Abstract
본 발명은 신장질환을 진단하거나 예후를 예측할 수 있는 조성물, 키트, 및 방법에 관한 것으로, 보다 구체적으로 사구체여과율과 혈액요소수치의 두가지 임상 양상과 관련된 유전자위를 분석하고 LD block 및 사후 가능성을 이용한 통계 데이터를 통해 유전자 WDR72 (WD repeat-containing protein 72), IGF1R (Insulin-like growth factor 1 receptor) 및 GNAS (guanine nucleotide binding protein)가 만성 신장병 발생 위험인자 바이오마커로 활용될 수 있으며, 이들 유전자 이상을 동반한 대상체의 경우 신장질환과 관련된 지표들인 혈청 크레아티닌 또는 단백뇨가 확인되기 전부터 약물 등의 적극적인 치료를 통해 질병 발생 위험을 조절할 수 있다.
Description
본 발명은 신장질환 예측 또는 진단용 바이오마커 및 이를 이용한 신장질환의 예측 또는 진단 방법에 관한 것이다.
신장은 생체의 항상성 (homeostasis)을 유지하는 중요한 장기로, 약 100만개의 네프론 (nephron)이라는 기본구조로 이루어지며, 하나의 네프론은 사구체라고 불리우는 미세한 모세혈관 덩어리와 세뇨관으로 구성된다. 사구체에서의 여과, 세뇨관에서의 재흡수와 분비 과정을 통해 노폐물과 불필요하게 많은 수분 및 무기염류를 오줌으로 만들어 내보냄으로써, 체내 체액량, 혈액 내의 이온 농도와 pH를 조절하고, 대사성 노폐물, 독소, 약물 등의 노폐물을 배설하며, 혈압 조절 및 기타 대사성, 내분비 기능을 수행한다.
이러한 신장이 배설, 조절, 대사 및 내분비적 기능을 정상적으로 수행하지 못하고 전체적으로 기능이 저하되거나, 이상이 초래된 상태를 신장질환이라고 한다. 넓은 의미의 신장질환은 만성적인 모든 신장병이 포함되고, 좁은 의미로는 원인이 뚜렷하지 않고 기질적 변화와 사구체 여과 기능이 저하되어 가는 질환들을 의미한다. 신장의 손상으로 인한 기능의 저하는 신장 및 관련 구조의 증대, 신장의 위축, 체액량의 변화, 전해질 불균형, 대사성 산증, 가스교환장애, 항감염 기능 손상, 요독성 독소의 축적 등을 초래한다.
만성 신장질환 또는 만성 신부전으로 알려진 만성 신장병 (CKD)은 신장 장애 또는 신장 기능 저하가 수개월 또는 수년의 기간에 걸쳐 지속되는 병태이며, 만성 신장병에 포함되는 질환으로는 당뇨병성 신장병이 있다. 만성 신장병이 발생하였는지 진단하는 방법은 혈청 크레아티닌과 소변 알부민을 측정하는 것이 알려져 있다.
그러나, 소변에서 수득되는 여러 전사체, 대사체 수준을 측정하는 것은 측정할 때마다 검사 수치가 일정하지 않고 변하기 때문에 일관적인 결과를 얻기 어렵다. 또한, 소변은 섭취하는 음식의 대사 산물에 영향을 크게 받을 수밖에 없기 때문에 한계가 있다. 반면에, 환자의 유전자 (DNA)는 변하지 않기 때문에, 병의 진행이 시작되기도 전에 향후 질병 발생 위험 등을 예측할 수 있다.
이러한 배경 하에, 본 발명자들은 신장질환의 예측을 위한 바이오마커로 활용될 수 있는 유전자를 찾기 위해 예의 연구한 결과, 사구체여과율과 혈액요소수치의 두가지 임상 양상과 관련된 유전자위를 분석하고 LD block 및 사후 가능성을 이용한 통계 데이터를 통해 유전자 WDR72 (WD repeat-containing protein 72), IGF1R (Insulin-like growth factor 1 receptor) 및 GNAS (guanine nucleotide binding protein)를 선별하였고, 이들이 신장질환 발생 위험인자 바이오마커로 활용될 수 있음을 확인함으로써 본 발명을 완성하였다.
본 발명의 목적은 WDR72 (WD repeat-containing protein 72), IGF1R (Insulin-like growth factor 1 receptor) 및 GNAS (guanine nucleotide binding protein)로 이루어진 군 중에서 선택되는 하나 이상의 유전자에 특이적인 프라이머 또는 프로브를 포함하는 신장질환의 예측 또는 진단용 조성물을 제공하는 것이다.
본 발명의 다른 목적은 WDR72 (WD repeat-containing protein 72), IGF1R (Insulin-like growth factor 1 receptor) 및 GNAS (guanine nucleotide binding protein)로 이루어진 군 중에서 선택되는 하나 이상의 유전자에 특이적인 프라이머 또는 프로브를 포함하는 신장질환의 예측 또는 진단용 키트를 제공하는 것이다.
