Abstract
A box (1) for storing and/or transporting surgical items (2), comprising a box body (3) and a matching box cover (4), wherein the box (1) is arranged for receiving a storage holder (11) equipped. to keep a set (2) of surgical items in said storage holder (11), wherein the storage holder (11) is configured to be pressurized at an above—atmospheric pressure.
Description
A box for storing and/or transporting surgical items The invention relates to the field of logistics of surgical instruments.
More particularly, the invention relates to a box for transporting and/or storing surgical instruments, particularly but not only after sterilization of the instruments.
After every surgical operation the used surgical instruments are disinfected and sterilized.
The process of disinfection and sterilization takes place in a Central Sterilization Department (CSD). After the disinfection step and before the sterilization step, the surgical instruments are packed in special steam permeable sterilization paper, usually made of polypropylene.
The packed surgical instruments are then sterilized in a sterilizer (autoclave) and stored in a sterile storage room ready to be used.
After being used in an Operating Room (OR), the contaminated surgical instruments are returned to the CSD and the process starts again.
Another known way of sterilizing used surgical instruments is to place the surgical instruments in boxes, made of stainless steel or aluminum, wherein sterilizing air or steam is introduced through a special filter in order to sterilize the surgical instruments.
However, such a filter needs to be regularly replaced interfering with the operational availability of such boxes.
The current way of routing surgical instruments involves a complex logistics and stock management.
The CSD can be located inside or outside the hospital where the operation room is located.
Some independent treatment centers or private clinics, for example, do not have a CSD themselves and outsource the disinfection and the sterilization process, further contributing to the complexity of the logistics and stock management of such surgical instruments.
The current way of routing the surgical instruments presents a number of limitations and/or drawbacks, to note: — The sterile surgical instruments must be stored in a designated sterile area, usually equipped with a laminar airflow.
- 2 = — This entails additional costs for the design, construction and maintenance. — A sterile storage takes up a lot of space. — The containers have no sterility indicator and can oxidize over time.
This in turn has adverse effects for the disinfecting washing machines. — The functionality of the filters must be regularly checked and validated, incurring additional costs. — Currently logistics/transport takes place by means of general distribution/shipment containers, available from logistics traders, and have no further medical quality marks.
The present invention aims to overcome the above disadvantages and to provide a new solution for storage and/or logistics of a sterilized set of surgical instruments, or at the very least the current invention aims to provide a useful alternative for storing and/or transporting sterilized surgical items.
To this end a box and a method for handling sterilized surgical items is proposed having the features of one or more of the appended claims.
According to a first aspect of the invention the box comprises a box body and a matching box cover, wherein the box is arranged for receiving a storage holder equipped to keep a set of packed sterilized surgical items in the storage holder, and wherein the storage holder is configured to be pressurized at an above-atmospheric pressure.
It will be appreciated that “above-atmospheric” is understood to be a pressure exceeding the local environmental pressure in which the box subsists.
The set of packed sterilized surgical items is usually embodied as a tray holding one or more surgical instruments, which are sterilized and subsequently packed.
The set of packed sterilized surgical items is thereafter placed in the storage holder, which is subsequently pressurized.
The storage holder can thus provide a sterile storage wherein the sterilized surgical items can maintain their sterile condition as long as the overpressure in the storage holder remains.
The box, containing the storage holder, can be kept in a generic, less expensive, and non-sterile internal or
- 3 = external storage. When the box and the storage holder are opened the sterile surgical instruments can be taken out and removed from their packing and are ready for use in the operation room. The storage holder is for single use and it is disposed after the surgical instruments are removed therefrom. After the surgical operation, the contaminated surgical instruments are placed back in the box, the box is closed using for instance tie wraps and/or separate snap finger clips at each corner, and the box can then be sent back to the CSD properly closed and secured. The box, together with the contaminated surgical instruments, are then systematically disinfected in a disinfecting washing machine. The box can then be reused for receipt of a new storage holder in which a new set of sterilized surgical instruments can be placed.
