Išradimas priklauso medicinos, tiksliau širdies ritmo nustatymo, matavimo ir registracijos sričiai.The invention relates to the field of medicine, more particularly to the determination, measurement and recording of heart rate.
Norint atlikti širdies ritmo rodiklių analizę, būtina ilgalaikėse RR intervalų reikšmių sekose (ritmogramose) išskirti sinusinį ritmą atitinkančias RR intervalų reikšmes iš ektopinių ritmų (ritmo sutrikimų) ir neteisingai išmatuotų (techninių triukšmų) intervalų reikšmių. RR intervalai gaunami iš elektrokardiosignalo, išmatavus laiko trukmes tarp gretimų širdies susitraukimų - atstumus tarp gretimų QRS komplekso R dantelių. Ritmograma - tai vertikalių atkarpų eilė, kurioje kiekviena atkarpa atitinka RR intervalą. Atkarpos ilgis tiesiai proporcingas RR intervalo trukmei (fig.la). Gretimų RR intervalų reikšmės paprastai viena nuo kitos skiriasi. Esant sinusiniam ritmui, šis skirtumas neviršija 100 - 150 ms. Jei šis skirtumas yra didesnis - tai bus ritmo sutrikimai. Ritmo sutrikimai ir techniniai triukšmai gali pasitaikyti pavieniai ir serijomis. Atliekant tiriamajam (pacientui) įvairius funkcinius mėginius (ramybės būsena, aktyvi ortostazė, veloergometrija, nakties miegas) ilgalaikėse ritmogramose dažnai pasitaiko RR intervalų reikšmės, atitinkančios ritmo sutrikimus bei techninius triukšmus. Visus juos reikia atpažinti, pažymėti ir prieš analizuojant sinusinį širdies ritmą, jų reikšmes reikia pervesti į sinusinio ritmo reikšmes (atlikti interpoliaciją).To perform heart rate analysis, it is necessary to distinguish between sinus rhythm RR interval values in long-term RR interval values (rhythmograms) and ectopic rhythms (rhythm disorders) and incorrectly measured (technical noise) intervals. The values of the adjacent RR intervals are usually different. At sinus rhythm, this difference does not exceed 100 - 150 ms. If this difference is greater - it will be rhythm disturbances. Arrhythmias are very diverse: ventricular and atrial extrasystoles, paroxysms, bigemias, trigeminisms and so on. t. If the RR interval values are lower or higher than the possible heart rate values, they are considered incorrectly measured RR intervals or technical noise. Rhythm disturbances and technical noises can occur singly and in series. In the case of various functional samples (resting state, active orthostasis, veloergometry, night sleep) of the subject (patient), RR interval values corresponding to rhythm disturbances and technical noise are often found in long-term rhythmograms. All of these should be recognized, noted, and their values converted to sinus rhythm (interpolation) before being analyzed.
Yra žinomi įvairūs būdai ektopiniams ritmams išskirti. Patentuose JP9173310A, JP6022915A, US5794624, US5645070, US5630425 aprašomi būdai ritmo sutrikimamsi nustatyti remiasi elektrokardiosignalo bangų, pagrindinai QRS kompleksų, formų atpažinimu ir klasifikavimu į normalius bei įvairių rūšių ekstrasistolinius, triukšminis ir t.t. kompleksus. Minėti būdai turi trūkumą, nes jie išskiria tik vienos rūšies ritmo sutrikimus, ir jie nėra tinkami ilgiems, besitęsiantiems kelias valandas, tyrimams nes jų metu neišvengiama triukšmų, o kartais atsiranda įvairių tipų ritmo sutrikimų. Patente US 5357970 pateikiamas būdas, kai širdies ritmo dažnio reikšmės suskirstomos į zonas (langus) ir ieškoma dominuojanti zona. Į ją patenkantys elementai priskiriami normaliam ritmui, o nepatenkantys - ritmo sutrikimams arba techniniams triukšmams. Patente JP5317277A RR intervalų sekos elementai surikiuojami mažėjimo ar didėjimo tvarka ir ieškomos ribos, skiriančios įvairius ritmus atitinkančias zonas. Patente JP4090744A siūlomas būdas, kuriame RR intervalai rūšiuojami ne tik pagal jų trukmę, bet ir pagal gretimų reikšmių santykį. Patentuose US6026320, JP7284482A įvertinamos RR intervalų sekų spektrinės charakteristikos ir pagal jas nustatomi ritmų tipai. Visi šie paminėti būdai yra nepatogūs atliekant vizualinį rezultatų įvertinimą, dirbant su ilgalaikėmis RR intervalų sekomis.Various ways of isolating ectopic rhythms are known. Išradimo tikslas yra iš ilgalaikių RR intervalų reikšmių sekų (ritmogramų), susidedančių iš sinusinio ir ektopinių ritmų bei techninių triukšmų, išskirti norimą ritmą kad galima būtų jį atvaizduoti ir atlikti tolimesnę jo analizę. Norint suformuoti ilgalaikę RR intervalų seką (ritmogramą), reikia sustiprintą elektrokardiosignalą įvesti į kompiuterį, išskirti jame QRS kompleksus, išmatuoti RR intervalus ir kaupti jų reikšmių seką atmintyje. Siūlomame būde RR intervalų reikšmių poros yra atvaizduojamos monitoriaus ekrane taškų aibe. Kiekviena gretimai esančių RR intervalų sekos RR, ir RR,., pora atvaizduojama tašku kvadratinės formos diagramoje su stačiakampe koordinačių sistema, pritaikyta prie RR intervalų reikšmių sekos minimalios ir maksimalios reikšmių arba prie realiai galimų RR intervalų sekos minimalios ir maksimalios reikšmių. In order to form a long-term sequence (rhythm) of RR intervals, it is necessary to input the amplified electrocardiogram into the computer, extract QRS complexes, measure the RR intervals, and store their sequence of values in memory. In the proposed method, pairs of RR interval values are displayed on the monitor screen in a set of dots. Each pair of adjacent RR intervals RR, and RR,., Is represented by a point on a square chart with a rectangular coordinate system adjusted to the minimum and maximum values of the RR interval values, or to the real and minimum RR intervals. By considering only sinus rhythm, the values of ectopic rhythms and intervals of technical noise are artificially converted to sinus rhythm values (9).