본 발명의 또 다른 목적은 대상체로부터 수득한 DNA에서 유전자 WDR72 (WD repeat-containing protein 72), IGF1R (Insulin-like growth factor 1 receptor) 또는 GNAS (guanine nucleotide binding protein)의 변이를 확인하는 경우, 대상체가 추정 사구체여과율 및 혈액요소질소의 양적 변화와 연관된 신장질환에 걸릴 위험이 높은 것으로 판정하는 단계를 포함하는, 신장질환의 발병 예측에 필요한 정보를 제공하는 방법을 제공하는 것이다.
본 발명의 또 다른 목적은 대상체로부터 수득한 DNA에서 유전자 WDR72 (WD repeat-containing protein 72), IGF1R (Insulin-like growth factor 1 receptor) 또는 GNAS (guanine nucleotide binding protein)의 변이를 확인하는 경우, 대상체가 추정 사구체여과율 및 혈액요소질소의 양적 변화와 연관된 신장질환을 가진 것으로 판정하는 단계를 포함하는, 신장질환의 진단에 필요한 정보를 제공하는 방법을 제공하는 것이다.
본 발명의 이점 및 특징, 그리고 그것들을 달성하는 방법은 첨부되는 도면과 함께 상세하게 후술되어 있는 실시예들을 참조하면 명확해질 것이다. 그러나, 본 발명은 이하에서 개시되는 실시예들에 제한되는 것이 아니라 서로 다른 다양한 형태로 구현될 수 있으며, 단지 본 실시예들은 본 발명의 개시가 완전하도록 하고, 본 발명이 속하는 기술 분야의 통상의 기술자에게 본 발명의 범주를 완전하게 알려주기 위해 제공되는 것이며, 본 발명은 청구항의 범주에 의해 정의될 뿐이다.
본 명세서에서 사용된 용어는 실시예들을 설명하기 위한 것이며 본 발명을 제한하고자 하는 것은 아니다. 본 명세서에서, 단수형은 문구에서 특별히 언급하지 않는 한 복수형도 포함한다. 명세서에서 사용되는 "포함한다 (comprises)" 및/또는 "포함하는 (comprising)"은 언급된 구성요소 외에 하나 이상의 다른 구성요소의 존재 또는 추가를 배제하지 않는다. 명세서 전체에 걸쳐 동일한 도면 부호는 동일한 구성 요소를 지칭하며, "및/또는"은 언급된 구성요소들의 각각 및 하나 이상의 모든 조합을 포함한다. 비록 "제1", "제2" 등이 다양한 구성요소들을 서술하기 위해서 사용되나, 이들 구성요소들은 이들 용어에 의해 제한되지 않음은 물론이다. 이들 용어들은 단지 하나의 구성요소를 다른 구성요소와 구별하기 위하여 사용하는 것이다. 따라서, 이하에서 언급되는 제1 구성요소는 본 발명의 기술적 사상 내에서 제2 구성요소일 수도 있음은 물론이다.
다른 정의가 없다면, 본 명세서에서 사용되는 모든 용어(기술 및 과학적 용어를 포함)는 본 발명이 속하는 기술분야의 통상의 기술자에게 공통적으로 이해될 수 있는 의미로 사용될 수 있을 것이다. 또한, 일반적으로 사용되는 사전에 정의되어 있는 용어들은 명백하게 특별히 정의되어 있지 않는 한 이상적으로 또는 과도하게 해석되지 않는다.
본 발명은 WDR72 (WD repeat-containing protein 72), IGF1R (Insulin-like growth factor 1 receptor) 및 GNAS (guanine nucleotide binding protein)로 이루어진 군 중에서 선택되는 하나 이상의 유전자에 특이적인 프라이머 또는 프로브를 포함하는 신장질환의 예측 또는 진단용 조성물을 제공한다.
상기 목적을 달성하기 위해, 본 발명자들은 양적 형질 (quantitative trait)에 영향을 미치는 유전 인자의 동정을 위해 모든 유전자형 데이터는 한국보건연구원 (Korea National Institute of Health)의 Korea Biobank Array Project로부터 수득하였다. 상기 양적 형질은 특정 형질에 관여하는 많은 유전자 각각의 작용을 말하며, 이들 유전자의 상호작용과 유전자와 환경의 상호작용에 의한 변이를 나타내는 형질을 의미한다. 샘플 및 임상 데이터는 인구기반 코호트인 한국 유전체 및 역학 연구 (Korean Genome and Epidemiology Study; KoGES)를 통해 수집하였다. 본 발명의 코호트는 KARE (Korean Association Resource), HEXA (Health Examinee), CAVAS (심혈관질환 연관 연구; cardiovascular disease association study)를 포함한 3개의 하위 코호트로 구성되었다. 이들은 각각 지역사회 기반 코호트 (안산, 안성 코호트), 도시 기반 코호트, 농촌 기반 코호트이며, 이들 유전체 데이터를 활용하여 한국인에 적합한 타겟 유전자를 발굴하였다.