To facilitate the above process, it is preferable that the box cover comprises a reversibly movable locking plate, which defines an inner volume of the box together with the box body, wherein the inner volume is tailored to receive the storage holder.
In one embodiment of the invention, the storage holder is a flexible bag, although this is not the only option.
Preferably the storage holder comprises an air- impermeable material, and is provided with a closable open end for placing the set of packed sterilized surgical items inside said bag.
More suitably, the storage holder is configured to be pressurized by receiving a gas. Pressurization of the storage holder occurs preferably through a lockable valve, wherein said lockable valve is configured to fit through an opening in a wall of the box body, such that it remains accessible from the outside of the box. The gas is preferably micro-organism destructive. The gas may be introduced into the storage holder unfiltered such as directly from a cleanroom via a pump.
However, in order to guarantee the cleanliness of the gas pumped inside the storage holder also outside of cleanroom conditions, the box maybe arranged so that a gas filter can be placed between the bag and the pump (e.g. a standard 0.2 micron hydrophobic or hydrophilic air filter). Possibly, this
- 4 - filter is provided as part of an inlet or valve of the storage holder.
Advantageously, the storage holder comprises a plurality of chambers configured to immobilize the set of packed surgical items, when the storage holder is pressurized. This has the benefit of fixing the set of packed surgical items inside the box.
More advantageously, the storage holder is configured to engage the locking plate when said storage holder is pressurized, causing movement of the locking plate towards the box cover.
In a preferred embodiment of the invention, the locking plate comprises at least one upper protrusion, and/or at least one flexible lateral protrusion.
Suitably, the box body and the box cover comprise a series of openings, wherein said openings are configured to be coaxially aligned when the storage box is assembled.
More suitably, the at least one flexible protrusion of the locking plate is configured to lock the box body with the matching box cover by protruding through the series of openings of the box body and the box cover when the storage holder is pressurized, wherein the storage holder engaging the locking plate causes movement of the locking plate towards the box cover. Equally, the at least one flexible protrusion of the locking plate is configured to unlock the box body and the matching box cover by retracting from the series of openings of the box body and the box cover when the storage holder is depressurized, causing the movement of the locking plate away from the box cover.
It is beneficial that the protrusion level of the at least one protrusion into the series of openings of the box body and the box cover is dependent on a pressure level in the storage holder.
In an advantageous aspect of the invention, the at least one upper protrusion of the locking panel is configured to protrude into at least one upper opening of the box cover. The protrusion level of the at least one upper protrusion into the at least one upper opening of the box cover is dependent on the movement of the locking plate caused by the pressure
— 5 = level in the storage holder engaging the locking plate. This simplifies the verification of the pressure level inside the box since a quick visual inspection of the level of protrusion of said upper protrusion is enough for verifying the pressure level inside the storage holder, and in consequence, verifying the integrity of the sterilization of the items stored inside the storage holder. For safe transport and storage of the boxes and the sterilized surgical items therein, the box comprises at least one bumper made of silicone or EPDM rubber, provided to an outer and/or inner surface of the box body and/or the box cover. This prevents the stored items from sliding during transport.
The invention is also embodied in a method of using the box, for storing and/or transporting sterilized items, comprising the steps of: — placing the sterilized items in the storage holder through a closable open end of said storage holder; — closing the open end; — placing the storage holder holding the sterilized items inside the box body; — covering the box body with the matching box cover; — locking the box cover with the box body; and — pumping a gas inside the storage holder through a lockable valve accessible through a wall of the box body so as to pressurize the storage holder. Advantageously, the method also comprises the step of monitoring a pressure level inside the storage holder by: — visually monitoring the level of protrusion of the at least one flexible protrusion through the series of openings of the box body and the box cover; and/or — visually monitoring the level of protrusion of at least one upper protrusion of the locking plate through at least on upper opening of the box cover.
The invention will hereinafter be further elucidated with reference to the drawing of an exemplary embodiment of a deskinning apparatus according to the invention that is not limiting as to the appended claims.

