Description
본 발명은 오베티콜산 및/또는 피브레이트에 대해 내약성이 없거나 불충분한 반응을 보이는 대상체에서의 담관병증의 치료에 관한 것이다.
담관병증
담관세포는 간내담관과 간외담관을 둘러싸고 있는 상피세포로 담즙 생산과 항상성에 참여한다. 건강한 간에서, 담관세포는 중탄산염 및 물의 순 방출을 통해 담즙 분비에 기여한다. 담관세포는 전해질의 능동 수송에 의해 유도되는 담즙산에 독립적인 담즙 흐름을 통해 작용한다. 담관세포는 담관병증이라 불리는 다양한 인간 질환에서 손상된다. 담즙 정체는 간에서 십이지장으로 담즙의 흐름이 느려지거나 막히는 상태로, 담즙이 간실질에 축적된다. 담즙 정체는 2가지 유형으로 편리하게 나눌 수 있다: 담즙 형성이 다양한 질환, 확장된 정맥내 영양, 또는 특정 약물 (일부 항생제 등) 의 부작용과 같은 상태에 의해 방해되는 간 내부에서 일어나는 간내 담즙 정체; 및 담즙의 흐름이 담관 종양, 낭종, 담관 결석, 협착증, 또는 담관의 압력과 같은 담관의 물리적인 일부 또는 완전한 폐쇄에 의해 방해되는 간 외부에서 일어나는 간외 담즙 정체; 원발성 경화성 담낭염 (primary sclerosing cholangitis) (PSC) 은 간내 또는 간외일 수 있다. 담즙 정체의 일반적인 증상은 피로, 가려움증 (소양증), 황달 및 황색종 (피부 아래에 콜레스테롤-풍부 물질의 침착) 을 포함한다. 담즙 정체의 효과는 현저하고 광범위하며, 간 질환 악화와 전신 질환, 간 부전, 및 간 이식의 필요성을 초래한다. 전체적으로, 담관병증은 성인 간 이식의 약 18%, 소아 간 이식의 대부분을 차지한다.
간내 담관병증은 빈도가 감소하는 순서로 원발성 담즙성 담관염 (primary biliary cholangitis) (PBC, 이전에 원발성 담즙성 간경화증으로 알려짐); 원발성 경화성 담관염 (primary sclerosing cholangitis) (PSC) - 이는 또한 상기와 같이 간외일 수 있음; 진행성 가족성 간내 담관병증 (progressive familial intrahepatic cholestasis) (PFIC); 및 알라길 증후군 (Alagille syndrome) (AS) 을 포함한다. 다른 담관병증은 낭성 섬유증 관련 담관병증 및 면역-매개 담관병증 자가면역 담관염 및 간과 관련된 이식편대숙주 질환을 포함한다. 다른 담관병증은 예를 들어, Banales et al., "Cholangiocyte pathobiology", Nature Rev. PBC 은 간의 작은 담관이 서서히 점진적으로 파괴되는 간 자가면역 질환으로, 소엽내관이 질환 초기에 영향을 받는다. 이러한 관이 손상되면 담즙이 간에 축적되고 (담즙 정체) 시간이 지남에 따라 조직이 손상되어 흉터, 섬유증 및 간경화증을 유발할 수 있다. 