Abstract
Изобретение относится к медицине, в частности к травматологии и ортопедии, реконструктивной хирургии и может быть использовано для восстановления дефектов длинных трубчатых костей.Сущность изобретения состоит в том, что выполняют в два этапа, а именно на первом этапе выполняют разрез на уровне голени, на передне-медиальной стороне, от большого бугорка большеберцовой кости и до дистальной 1/3 голени, затем удаляют надкостницу большеберцовой кости, формируют кольцевой костный дефект длиной 3,0 см в средней 1/3 голени, костные фрагменты располагают в правильном положении и фиксируют с помощью металлической пластины с шурупами, а полость дефекта заполняют медицинским цементом c антибиотиком, после чего выполняют лаваж раны антисептическими растворами и рану ушивают послойно. Через 2 недели проводят второй этап, который влючает разрез мягких тканей на том же уровне, удаляют цементную прокладку, а оставшуюся полость заполняют трубчатым кортикальным деминерализованным костным трансплантатом и затем рану ушивают послойно, а местно инокулируют аллотрасплантат содержащий культуру мезенхимальных стволовых клеток с концентрацией 4,5 x 106/мл, затем на 6-й неделе делают разрез на том же уровне удаляют металлическую пластину и шурупы, а рану ушивают послойно.The invention relates to medicine, in particular to traumatology and orthopedics, reconstructive surgery and can be used to restore defects in long tubular bones. The essence of the invention is that it is performed in two stages, namely, at the first stage, an incision is made at the level of the lower leg, in the anterior -the medial side, from the greater tubercle of the tibia to the distal 1/3 of the tibia, then the periosteum of the tibia is removed, an annular bone defect with a length of 3.0 cm is formed in the middle 1/3 of the tibia, the bone fragments are placed in the correct position and fixed with a metal plates with screws, and the cavity of the defect is filled with medical cement with an antibiotic, after which the wound is lavaged with antiseptic solutions and the wound is sutured in layers. After 2 weeks, the second stage is carried out, which includes the incision of soft tissues at the same level, the cement pad is removed, and the remaining cavity is filled with a tubular cortical demineralized bone graft and then the wound is sutured layer by layer, and the allograft containing the culture of mesenchymal stem cells with a concentration of 4.5 is locally inoculated. x 106 / ml, then at the 6th week an incision is made at the same level, the metal plate and screws are removed, and the wound is sutured in layers.