Figure 1 shows a box 1 according to the invention comprising a box body 3, a box cover 4, a locking plate 7, and a storage holder 11 for holding a set 2 of packed sterilized surgical items. The locking plate 7 is removably attachable to the box cover 4 and the box body 3. The storage holder 11, holding the set 2 of packed sterilized surgical items, is tailored to fit inside the box body 3 below the locking plate
7. The locking plate 7 comprises upper protrusions 8 designed to protrude through upper openings 6 of the box cover 4 when the box 1 is assembled. The locking plate 7 also comprises flexible protrusions 9 designed to protrude through openings 5, 5’ of the box cover 4 and through openings 5” of box body 3 as depicted in figures 2 and 3.
In figure 4 it can be further observed that the box cover 4 comprises outer later openings 5’ and inner openings 5 that are coaxially aligned with the openings 5% of the box body 3 when the box is assembled.
When the storage holder 11 is pressurized, as depicted in figure 3, it engages and presses against the box body 3 and more importantly, it also engages and presses against the locking plate 7 causing said locking plate 7 to move towards the box cover 4. In consequence, the flexible protrusions 9 will protrude through the aforementioned openings 5, 5’, 5” of the box body 3 and the box cover 4, respectively.

- 7 = 7 will protrude through the aforementioned upper openings 6 of the box cover 4, caused by the storage holder 11 engaging the locking plate 7 and pressing it to move towards the box cover

This latter mechanism creates a visual indicator of the overpressure still being present inside the storage holder 11, since the extent of protrusion through the openings 6 is dependent on the pressure level inside the storage holder 11, as depicted in figure 5. Accordingly this provides a visual indicator of the integrity of the sterilization of the surgical items 2 held inside the storage holder 11. A visual indication of the overpressure inside the storage holder 11 can also be derived from the extent at which the flexible protrusions 9 will protrude through the openings 5, 5’, 5” of the box body 3 and the box cover 4, respectively.

6. The outer bottom face of the box body 3 form-fits the outer upper face of the box cover 4. In figure 6 this is portrayed by the box body 3 comprising a male part, and the box cover 4 comprising a female part of a male-female connector.
Naturally, the reverse is also possible.
Although the invention has been discussed in the foregoing with reference to an exemplary embodiment of the box of the invention, the invention is not restricted to this particular embodiment which can be varied in many ways without departing from the invention. The discussed exemplary embodiment shall therefore not be used to construe the appended claims strictly in accordance therewith. On the contrary the embodiment is merely intended to explain the wording of the appended claims without intent to limit the claims to this exemplary embodiment. The scope of protection of the invention shall therefore be construed in accordance with the appended claims only, wherein a possible ambiguity in the wording of the claims shall be resolved using this exemplary embodiment.

1. A box (1) for storing and/or transporting surgical items (2), comprising a box body (3) and a matching box cover
- 8 - (4), characterized in that the box (1) is arranged for receiving a storage holder (11) equipped to keep a set (2) of surgical items in said storage holder (11), wherein the storage holder (11) is configured to be pressurized at an above-atmospheric pressure.
2. The box (1) according to claim 1, characterized in that the box cover (4) comprises a reversibly movable locking plate (7), which defines an inner volume of the box (1) together with the box body (3), wherein the inner volume is tailored to receive the storage holder (11).
3. The box (1) according to claim 1 or 2, characterized in that the storage holder (11) comprises an air-impermeable material, and is provided with a closable open end (13) for placing the set (2) of surgical items inside the storage holder (11).
4. The box (1) according to anyone of claims 1-3, characterized in that the storage holder (11) is configured to be pressurized by receiving a gas through a lockable valve (14).
5. The box (1) according to anyone of claims 1-4, characterized in that the storage holder (11) comprises a filter.
6. The box (1) according claim 4 or 5, characterized in that the lockable valve (14) is configured to fit through an opening (15) of the box body (3).
7. The box {1) according to anyone of claims 1-6, characterized in that the storage holder (11) comprises a plurality of chambers (12) configured to immobilize the set (2) of surgical items when the storage holder (11) is pressurized.
8. The box (1) according to anyone of claims 1-7, characterized in that the storage holder (11) is configured to engage the locking plate (7) when said storage holder (11) is pressurized, causing movement of the locking plate (7) towards the box cover (4).
9. The box (1) according to anyone of claims 2-8, characterized in that the locking plate (7) comprises at least one upper protrusion (8), and/or at least one flexible lateral protrusion (9).