최근 연구에 따르면 3,000명 - 4,000명 중 최대 1명에게 영향을 미칠 수 있으며, 여성 대 남성 성비는 최소 9:1 이다. PBC 에 대한 치료법이 없고, 간 이식이 필요한 경우가 많지만, 담즙 정체를 줄이고 간 기능을 개선하는 우르소데옥시콜산 (ursodeoxycholic acid) (UDCA, 우르소디올), 담즙산을 흡수하는 콜레스티라민, 피로를 개선하는 모다피닐, 및 지용성 비타민 (담즙 흐름이 감소하면 이들 비타민의 흡수가 어려우므로 비타민 A, D, E, K) 등의 약물치료는 진행을 늦추어 수명과 삶의 질을 정상으로 유지할 수 있다. UDCA 은 미국에서 PBC 치료제로 승인되었지만, 환자의 약 40% 가 UDCA 에 대해 불충분한 반응을 보이는 것으로 보고되고, 약 5% 가 UDCA 치료에 대해 내약성이 없는 것으로 보고되고 있다. 일본 연구자들은 UDCA 에 퍼옥시좀 증식인자 활성화 수용체 (PPAR) 범작용제 및 프레그난 X 수용체 작용제인 베자피브레이트를 추가하면 UDCA 단독 요법에 불응하는 환자를 치료하고 혈청 담즙 효소, 콜레스테롤 및 중성지방을 개선하는 데 도움이 된다고 보고했으며; 한국 연구자들은 UDCA 에 페노피브레이트 또는 베자피브레이트를 추가했다고 보고한 바 있으며; BEZURSO 연구에서는 UDCA 에 베자피브레이트를 추가했다. 매우 강력한 파르네소이드 X 수용체 작용제인 반합성 담즙산 유사체인 오베티콜산 (obeticholic acid) (OCA, 6α-에틸케노데옥시콜산, Intercept's OCALIVA) 은 2016년에 미국에서 UDCA 에 추가하거나 UDCA 에 대해 내약성이 없는 경우 단독 치료제로서 PBC 치료제로 승인받았다. 그러나, 환자의 약 50% 가 OCA 에 대해 불충분한 반응을 보이는 것으로 보고되고, OCA 은 PBC 의 증상 중 하나인 가려움증을 악화시키는 것으로 널리 보고되고 있다. OCALIVA 처방 정보에 따르면, 216명의 환자의 12개월 이중 맹검 무작위 대조 시험에서 OCALIVA 10 mg 아암 내 환자의 23%, OCALIVA 적정 아암 내 환자의 19%, 및 위약 아암 내 환자의 7% 에서 중증 가려움증이 보고되었다. 전향적, 관찰적, 다기관 연구는 OCA 중단율이 12% 내지 17% 이고, 상당한 비율의 환자 (45% 내지 71%) 가 치료로 인한 가려움증 때문에 치료를 중단한 것으로 보고했다.
PSC 은 간내 또는 간외 담관 염증 및 섬유증을 특징으로 하는 만성 담즙 정체성 간 질환으로, 결국 간경화증으로 이어진다. 염증의 근본적인 원인은 자가면역으로 추정되며, PSC 환자의 약 4분의 3 이 염증성 장 질환, 일반적으로 궤양성 대장염을 앓고 있지만 유병률 (일반적으로 약 10,000 명 중 1명으로 보고됨) 과 성비 (일반적으로 주로 남성으로 보고됨) 는 국가마다 차이가 있는 것으로 보고되고 있다. 표준 치료법에는 PSC 환자의 간 효소 수치 상승을 낮추는 것으로 나타났지만 간 생존율이나 전체 생존율을 개선하지는 못한 UDCA 이 포함되며; 항가려움증제, 콜레스티라민, 지용성 비타민, 감염 (세균성 담관염) 치료를 위한 항생제 등이 또한 포함된다. 2009 년에 보고된 한 연구에 따르면, 장기간의 고용량 UDCA 요법은 혈청 간 시험에서 PSC 의 개선과 관련되었지만, 생존율을 개선하지는 못했으며, 심각한 부작용 발생률이 더 높은 것으로 나타났다. 페노피브레이트 단독이 PSC 에서 시험되었으며 (NCT01142323), UDCA 에 대한 반응이 불충분한 환자에서 페노피브레이트 또는 베자피브레이트를 UDCA 에 추가하는 것이 보고되었다. 간 이식은 유일하게 입증된 장기 치료법이다.