한국인 유전체 분석 사업 (KARE)은 2001년에 한국인 유전체 역학조사사업 (KoGES, Korean Genome Epidemiology Study)의 일환으로 수행된 안성 및 안산 지역사회 기반 코호트로 수행되었다. 만 명의 참여자로부터 수득된 50만 단일염기다형성을 Affymetrix Genome-Wide Human SNP Array 5.0 (Affymetrix Inc., 미국)로 유전자형 분석 (지노타이핑; genotyping)을 하였고, 공복 혈장 혈당 (FPG, fasting plasma glucose), LDL 콜레스테롤 (LDLc, low-density lipoprotein cholesterol), HDL 콜레스테롤 (high-density lipoprotein cholesterol LDLc), 트리글리세라이드 (TG, triglycerides), 알부민 (ALB, albumin), 요소질소 (BUN, blood urea nitrogen), γ글루타밀 트랜스펩티드가수 분해효소 (GGT, gamma-glutamyl transpeptidase), 알라닌아미노기전달효소 (ALT, alanine aminotransferase) 및 아스파르트산아미노기전달효소 (AST, aspartate aminotransferase)와 같은 혈장에서 관찰될 수 있는 대사 형질들과의 연관성을 분석하였다.
HEXA (Health Examinee) 공유 대조군 연구의 참여자들은 신장질환 (kidney disease)의 유전체 연관 연구를 위한 대조군으로 사용되었다. 유전체 연관 연구를 위하여, HEXA 코호트는 전국 건강검진 (nationwide health examination) 데이터를 기반으로 하였다. 각각의 검사결과는 건강검진을 받은 각 기관에서 측정하였다.
본 발명에서, 공변량으로는 연령, 성별, 체질량지수 (BMI), 수축기 혈압 (SBP), 고혈압 과거 병력 (HTN), 당뇨병 병력 (DM) 및 알부민뇨를 사용하였다. 고혈압의 과거 병력은 수축기 혈압 (SBP) ≥140 mmHg, 이완기 혈압 (DBP) ≥90 mmHg 및 참가자에 의한 자가 보고로 정의되었다. 당뇨병의 병력은 HbA1c ≥6.5 %, 공복 혈당 ≥126 mg/dL 및 참가자에 의한 자가 보고로 정의되었다. 알부민뇨는 소변 딥스틱 (urine dipstick) 검사에서 ≥ 1+로 정의되었다.
본 발명의 연관 연구는 3개의 코호트 즉, KARE (Korean Association Resource), HEXA (Health Examinee), CAVAS (심혈관질환 연관 연구; cardiovascular disease association study)의 코호트로 구성되어 수행하였다. 본 발명의 구체적인 일 실시예에서, 상기 KARE, HEXA 및 CAVAS 코호트에는 각각 10030, 177357 및 28338명의 참가자가 있고, 개별적으로 수행되었으며, 그 다음 METAL과 게놈 컨트롤 보정 (genomic control correction)을 사용하여 메타 분석 (meta-analysis)을 수행하였다. 전체-게놈 유의 수준 (genome-wide significance level) (P < 5 ×10-8)으로 추정 사구체여과율 (estimated glomerular filtration rate, eGFR) 및 혈액요소질소 (blood urea nitrogen, BUN)의 모두와 연관된 유전자를 선택하였다.
본 발명의 구체적인 일 실시예에서, eGFR 및 BUN과 연관된 유전자로 WDR72 (WD repeat-containing protein 72), IGF1R (Insulin-like growth factor 1 receptor) 및 GNAS (guanine nucleotide binding protein)임을 확인하였다.
상기 WDR72 (WD repeat-containing protein 72)는 WD repeat 도메인을 지닌 것으로, 7개의 WD40 repeat이 포함되어 있으며, 이는 7개의 베타 프로펠러 구조의 블레이드를 형성할 것으로 예측된다. 상기 WDR72 유전자의 돌연변이는 상염색체 열성 저성숙 아멜로제닉 불완전성(hypomaturation amelogenesis imperfecta)을 유발시키는 것으로 알려져 있다.
상기 IGF1R (Insulin-like growth factor 1 receptor)은 다양한 종류의 세포에서 성장을 자극하고 다중 신호경로에서 세포사멸을 막는 타이로신 키나아제 활성을 가진다. IGF1R은 당 기능에 있어서 GLP-1 증가 및 베타 세포 증식을 매개하는데 포함된다.
