Abstract

Inventia se refera la medicina, in special la traumatologie si ortopedie, si poate fi aplicata in tratamentul leziunilor coloanei vertebrale, si anume a fracturilor prin compresiune ale corpurilor vertebrale. Metoda de spondilodeza corectoare consta in aceea ca preventiv, conform datelor radiotomografiei computerizate se determina inaltimea leziunii corpului vertebrei sau a vertebrelor si diametrul anterior-posterior al corpurilor vertebrelor adiacente, in conformitate cu care se pregatesc alogrefe osoase ovale cuneiforme din diafiza oaselortubulare. Apoi se efectueaza un acces chirurgical anterior sau anterior-lateral la vertebra lezata, precum si la cele adiacente. Se efectueaza razuirea discurilor lezate si extirparea partii lezate a corpului vertebrei, se modeleaza patul pentru alogrefa osoasa. Pe suprafata anterioara a corpului vertebrei lezate, precum si a celor adiacente se introduc suruburi cu perforarea ambelor straturi corticale. Prin capurile suruburilor se trece o tija longitudinala, care preventiv se indoaie conform curburilor fiziologice ale regiunii respective a coloanei vertebrale. Apoi tija longitudinala indoita se fixeaza flexibil de capurile suruburilor cu partea concava orientata spre suprafata laterala a corpurilor vertebrelor, dupa care tija longitudinala indoita se roteste la 90° in directie posterioara in regiunea lombara si anterioara in regiunea toracica. Cavitatea alogrefei osoase se umple cu tesut osos spongios din fragmentele extirpate ale corpului vertebrei lezate sau cu un material osteoinductiv. Se efectueaza distractia cu depasirea cu 2-3 mm a diastazei intervertebrale fiziologice a fiecarui segment vertebral supus interventiei. Apoi in fiecare spatiu se introduce cate o alogrefa cuneiforma in patul modelat, se slabeste distractia, dupa care plaga se sutureaza pe straturi.The invention relates to medicine, in particular to traumatology and orthopedics, and can be applied in the treatment of spinal cord injuries, namely compression fractures of the vertebral bodies. The corrective spondylodesis method consists in the fact that, according to the data of the computerized radiography, the height of the lesion of the vertebral body or of the vertebrae and the anterior-posterior diameter of the bodies of the adjacent vertebrae are determined, according to which cuneiform oval bone allografts from the bone-diaphysis are prepared. Then a surgical or anterior-lateral access is performed to the injured vertebrae, as well as to the adjacent ones. The injured discs are scratched and the injured part of the vertebral body is removed, the bed for bone allograft is modeled. On the anterior surface of the body of the injured vertebrae, as well as of the adjacent ones, screws are introduced with perforating both cortical layers. Through the heads of the screws a longitudinal rod is passed, which preventively bends according to the physiological curves of the respective region of the spine. Then the bent longitudinal rod is flexibly fixed by the heads of the screws with the concave side facing the lateral surface of the vertebral bodies, after which the bent longitudinal rod rotates 90 ° in a posterior direction in the lumbar region and anterior in the thoracic region. The bone allograft cavity is filled with spongy bone tissue from the excised fragments of the injured vertebral body or an osteoinductive material. The distraction is performed by exceeding by 2-3 mm the physiological intervertebral diastasis of each vertebral segment subjected to the intervention. Then in each space a cuneiform allograft is introduced into the modeled bed, the distraction is weakened, after which the wound is sutured on the layers.