PFIC 는 간내 담즙 정체와 관련된 세 가지 유형의 소아 상염색체 열성 장애, 즉 가족성 간내 담즙 정체 1 의 결핍 (PFIC-1), 담즙 염 배출 펌프의 결핍 (PFIC-2), 및 다중약물 내성 단백질 3 의 결핍 (PFIC-3) 을 지칭한다. 이들 질환의 총 발병률은 50,000 - 100,000 명 중 1 명이다. 이 질환의 발병 시기는 보통 2세 이전이며, PFIC-3 가 보통 가장 일찍 나타나지만, 청소년기에도 PFIC 진단을 받는 환자가 있다. 환자는 일반적으로 담즙 정체, 황달, 및 발육 부진을 보이며 심한 가려움증이 특징적으로 나타난다. 지방 흡수 장애 및 지용성 비타민 결핍이 나타날 수 있다. 생화학적 마커로는 PFIC-1 및 PFIC-2 에서는 정상인 γ-글루타밀 트랜스펩티다제 (γ-glutamyl transpeptidase) (GGT), 그러나 PFIC-3 에서는 현저히 상승된 GGT 가 있으며; 혈청 담즙산 수치는 크게 상승되지만; 담즙 정체에서 일반적으로 볼 수 있는 것처럼 혈청 콜레스테롤 수치는 상승하지 않는데, 이는 질환이 담도 세포의 해부학적 문제가 아니라 수송체에 기인하기 때문이다. 이 질환은 일반적으로 간 이식을 받지 못하면 진행되어 소아기에 간부전 및 사망으로 이어질 수 있으며, 간세포암종이 아주 어린 나이에 PFIC-2 에서 발생할 수 있다. 알라길-왓슨 (Alagille-Watson) 증후군, 증후성 담관 부족증 및 동맥간 이형성증으로도 알려진 AS 은 간, 심장, 눈 및 골격 이상 뿐만 아니라 특징적인 얼굴 특색과 관련된 상염색체 우성 질환으로, 발병률이 100,000 명 중 1 명이다. 간 이상은 간 내 담관이 좁아지고 기형이 생기는 것으로, 담즙 흐름이 방해되어 간경화증 (흉터) 을 유발한다. AS 은 주로 20번 염색체에 위치한 Jagged1 유전자의 변화에 의해 유발된다. 사례의 3 내지 5% 에서, 20번 염색체의 한 사본에서 전체 유전자가 결실 (결손) 되고; 나머지에서는, Jagged1 DNA 서열에 변화 또는 돌연변이가 존재한다. 사례의 1% 미만의 극소수에서, 다른 유전자인 Notch2 의 변화로 인해 AS 이 발생한다. 사례의 약 3 분의 1 에서, 돌연변이가 유전되고, 약 3분의 2의 경우 돌연변이가 새로 발생한다. AS 에 대한 치료법은 없지만, 간 질환의 중증도는 일반적으로 3~5 세에 최고조에 달하고 7~8 세에 해결되는 경우가 많다. 일부에서는 간 질환이 말기 간 질환으로 진행되어 간 이식이 필요할 수 있으며, AS 환자의 약 15% 는 간 이식이 필요하다. 답즙 흐름을 개선하고 가려움증을 줄이기 위해 UDCA 와 같은 다양한 약물이 사용되며, 많은 환자에게 고용량의 지용성 비타민이 투여된다.
낭성 섬유증 간 질환 또는 CFLD 로 더 잘 알려진 낭성 섬유증 관련 담관병증은 낭성 섬유증 환자의 약 30% 에 영향을 미치며 낭성 섬유증 환자의 세 번째로 가장 빈번한 사망 원인으로 보고된 낭성 섬유증의 합병증이다. CFLD 은 간염을 동반하는 간 지방증을 거쳐 국소 담즙성 간경화증 및 다엽성 간경화증으로 진행될 수 있다. 일부 성인 CFLD 환자의 경우 증상이 간경화증과 유사하다. 지용성 비타민을 보충한 UDCA 이 일반적인 치료법이다.