상기 GNAS (guanine nucleotide binding protein)는 복잡한 각인 발현 패턴을 가진다. 이는 별개의 5' 엑손을 갖는 네개의 대안적 프로모터로부터 유래된 전사체를 발생시킨다. 상기 GNAS 유전자의 돌연변이는 골이영양증, 부갑상선 기능 저하증, 진행성 골수 이형성, 다발성 섬유성 이형성증 및 일부 뇌하수체 종양을 초래할 수 있다.
본 발명의 GFR은 단위 시간당 여과되는 체액의 양을 나타낸다. 본 발명의 eGFR (estimated GFR)은 혈중 크레아티닌 농도, 나이, 성별 및 인종을 변수로 하여 계산될 수 있다.
본 발명의 크레아티닌 (creatinine; Cr)은 근육 대사의 부산물로서, 건강한 신장은 크레아티닌을 혈액으로부터 제거하고 소변으로 몸에서 배출시킨다. 하루 중 실질적으로 일정한 양이 소변에 배출되는 특징을 갖는다. 대상체의 단백뇨 측정을 위하여 24시간 동안의 소변 검사가 가장 정확할 것이나, 현실적인 어려움으로 인하여, 소변에 포함된 특정 단백질과 크레아티닌의 비율을 측정함으로써, 시간에 구애받지 않고, 신장 기능을 확인하는 데에 이용된다. 소변 중 크레아티닌의 농도는, 예를 들어, Jaffe 법, 액체 크로마토그래피, 질량분석, 또는 ELISA를 이용하여 측정할 수 있으나, 이에 제한되는 것은 아니고, 본 발명의 기술 분야에서 널리 알려진 방법에 따라 측정이 가능하다.
본 발명의 혈액요소질소의 양적 변화는 신장질환과 연관된 것으로 알려져 있으며, 수치화된 혈액요소질소의 양과 유전자와의 관계를 선형회귀분석을 통해 통계적으로 유의한지 확인함으로써, 혈액요소질소와 연관된 신장질환의 위험도를 예측할 수 있다.
본 발명의 신장질환은 신장의 손상을 야기하는 질환을 의미하며, 이에 제한되는 것은 아니나, 예를 들어, 만성 신장병, 급성 신손상, ESKD, 전신성 홍반성 루푸스, 당뇨병성 신장 질환 (diabetic nephropathy, DN; 또는 diabetes mellitus nephropathy, DMN), IgA 신장염 (IgAN), HIV-관련 신증, 비 당뇨병성 만성 신장질환, 국소 분절사구체경화증 (focal segmental glomerulosclerosis; FSGS), 미세변화형 신증후군 (MCD), 및 크산틴 옥시다아제 결핍증으로 구성된 군으로부터 선택된 하나 이상의 질환을 포함한다.
본 발명의 예측은 특정 질병 또는 질환이 나타날 가능성과 관련이 있는 것으로, 본 발명의 예측은 신장질환이 발생할 위험이 있는 대상체에 대한 바이오마커 검사를 실시하여 신장질환이 나타날 가능성을 판정하는 것으로, 임상적으로 사용될 수 있다.
본 발명의 진단은 특정 질병 또는 질환에 대한 한 대상체의 감수성을 판정하는 것, 한 대상체가 특정 질병 또는 질환을 현재 가지고 있는지 여부를 판정하는 것, 특정 질병 또는 질환에 걸린 한 대상체의 예후를 판정하는 것을 포함할 수 있다.

본 발명의 신장질환의 예측 또는 진단용 조성물은 신장 질환의 예측 또는 진단에 필요한 바이오마커를 포함할 수 있으며, 상기 바이오마커로는 WDR72 (WD repeat-containing protein 72), IGF1R (Insulin-like growth factor 1 receptor) 및/또는 GNAS (guanine nucleotide binding protein)를 포함할 수 있다.
본 발명의 바이오마커는 유전자 마커, 단백질 마커, 예측 또는 진단용 마커, 예측 또는 진단하기 위한 마커, 또는 진단 마커와 상호 교환적으로 사용 가능하고, 생체 내에 존재하는 생물학적 표지물질로써 질병의 진단, 예후, 치료에 대한 반응 등을 나타내거나 예측할 수 있는 물질을 의미하고, 위험군인 신장질환 환자 유래 세포에서 증가 양상을 보이는 폴리펩티드 또는 핵산 (예를 들어, DNA, mRNA 등), 지질, 당지질, 당단백질, 당 등과 같은 유기 생체 분자 등을 포함할 수 있고, 바람직하게는 유전인자일 수 있으나, 이에 제한되지 않는다.
본 발명의 프라이머는 표적 유전자 서열을 인지하는 단편으로서, 정방향 및 역방향의 프라이머 쌍을 포함하나, 바람직하게는 특이성 및 민감성을 가지는 분석 결과를 제공하는 프라이머 쌍이다. 프라이머의 핵산 서열이 시료 내 존재하는 비-표적 서열과 불일치하는 서열이어서, 상보적인 프라이머 결합 부위를 함유하는 표적 유전자 서열만 증폭하고 비특이적 증폭을 유발하지 않는 프라이머일 때, 높은 특이성이 부여될 수 있다.
