Abstract

Inventia se refera la medicina, in special la oncologie si poate fi aplicata pentru efectuarea tiroidectomiei in caz de tumoare maligna raspandita pe tesuturile moi ale partii anterioare a gatului, pe trahee si cartilajul tiroid, mai sus de istmul glandei tiroide. Esenta inventiei consta in aceea ca se efectueaza o incizie a pielii si tesutului subcutanat in forma de arc de la nivelul treimii medii a gatului, apoi pe marginea anterioara a muschiului sternocleidomastoidian in jos pana la incizura jugulara si se excizeaza zona de piele afectata deasupra tumorii. Apoi se deschid tecile musculare ale muschilor sternocleidomastoidieni cu separarea lor, se rezeca in partea inferioara a glandei tiroide fascia a treia a gatului (aponevroza cervicala Richet) si muschii sternotiroidian, tirohioidian si sternohioidian. Apoi se efectueaza revizia tumorii, se efectueaza mobilizarea lobului glandei tiroide clinic neafectat, se ligatureaza si se excizeaza artera tiroidiana superioara, medie si inferioara. Se mobilizeaza nervul recurent, dupa care se inlatura in bloc lobul glandei tiroide neafectat, muschii anteriori ai gatului, tesutul paratraheal, inelele traheale afectate si portiunea afectata a nervului recurent, care apoi se sutureaza si se restabileste continuitatea lui. Apoi se separa si se inlatura blocul lobului afectat, dupa care se efectueaza hemostaza definitiva, se dreneaza si se sutureaza tesuturile pe straturi in caz daca defectul nu este mare, iar in cazul unui defect masiv, se efectueaza plastia cutanata.The invention relates to medicine, in particular to oncology and may be applied for thyroidectomy in the case of malignant tumor spreading on the soft tissues of the anterior part of the neck, on the trachea and the thyroid cartilage, above the isthmus of the thyroid gland. The essence of the invention consists in the fact that an incision of the skin and subcutaneous arch tissue in the middle third of the neck, then on the anterior edge of the sternocleidomastoid muscle down to the jugular incision and the affected skin area is excised above the tumor. Then the muscle tissues of the sternocleidomastoid muscles are opened with their separation, resected in the lower part of the thyroid gland of the third fascia of the neck (Richet cervical aponeurosis) and the sternothyroid, thyroid and sternohioid muscles. Then the tumor is reviewed, the mobilization of the clinically unaffected thyroid gland is mobilized, the upper, middle and lower thyroid arteries are ligated and excised. The recurrent nerve is mobilized, after which the lobe of the unaffected thyroid gland is removed, the anterior neck muscles, the paratracheal tissue, the affected tracheal rings and the affected portion of the recurrent nerve, which are then sutured and restored to its continuity. Then the block of the affected lobe is separated and removed, after which definitive haemostasis is performed, the tissues are drained and sutured on the layers in case the defect is not large, and in case of a massive defect, the cutaneous plasticity is performed.