Heathcote, "Autoimmune cholangitis", Clinics Liver Dis., vol. 2(2), pages 303-311 (1998) 에 따르면, '자가면역 담관염' 은 만성 간 질환이 원발성 담즙성 간경화증 (PBC) 에 전형적인 담도 패턴을 가지고 있지만 자가면역성 간염에 전형적인 비장기 특이적 항체와 관련된 환자를 설명하는 용어이다. 어떤 사람들은 이 질환을 고전적인 제 1 형 자가면역성 간염의 변종이라고 생각하지만, 다른 사람들은 혈청 미토콘드리아 항체가 없기 때문에 PBC 의 변종으로 간주한다." 치료를 위해 UDCA 및 아자티오프린과 같은 면역억제제 또는 프레드니솔론과 같은 코르티코스테로이드가 제안되었다.
이식편대숙주 질환 (GVHD) 은 동종 조혈모세포 이식 (HCT) 후 흔히 발생하는 합병증으로 일반적으로 피부, 위장관 점막, 및 간 손상으로 나타난다. 만성 GVHD 은 일반적으로 동종 HCT 후 100일 이후에 나타나는 피부, 구강 점막 및 눈물샘의 이상과 관련된 무통성 담즙 정체 증후군이다. 일반적인 치료는 면역억제 요법이다.
알칼리성 포스파타제 (alkaline phosphatase) (ALP) 와 GGT 는 담즙 정체의 주요 마커이다. 두 수치 중 하나만 상승한다고 해서 담즙 정체를 나타내는 것은 아니며 확인을 위해서는 다른 매개 변수가 필요하지만, ALP 와 GGT 가 모두 상승하면 담즙 정체를 나타내며, 두 수치가 모두 감소하면 담즙 정체가 개선되었음을 나타낸다. 따라서 ALP 및 GGT 수치는 간내 담관병증에 존재하는 담도 병리생리학의 존재에 대한 생화학적 마커 역할을 하며, ALP 수치는 PBC 와 같은 간내 담관병증에 대한 임상 연구 (미국에서 OCA 승인을 이끌어낸 연구 포함) 에서 주요 결과 마커로 사용되었다. 다른 관련 마커는 담관 변성의 바이오마커, 예컨대 CK19, miR-506 등 (예를 들어, Baghdasaryan et al., "Inhibition of intestinal bile acid absorption improves cholestatic liver and bile duct injury in a mouse model of sclerosing cholangitis" J. 셀라델파르 (seladelpar)

의약 제형 분야의 통상의 기술자는 과도한 실험 없이 그리고 개인 지식 및 본 출원의 개시 내용에 의존하여 치료적으로 효과적인 제형을 획득하기에 적합한 투여 형태, 부형제, 포장 등을 선택함으로써 셀라델파르의 적합한 의약 조성물을 제조할 수 있을 것이다.

셀라델파르 또는 셀라델파르 염 및 또다른 항담즙정체제가 병용 투여될 때, 셀라델파르 또는 셀라델파르 염의 적합한 양은 셀라델파르 또는 셀라델파르 염이 단독으로 투여될 때와 동일할 것으로 예상되며; 또다른 항담즙정체제의 적합한 양은 배경 기술에서 설명된 바와 같이 임상 시험에서 승인되거나 사용된 양과 유사할 것으로 예상된다. 즉, 병용 요법의 치료 유효량을 달성하기 위한 셀라델파르 또는 셀라델파르 염 및 또다른 항담즙정체제의 적합한 양은 임상 시험에서 사용된 양과 유사할 것이다. 그러나, 둘 중 하나의 치료 유효량은 단일요법으로 사용될 때보다 병용요법에서 더 적을 수 있는데, 이는 이들 각각이 담관병증을 치료하는데 효과적일 것으로 예상되기 때문이다.