본 발명의 프로브는 시료 내의 검출하고자 하는 표적 물질과 특이적으로 결합할 수 있는 물질을 의미하며, 상기 결합을 통하여 특이적으로 시료 내의 표적 물질의 존재를 확인할 수 있는 물질을 의미한다. 프로브의 종류는 당업계에서 통상적으로 사용되는 물질로서 제한은 없으나, 바람직하게는 PNA(peptide nucleic acid, 펩타이드 핵산), LNA(locked nucleic acid, 잠금 핵산), 펩타이드, 폴리펩타이드, 단백질, RNA 또는 DNA 일 수 있다. 보다 구체적으로, 상기 프로브는 바이오 물질로서 생물에서 유래되거나 이와 유사한 것 또는 생체 외에서 제조된 것을 포함하는 것으로, 예를 들어, 효소, 단백질, 항체, 미생물, 동식물 세포 및 기관, 신경세포, DNA, 및 RNA일 수 있으며, DNA는 cDNA, 게놈 DNA, 올리고뉴클레오타이드를 포 함하며, RNA는 게놈 RNA, mRNA, 올리고뉴클레오타이드를 포함하며, 단백질의 예로는 항체, 항원, 효소, 펩타이드 등을 포함할 수 있다.
또한, 본 발명은 WDR72 (WD repeat-containing protein 72), IGF1R (Insulin-like growth factor 1 receptor) 및 GNAS (guanine nucleotide binding protein)로 이루어진 군 중에서 선택되는 하나 이상의 유전자에 특이적인 프라이머 또는 프로브를 포함하는 신장질환의 예측 또는 진단용 키트를 제공한다. 본 발명은 상기 조성물을 포함하는 신장질환의 예측 또는 진단용 키트를 제공한다.
본 발명의 WDR72, IGF1R, GNAS, 프라이머, 프로브, 신장질환, 예측 및 진단은 상기 기재된 바와 같다.
본 발명의 키트는 당업계에 알려져 있는 통상의 제조방법에 의해 제조될 수 있다. 상기 키트는 예를 들면, 동결 건조 형태의 항체와 완충액, 안정화제, 불활성 단백질 등을 포함할 수 있다.
상기 키트는 검출가능한 표지를 더 포함할 수 있다. 상기 검출가능한 표지는 표지가 없는 동일한 종류의 분자들 중에서 표지를 포함하는 분자를 특이적으로 검출하도록 하는 원자 또는 분자를 의미한다. 상기 검출가능한 표지는 상기 단백질 또는 그의 단편에 특이적으로 결합하는 항체, 상호작용 단백질, 리간드, 나노입자, 또는 압타머에 부착된 것일 수 있다. 상기 검출가능한 표지는 방사성 핵종 (radionuclide), 형광원 (fluorophore), 효소 (enzyme)를 포함할 수 있다.
상기 키트는 당업계에 알려진 다양한 면역분석법 또는 면역염색법에 따라 이용될 수 있다. 상기 면역분석법 또는 면역염색법은 방사능면역분석, 방사능면역침전, 면역침전, ELISA, 캡처-ELISA, 억제 또는 경쟁 분석, 샌드위치 분석, 유세포 분석, 면역형광염색 및 면역친화성 정제를 포함할 수 있다. 바람직하게, 상기 키트는 RT-PCR (reverse transcription polymerase chain reaction, 역전사 중합효소 연쇄 반응) 키트, DNA 칩 키트, ELISA (enzyme linked immunosorbent assay, 효소결합 면역흡착 분석) 키트, 단백질 칩 키트, 래피드 (rapid) 키트 또는 MRM (multiple reaction monitoring, 다중 반응 모니터링)인 것일 수 있다.
또한, 본 발명은 대상체로부터 수득한 DNA에서 유전자 WDR72 (WD repeat-containing protein 72), IGF1R (Insulin-like growth factor 1 receptor) 또는 GNAS (guanine nucleotide binding protein)의 변이를 확인하는 경우, 대상체가 추정 사구체여과율 및 혈액요소질소의 양적 변화와 연관된 신장질환에 걸릴 위험이 높은 것으로 판정하는 단계를 포함하는, 신장질환의 발병 예측에 필요한 정보를 제공하는 방법을 제공한다.
또한, 본 발명은 대상체로부터 수득한 DNA에서 유전자 WDR72 (WD repeat-containing protein 72), IGF1R (Insulin-like growth factor 1 receptor) 또는 GNAS (guanine nucleotide binding protein)의 변이를 확인하는 경우, 대상체가 추정 사구체여과율 및 혈액요소질소의 양적 변화와 연관된 신장질환을 가진 것으로 판정하는 단계를 포함하는, 신장질환의 진단에 필요한 정보를 제공하는 방법을 제공한다.