The box (1) according to anyone of claims 1-9, characterized in that the box body (3) comprises at least one lateral opening (5), and the box cover (4) comprises at least one outer opening (5') and/or at least one inner opening (57), wherein said openings (5, 57, 57) are configured to be coaxially aligned when the storage box (1) is assembled.
11. The box (1) according to any one of claims 1-10, characterized in that the at least one flexible protrusion (9) of the locking plate (7) is configured to lock the box body {3) with the matching box cover (4) by protruding through the at least one lateral opening (5) of the box body (3) and the at least one outer and/or inner opening (5", 5”) of the box cover (4) when the storage holder (11) is pressurized, wherein the storage holder (11) engaging the locking plate (7) causes movement of the locking plate (7) towards the box cover (4).

The box (1) according to any one of claims 1-11, characterized in that the at least one flexible protrusion (9) of the locking plate (7) is configured to unlock the box body (3) and the matching box cover (4) by retracting from the at least one lateral opening (5) of the box body (3) and the at least one outer and/or inner opening (57, 5”) of the box cover (4) when the storage holder (11) is depressurized, causing the movement of the locking plate (7) away from the box cover (4).
13. The box (1) according to any one of claims 1-12, characterized in that the protrusion level of the at least one protrusion (9) into the at least one lateral opening (5) of the box body (3) and into the at least one outer and/or inner opening (5, 5”) of box cover (4) is dependent on a pressure level in the storage holder (11).
14. The box (1) according to any one of claims 1-13, characterized in that the at least one upper protrusion (8) of the locking panel (7) is configured to protrude into at least one upper opening (6) of the box cover (4).
15. The box (1) according to any one of claims 1-14, characterized in that the protrusion level of the at least one upper protrusion (8) into the at least one upper opening (6) of the box cover (4) is dependent on the movement of the locking plate (7) caused by the pressure level in the storage holder (11) engaging the locking plate (7).

16. The box (1) according to any of the preceding claims 1-15, characterized in that the box (1) comprises at least one bumper, such as made of silicone or EPDM rubber, provided to an outer and/or inner surface of the box body (3) and/or the box cover (4).
17. Assembly of two or more boxes (1) according to any one of claims 1-16, characterized in that an upper side (17) of any box (1) is configured to stackably form-fit a lower side (18) of any other box (1).
18. Method for using a box (1) for storing and/or transporting sterilized surgical items (2) wherein the box (1) comprises: — a box body (3); — a matching box cover {4) comprising a reversibly movable locking plate (7); and — a storage holder (11) comprising an air-impermeable material; characterized in that the method comprises the steps of: — placing the set (2) of sterilized surgical items in the storage holder (11) through a closable open end (13) of said storage holder (11); — closing the open end (13); — placing the storage holder (11) holding the set (2) of sterilized surgical items inside the box body (3); — covering the box body (3) with the matching box cover (4); — locking the box cover (4) with the box body (3); and — pumping a gas inside the storage holder (11) through a lockable valve (14) accessible through a wall of the box body (3) so as to pressurize the storage holder (11).
19. Method for using a box (1) according to claim 15, wherein the locking plate (7) comprises at least one flexible protrusion (9) protruding through at least one lateral opening {5) of the box body (3) and through at least one opening (5, 5”) of the box cover (4), characterized in that the method comprises the step of monitoring a pressure level inside the storage holder (11) by: — monitoring the level of protrusion of the at least one flexible protrusion (9) through the at least one opening
- 11 - (5) of the box body (3) and the at least one lateral opening (5, 5”) of the box cover (4); and/or — monitoring the level of protrusion of at least one upper protrusion (8) of the locking plate (7) through at least an upper opening (6) of the box cover (4).