담관병증 치료 분야의 통상의 기술자는 단독으로 또는 또다른 항담즙정체제와 병용하여 사용되는 셀라델파르 또는 셀라델파르 염의 치료 유효량 및 병용투여에 사용되는 또다른 항담즙정체제의 치료 유효량을 담관병증의 특정 환자 및 단계에 대해 확인하여, 과도한 실험 없이 그리고 개인 지식 및 본 출원의 개시 내용에 의존하여 치료 유효량을 얻을 수 있을 것이다.
실시예
실시예 1: 원발성 담즙성 담관염
우르소데옥시콜산에 대해 내약성이 없거나 불충분한 반응을 보인 피험자를 대상으로 한 셀라델파르의 공개 라벨 2상 연구 (NCT02955602, EudraCT 2016-002996-91) 또는 무작위 위약 대조 3상 연구 (NCT03602560, EudraCT 2018-001171-20) 에 등록한 원발성 담즙성 담관염 피험자를 모으고; 오베티콜산 및/또는 피브레이트에 대해 내약성이 없거나 불충분한 반응을 보인 피험자를 분석 대상으로 선별했다. 시험 대상자는 다음의 3가지 기준 중 적어도 두 가지에 의해 PBC 진단을 받은 성인, 남성 또는 여성이었다: (a) 적어도 6개월 동안 정상의 상한치 (ULN) 를 초과하는 ALP 의 이력, (b) 면역형광법에서 양성 항-미토콘드리아 항체 역가 > 1/40 또는 효소 결합 면역흡착 분석법에 의한 M2 양성 또는 양성 PBC-특이적 항핵성 항체, 및 (c) 지난 12개월 동안 안정한 권장 용량의 UDCA 을 투여받거나 UDCA 에 대해 내약성이 없는 경우 PBC 와 일치하는 기록된 간 생검 결과, 및 ALP ≥ 1.67 x ULN. 제외 기준은 AST 또는 ALT ≥ 3 x ULN, TBIL ≥ 2 x ULN, 자가면역성 간염 또는 만성 바이러스성 간염 병력, PSC, 현재 피브레이트 또는 심바스타틴 사용, 지난 2개월 동안 콜히친, 메토트렉세이트, 아자티오프린, 또는 전신 스테로이드 사용, PBC 에 대한 실험적 치료법 사용, 및 실험적 또는 미승인 면역억제제 사용을 포함하였다. 피험자는 10 mg/일 또는 5 mg/일의 셀라델파르 (S) 와 동등한 양의 셀라델파르 L-리신 이수화물 염, 또는 위약을 캡슐 형태로 1일 1회 경구 투여받았다. 3개월 동안 치료받은 피험자를 대상으로 효능을 평가했다. 복합 종점은 ALP < 1.67 x ULN, ALP 15% 초과 감소, 및 TBIL ≤ ULN 을 달성한 피험자의 반응률이었다. 추가 종점은 ALP ≤ ULN, 기준선으로부터의 ALP 변화, 및 다른 간 기능 마커였다. 안전성은 1년에 걸쳐 평가되었다.
두 연구에서 등록된 384 명의 피험자 중 총 71명의 피험자가 OCA (47), 피브레이트 (16), 또는 둘 다 (8) 에 대해 내약성이 없거나 불충분한 반응을 보였다. 51명의 피험자를 3개월 동안 치료하고; 37명을 평가하였으며; 결과는 아래 표에 있다:
71명의 모든 피험자에 대해 안전성 분석을 수행했다. 4명의 피험자가 심각한 부작용을 경험했으며 (3명은 셀라델파르 5 mg/일, 1명은 위약 복용중이었다), 이들 모두 셀라델파르와 무관했다; 2명의 피험자가 부작용 때문에, 1명은 ALT/AST 상승으로, 1명은 위식도 역류 질환으로 치료를 중단했으며, 둘 다 셀라델파르 5 mg/일 복용중이었다. 오베티콜산 및/또는 피브레이트에 대해 내약성이 없거나 불충분한 반응을 보인 PBC 환자에서, 셀라델파르는 안전하고, 내약성이 우수한 것으로 나타났으며, 담즙 정체의 생화학적 마커에서 유의미한 개선을 보였다.