본 발명에 있어서, 상기 방법은 시퀀싱 (sequencing), 엑솜 시퀀싱 (exome sequencing), 마이크로어레이에 의한 혼성화 (microarray hybridization), 대립유전자 특이적인 PCR (allele specific PCR), 다이나믹대립유전자 혼성화 기법 (dynamic allele-specific hybridization), PCR 연장 분석 및 Taqman 기법으로 이루어진 군으로부터 선택되는 하나 이상의 방법으로 수행될 수 있으나, 이에 제한되지 않는다.
본 발명의 대상체는 발병 예측 또는 진단을 하기 위한 피험자를 의미하며, 한국인을 포함할 수 있으나, 이에 제한되지 않는다.
본 발명의 진단은 신장질환에 대한 대상체의 감수성을 판정하는 것, 대상체가 신장질환을 현재 가지고 있는지 여부를 판정하는 것, 신장질환에 걸린 대상체의 예후를 판정하는 것, 또는 테라메트릭스 (therametrics) (예를 들어, 치료 효능에 대한 정보를 제공하기 위해 대상체의 상태를 모니터링하는 것)를 모두 포함하는 개념이다. 본 발명에 있어서, 상기 진단은 조기 진단 또는 발병 예측을 의미할 수 있으나, 이에 제한되지 않는다.
본 발명의 일 실시예에서, GWAS 분석을 통해 신장질환 환자에서 유전체의 연관성을 확인한 결과, WDR72 (WD repeat-containing protein 72), IGF1R (Insulin-like growth factor 1 receptor) 및 GNAS (guanine nucleotide binding protein)의 3가지 마커가 신장질환과 유의한 연관성을 나타내는 것을 확인하였다(도 3 내지 도 5).
본 발명은 신장질환 발생 예측 바이오마커를 제공하며, 보다 구체적으로 사구체여과율과 혈액요소수치의 두가지 임상 양상과 관련된 유전자위를 분석하고 LD block 및 사후 가능성을 이용한 통계 데이터를 통해 유전자 WDR72 (WD repeat-containing protein 72), IGF1R (Insulin-like growth factor 1 receptor) 및 GNAS (guanine nucleotide binding protein)가 신장질환 발생 위험인자 바이오마커로 활용될 수 있고, 이들 유전자 이상을 동반한 대상체의 경우 신장질환과 관련된 지표들인 혈청 크레아티닌 또는 단백뇨가 확인되기 전부터 약물 등의 적극적인 치료를 통해 질병 발생 위험을 조절할 수 있다.

이하, 본 발명의 내용을 하기의 실시예 및 실험예를 통해 더욱 상세히 설명하고자 한다. 다만, 본 발명의 권리범위가 하기 실시예 및 실험예에만 한정되는 것은 아니고, 이와 등가의 기술적 사상의 변형까지를 포함한다.
실시예 1. 참가자 및 유전자형 분석
(Participants and genotyping)
모든 유전자형 데이터는 한국보건연구원 (Korea National Institute of Health)의 Korea Biobank Array Project로부터 수득하였다. 샘플 및 임상 데이터는 한국 유전체 및 역학 연구 (Korean Genome and Epidemiology Study; KoGES)를 통해 수집되었다. 코호트는 KARE (Korean Association Resource), HEXA (Health Examinee), CAVAS (심혈관질환 연관 연구; cardiovascular disease association study)를 포함한 3개의 하위 코호트로 구성되었다. 각 코호트에는 각각 10030, 177357 및 28338명의 참가자가 있었다.
KARE 코호트 중 10004명의 참가자는 Affymetrix Genome-Wide Human SNP array를 사용하여 유전자형을 지정하였다. 이후, 획득한 유전자형 결과는 퀄리티 컨트롤 프로세스 (quality control process)를 통과하였다. 즉, KARE 코호트의 퀄리티 컨트롤은 높은 누락 유전자형 호출률 (high missing genotype call rate) (>5%), 높은 이형접합성 (high heterozygosity) (> 30%), 성 비일관성 (gender inconsistencies), 높은 평균 쌍별 정체성값 (high average pairwise identity-by-state value) (>0.80), 낮은 소수 대립 유전자 빈도 (low minor allele frequency) (MAF, < 0.01), 및 Hardy-Weinberg 평형 (equilibrium) (P < 1 × 10-6)을 포함한 미리 결정된 기준을 기반으로 하였다. 그 결과, 8840명의 참가자가 남았다.
HEXA 및 CAVAS 코호트의 경우 Korea Biobank Array가 사용되었으며, 그 중 58694명과 8105명의 참가자가 이용 가능하였다. HEXA 및 CAVAS 코호트의 유전자형도 퀄리티 컨트롤 프로세스를 준수하였다.
