiddel.
Protesen kan innplantes ved å maskinere bensiden hvortil den skal ligge an, slik at den mottar tappen, og ved å smette den pyrolytisk karbonbelagte netting over utsiden av benenden. Nettingen kan festes ved hjelp av skruer eller stifter om onsket. Etterfølgende vekst av benvev til protesen, omfattende den benskjotende side og den overlappende netting, sorger for en stabil binding. Den teknikk som er illustrert i dette eksempel, kan også anvendes ved skjoting av andre typer proteser til naturlige bensegmenter.
Skjbnt figurene og eksemplene bare illustrerer visse spesifikke protesetyper, festemetoder og substratmaterialer, vil det forstås at også andre utforelsesformer kan anvendes. Således kan i tillegg til kne- og hofteledd andre ledd som finger-, albu- og skulderledd repareres eller erstattes ved pyrolytisk karbonbelagte proteser. Dessuten kan lange ben og andre som fingerben, rygghvirvler etc. utbedres eller helt eller delvis erstattes.
Skjbnt eksemplene spesielt beskriver påforing av pyrolytisk karbon ved bruk av propan, er det underforstått at andre hydrokarboner eller blandinger av sådanne kan anvendes for påforing av det pyrolytiske karbonbelegg på substratet. De variable i selve påfbringsprosessen kan anvendes for å variere egenskapene til det pyrolytiske karbonbelegg, eller tilpasse størrelsen, temperatur-stabilitetsområdet eller strukturen av substratmaterialet.
Flere spesielle metoder for befestigelse av proteser til naturlig ben er blitt demonstrert. Det pyrolytiske karbonbelegg antas å ha> en spesiell grad av ruhet eller overflateporbsitet i påfort stand, hvilket medvirker til fasthengning av voksende benvev til protesen. Som stotte for denne teori, som ikke er ment å være begrensende for oppfinnelsen, har mikrofotografier under bruk av elektronmikroskop vist at foretrukne pyrolytiske karbonbeleggpå-
foringer har en overflate som ved sterk forstorrelse ser ut som korall. Porøsiteten er bare overfladisk, idet hovedmassen av belegget er tett med et foretrukket tetthetsområde på fra ca. 1,9
g/cm^ til ca. 2,2 g/cm^.
Skjont et vidt område av substratmaterialer kan anvendes, er
slike som har en elastisitetsmodul i nærheten av naturlig bens elastisitetsmodul spesielt foretrukne for hel utskiftning av ben.
Kunstig grafitt er spesielt foretrukket. For gitterverkproteser
kan forskjellige ildfaste fibre og strenger formet til vevede,
tvinnede og ikke-vevede strukturer anvendes. Proteser laget av slike strukturer er også anvendbare, for tilveiebringelse av stotte for reparerte knuste eller syke ben, hvilken stotte ikke korroderes,
svekkes eller forårsaker betennelse, slik som tidligere anvendte typer, og som kan forbli permanent som en strukturell del av det helbredede bensegment. Hvor det ikke bnskes å bruke hbymodul-
metallfibre eller -nettinger, kan lavmodul-fibre som karbonfibre,
duk eller -netting belegges med pyrolytisk karbon for'tilveie-
bringelse av et lavmodul-gitterverk som har hby styrke og den biologiske forenlighet samt andre egenskaper til pyrolytisk karbon.
Claims (6)
1. Sammensatt ortopedisk ledd-proteseanordning for reparasjon eller erstatning av benstrukturen i en levende kropp, omfattende et ildfast substrat med passende forutbestemt form og med et damp-påfbrt pyrolytisk karbonbelegg som er biologisk forenlig med levende vev og har en tetthet (densitet) på minst 1.5 g/cm^
og en tykkelse på minst 25 mikron,karakterisert ved at substratet har en elastisitetsmodul på 0.12x10 - 0.28x10 kg/cm og at proteseanordningen har en flate innrettet til å forbindes med benvev og en leddet sliteflate, hvor det pyrolytiske karbonbelegg på sliteflaten er polert og hvor det pyrolytiske karbonbelegg på i det minste en del av benvev-forbindelsesflaten har en
overflateporbsitet som fremkommer ved på-dampingen og uten ytterligere bearbeiding.
2. Proteseanordning som angitt i krav 1,
karakterisert ved at benvev-forbindelsesflaten er utformet med et eller flere fremspring innrettet til å samvirke låsende med en tilsvarende tilformet overflate i benvevet.
3. Proteseanordning som angitt i krav 1 eller 2, karakterisert ved at det pyrolytiske karbonbelegg har BAF-verdi på maksimum 1.3. h.
Proteseanordning som angitt i krav 1-3? karakterisert ved at den del av overflaten av det pyrolytiske karbonbelegg som skal forbindes med ben e.l. vev er forsynt med oksygenbærende polare grupper.
5. Proteseanordning som angitt i krav 1 - <>>+, karakterisert ved at substratet er en polykrystallinsk grafitt.
6. Proteseanordning som angitt i krav 5?
karakterisert ved at grafitten er en isotropisk grafitt med en gjennomsnitts-krystallstorrelse på ca. 300 Å, en tetthet (densitet) på ca. 1.9 g/cm^ 6g en elastisitetsmodul på ca. 0.12x10<6> kg/cm<2>.