실시예 2: 원발성 경화성 담관염
시험 대상자는 다음의 3가지 기준 중 적어도 두 가지에 의해 PSC 진단을 받은 성인, 남성 또는 여성이다: (a) 임의의 이전 실험실 결과로부터의 상승된 AP > ULN 의 과거 증거, (b) PSC 와 일치하는 간 생검, 및 (c) MRCP, ERCP, 또는 경피적 경간 담관조영술에 의해 측정된 PSC 와 일치하는 비정상 담관조영; 지난 6개월 동안 20 ㎎/㎏/일 이하의 안정적인 권장 용량의 UDCA 을 투여받았거나 적어도 12주 동안 UDCA 치료를 중단한 경우; 및 오베티콜산 및/또는 피브레이트에 대한 내약성이 없거나 불충분한 반응을 보인 경우. 다른 기준은 ALP ≥ 1.5 x ULN, TBIL ≤ 2 x ULN, ALT 및 AST 둘 다 ≤ 5 x ULN, eGFR > 60 mL/분/1.73 m2, 혈소판 ≥ 140 × 103/μL, INR ≤ 1.3 (와파린 또는 다른 항응고제 요법의 부재 시), 및 알부민 ≥ 3.5 g/dL 을 포함한다. 제외 기준은 PSC 이외의 병인의 임상적으로 유의미한 급성 또는 만성 간 질환, 중복되는 자가면역성 간염 (AIH) 및 PSC 의 진단, 이차성 또는 IgG4 관련 경화성 담관염, 소관 PSC, 담관조영술 또는 MRI 에 의한 담관암의 존재, 담관 스텐트 삽입, 담관암 또는 다른 간담도 악성 종양의 병력, 증거 또는 높은 의심, 12 주 내의 추정 또는 진단된 급성 담관염, 및 대상성 또는 비대상성 경화증의 증거를 포함한다. 1차 연구 종점은 24주차 기준선 혈청 ALP 의 상대적인 변화이고; 2차 종점은 치료로 인한 유해 사례의 발생률, PSC 관련 증상 또는 절차의 발생률 및 중증도, 및 간 질환 진행 사건의 발생률이다. 피험자를 무작위로 5, 10, 또는 20 mg/일의 셀라델파르와 동등한 양의 위약, 또는 셀라델파르 또는 그의 염을 24주 동안 1일 1회 경구 투여했다. 피험자는 용량과 관련된 ALP 감소를 보였으며, PSC 관련 증상이 개선된 것으로 나타났다.
Claims (20)
- 오베티콜산 및 피브레이트 중 적어도 하나에 대해 내약성이 없거나 불충분한 반응을 보이는 대상체에서 담관병증을 치료하는데 사용하기 위한 셀라델파르 및 그의 염으로부터 선택되는 화합물.
- 제 1 항에 있어서, 화합물이 셀라델파르 L-리신 염인 화합물.
- 제 2 항에 있어서, 화합물이 셀라델파르 L-리신 이수화물 염인 화합물.
- 제 1 항 내지 제 3 항 중 어느 한 항에 있어서, 화합물이 경구 투여되는 화합물.
- 제 1 항 내지 제 4 항 중 어느 한 항에 있어서, 화합물의 양이 0.5 - 50 mg/일의 셀라델파르와 동등한 화합물.
- 제 5 항에 있어서, 화합물의 양이 1 - 25 mg/일의 셀라델파르와 동등한 화합물.
- 제 6 항에 있어서, 화합물의 양이 2 - 10 mg/일의 셀라델파르와 동등한 화합물.
