Abstract
Invenţia se referă la medicină, în special la oncologie şi poate fi aplicată pentru efectuarea tiroidectomiei în caz de tumoare malignă răspândită pe ţesuturile moi ale părţii anterioare a gâtului, pe trahee şi cartilajul tiroid, mai sus de istmul glandei tiroide.Esenţa invenţiei constă în aceea că se efectuează o incizie a pielii şi ţesutului subcutanat în formă de arc de la nivelul treimii medii a gâtului, apoi pe marginea anterioară a muşchiului sternocleidomastoidian în jos până la incizura jugulară şi se excizează zona de piele afectată deasupra tumorii. Apoi se deschid tecile musculare ale muşchilor sternocleidomastoidieni cu separarea lor, se rezecă în partea inferioară a glandei tiroide fascia a treia a gâtului (aponevroza cervicală Richet) şi muşchii sternotiroidian, tirohioidian şi sternohioidian. Apoi se efectuează revizia tumorii, se efectuează mobilizarea lobului glandei tiroide clinic neafectat, se ligaturează şi se excizează artera tiroidiană superioară, medie şi inferioară. Se mobilizează nervul recurent, după care se înlătură în bloc lobul glandei tiroide neafectat, muşchii anteriori ai gâtului, ţesutul paratraheal, inelele traheale afectate şi porţiunea afectată a nervului recurent, care apoi se suturează şi se restabileşte continuitatea lui. Apoi se separă şi se înlătură blocul lobului afectat, după care se efectuează hemostaza definitivă, se drenează şi se suturează ţesuturile pe straturi în caz dacă defectul nu este mare, iar în cazul unui defect masiv, se efectuează plastia cutanată.