실시예 2. 유전자형 결측치 예측 (Genotype imputation)
퀄리티 컨트롤 후, 모든 코호트는 1000 Genome Project Phase 3 참조 패널의 아시아 인구를 이용하여 예측을 수행하였다. Haplotype phasing 및 imputation은 Beagle 5.4를 사용하여 수행되었다. 예측 프로세스 후 예측 정보 퀄리티 > 0.8 및 MAF < 0.05를 기준으로 추가 퀄리티 컨트롤을 수행하였다.
실시예 3. 표현형 및 공변량 (Phenotype and Covariates)
KARE 및 CAVAS 코호트에서는 베이스라인 (baseline) 연구의 결과가 사용되었다. 그러나, HbA1c (hemoglobin A1c)는 HEXA 코호트의 베이스라인 연구에서 일부 참가자에서만 측정되었으며, 첫번째 추적 (follow-up)에서 거의 모든 개인에게 사용할 수 있었다. 따라서, 첫번째 추적 연구의 결과를 사용하였다.
모든 코호트에서, 일차 표현형은 CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration) 방정식을 사용하여 계산된 추정 사구체 여과율 (estimated glomerular filtration rate; eGFR)을 사용하여 정의된 정량적 형질이었다. KARE 연구에서, 베이스라인 크레아티닌은 Hitachi Analyzer 7600 (Hitachi, Tokyo, Japan)을 사용한 효소적 방법을 이용하여 측정되었다. CAVAS 코호트에서, 수정된 Jaffe 방법은 ADIVA 1650 (Siemens Healthcare, Erlangen, Germany)을 사용하여 크레아티닌을 측정하는데 사용되었다. Cobas c702 (Roche, Basel, Swiss)를 사용하여 혈청 시스타틴 C를 측정하였다.
모든 KARE 및 HEXA 코호트 샘플은 Seoul Clinical Laboratories (SCL, Yongin, Korea)에서 측정되었다. HEXA 코호트는 전국 건강검진 (nationwide health examination) 데이터를 기반으로 하였다. 각각의 검사결과는 건강검진을 받은 각 기관에서 측정하였다. 공변량으로는 연령, 성별, 체질량지수 (BMI), 수축기 혈압 (SBP), 고혈압 과거 병력 (HTN), 당뇨병 병력 (DM) 및 알부민뇨를 사용하였다. 고혈압의 과거 병력은 수축기 혈압 (SBP) ≥140 mmHg, 이완기 혈압 (DBP) ≥90 mmHg 및 참가자에 의한 자가 보고로 정의되었다. 당뇨병의 병력은 HbA1c ≥6.5 %, 공복 혈당 ≥126 mg/dL 및 참가자에 의한 자가 보고로 정의되었다. 알부민뇨는 소변 딥스틱 (urine dipstick) 검사에서 ≥ 1+로 정의되었다. 공변량에 결측값 (missing value)이 있는 개인은 추가 분석에서 제외되었다.
정량적 형질로서의 eGFR 외에, 2차 표현형으로 혈액요소질소 (blood urea nitrogen; BUN) 수준을 사용하였다. CAVAS 코호트에서 혈청 시스타틴 C를 사용할 수 있었기 때문에 혈청 시스타틴 C (eGFRcys)를 사용하여 계산된 GFR이 계산되었다. 각 변수의 히스토그램이 정규 분포된 경우 원래의 값을 표현형으로 사용하였고, 그렇지 않은 경우에는 자연적으로 변형된 값을 사용하였다.
실시예 4. 통계 분석 (Statistical analysis)
GWAS는 plink version 1.9를 사용하여 수행되었다. 양적 특성은 추가 유전 모델의 추정과 함께 미리 지정된 공변량과 함께 plink 명령 “--linear”와 선형 회귀를 사용하여 분석하였다. 연관 연구는 3개의 코호트에서 개별적으로 수행되었으며, 그 다음 METAL과 게놈 컨트롤 보정 (genomic control correction)을 사용하여 메타 분석 (meta-analysis)을 수행하였다. 전체-게놈 유의 수준 (genome-wide significance level) (P < 5 ×10-8)으로 eGFR 및 BUN 모두와 관련된 SNP가 선택되었다. 그 중에서, SNP는 CAVAS 코호트 (P <0.05)에서 eGFRcys 와 연관성을 보인 것을 스크리닝하였다. 마지막으로, eGFR과의 연관성을 조사한 이전 메타 분석 연구에서 선택된 SNP가 유의미하다고 추정되었는지 확인하였다. 본 발명자들은 SNP가 사전 지정된 연구 모두에서 P < 0.05로 확인되었을 때 유의하다고 고려하였다.
메타 분석을 기반으로 한 eGFR 및 BUN에 대한 맨해튼 플롯 (Manhattan plot)이 도시되었다. 관찰된 P 값의 분포와 주어진 SNP의 추정된 P 값은 분위수 (quantile-quantile; QQ) 플롯을 사용하여 도시되었다. QQ 플롯과 맨해튼 플롯은 R 소프트웨어의 qqman R 패키지를 사용하여 도시되었다.
실험예 1. 모든 코호트의 베이스라인 특성화
Korea Biobank Array를 사용한 HEXA 및 CAVAS 코호트에서 58694 및 8105명의 유전자형 참가자 중 49877 및 6087명의 참가자가 남아있었다. KARE 연구에서는, 8788명의 참가자가 추가 분석을 위해 남아있었다. 모든 코호트가 일반 인구를 대상으로 했기 때문에 CKD 환자의 비율은 매우 적었고, 참가자의 3 %만이 알부민뇨가 있었다.
eGFR < 60 ml/min/1.73m2로 정의된 CKD의 비율은 CAVAS 코호트의 연령이 다른 코호트보다 높기 때문에 CAVAS 코호트에서 가장 일반적이었다. 따라서, 연령별 기준의 정의를 사용하면 그 비율이 1% 미만으로 감소하였다. 히스토그램은 각 코호트의 eGFR이 정규 분포를 보이는 것으로 나타났다.
실험예 2. 유전자형 확인
퀄리티 컨트롤 및 유전자형 결측치 예측 프로세스 후, 5,859,419 개의 변이체가 HEXA 및 CAVAS 코호트에 남아있었고, 6,461,358개의 변이체가 KARE 코호트에 남아있었다. 그 중 3229 및 2771개의 유전자좌는 각각 HEXA 및 KARE 코호트에서 eGFR과 게놈 전체에 유의한 연관성 (P < 5 ×10-8)을 보였다. CAVAS 코호트에서 가장 낮은 P 값은 14번 염색체에서 1.27 ×10-8 이었다. 한편, HEXA, KARE 및 CAVAS 코호트에서 각각 1221, 101 및 5 유전자좌가 게놈 전체에 유의미한 수준으로 BUN과 연관되었다.
해당 코호트의 통계 데이터를 사용하여 GWAS 메타 분석을 수행하였다. 도 1 및 도 2는 메타 분석 후 eGFR 및 BUN과의 연관성에 대한 맨해튼 플롯 (Manhattan plot)을 나타낸다.
즉, 신장 기능은 사구체여과율 (glomerular filtration rate, GFR)을 사용하여 측정하는데, 실제 GFR을 구할 수 있는 방법은 용이하지 않아 일반적으로 혈청 크레아티닌을 이용하여 계산된 추정 사구체여과율 (estimated glomerular filtration rate, eGFR)을 이용하였다. 사구체여과율 외에도 혈액요소질소 (blood urea nitrogen, BUN) 또한 혈청 크레아티닌과 같이 신기능 마커로서 사용되었다.
그 결과, 6085개 및 1676개의 유전자좌가 eGFR 및 BUN과 게놈 차원에서 유의미한 수준으로 연관되어 두 가지 양적 특성 모두에서 188개의 유의한 유전자좌가 발견되었다.
상기 사구체여과율과 혈액요소수치의 두 가지 임상 양상 (clinical phenotype)과 모두 관련된 유전자위를 확인하고, LD block 및 사후 가능성을 이용한 통계 분석을 통해 유전자 WDR72 (WD repeat-containing protein 72), IGF1R (Insulin-like growth factor 1 receptor) 및 GNAS (guanine nucleotide binding protein)를 선별하였다.
한국인의 유전체 데이터를 기반으로한 연관성 분석 (Korea biobank array-based genome-wide association analysis)을 통해 밝혀진 상기 유전자들을 타겟으로 하여 한국인의 만성 신장병을 포함한 신장질환의 발생 위험 인자에 대한 바이오마커로서 유용하게 사용될 수 있다.
본 발명은 유전자 WDR72 (WD repeat-containing protein 72), IGF1R (Insulin-like growth factor 1 receptor) 및 GNAS (guanine nucleotide binding protein)가 신장질환의 발생 위험인자 바이오마커로 활용될 수 있음을 확인함으로써, 이들 유전자 이상을 동반한 대상체의 경우 만성 신장병을 포함한 신장질환과 관련된 지표들인 혈청 크레아티닌 또는 단백뇨가 확인되기 전부터 약물 등의 적극적인 치료를 통해 질병 발생 위험을 조절할 수 있다.
이상, 첨부된 도면을 참조로 하여 본 발명의 실시예를 설명하였지만, 본 발명이 속하는 기술분야의 통상의 기술자는 본 발명이 그 기술적 사상이나 필수적인 특징을 변경하지 않고서 다른 구체적인 형태로 실시될 수 있다는 것을 이해할 수 있을 것이다. 그러므로, 이상에서 기술한 실시예들은 모든 면에서 예시적인 것이며, 제한적이 아닌 것으로 이해해야만 한다.
