Abstract

Description

SARS 및 MERS와 같은 발병 감염병에 대한 백신에 대한 과거의 노력은 에피데믹 피크(epidemic peak) 이후에 백신이 발생하였고, 사용된 기술이 후속 발병에서 더 넓은 적용 범위 및 재사용을 허용하지 않았기 때문에 영향이 제한적이었다. 인간 코로나바이러스(HCoVs: Human coronaviruses)는 건강한 사람들에게 "일반적인 감기"를 유발하는, 오랫동안 중요하지 않은 병원체로 간주되어 왔다. 그러나, 21세기에 2개의 고병원성 HCoV인 중증 급성 호흡기 증후군 코로나바이러스(SARS-CoV: severe acute respiratory syndrome coronaviruses), 중동 호흡기 증후군 코로나바이러스(MERS-CoV: Middle East respiratory syndrome coronaviruses)가 동물 저장소(animal reservoir)에서 출현하여 놀라운 이환율과 사망률을 가진 전 세계적인 전염병을 일으켰다.
코로나바이러스는 인간, 다른 포유류, 및 조류 사이에 광범위하게 분포하는 외피 RNA 바이러스로서 이는 호흡기, 장, 간 및 신경계 질환을 유발한다. 6가지 코로나바이러스 종은 인간 질병을 일으키는 것으로 알려져 있다. 4가지 바이러스 - 229E, OC43, NL63 및 HKU1-가 널리 퍼져 있으며 전형적으로 면역 능력이 있는 개인에서 일반적인 감기 증상을 유발한다. 두 가지 다른 균주 - 중증 급성 호흡기 증후군 코로나바이러스(SARS-CoV)와 중동 호흡기 증후군 코로나바이러스(MERS-CoV)는 인수공통 전염병이며 때로는 치명적인 질병과 관련이 있다. 2019년 12월, 원인 불명의 폐렴 환자 집단이 중국 우한의 해산물 도매 시장과 관련되었다. 이전에 알려지지 않은 베타코로나 바이러스가 폐렴 환자의 샘플에서 비편향 서열분석을 사용하여 발견되었다. 인간 기도 상피 세포를 사용하여 COVID-19라는 신규한 코로나바이러스가 단리되었으며, 이는 사베코바이러스 아속인 오르토코로나바이러스 아과 내에 또 다른 분기군을 형성하였다. MERS-CoV 및 SARS-CoV 둘 모두와 상이한, COVID-19는 인간을 감염시키는 코로나바이러스 계열의 7번째 구성원이다.

발병할 때마다 가속화된 백신 개발이 시작되었다. 그러나 개발 기간은 백신 후보가 1상을 통과할 때까지 발병률(따라서 백신 효능을 테스트할 가능성)이 이미 낮은 수준으로 떨어졌다. 후속 발병은 다른 바이러스 아형이므로 이전 노력은 사용이 불가하다.
SARS 발병을 기반으로, 다수의 미국, EU 및 아시아 백신 개발자가 전임상 개발을 통해 후보를 이동하였으며, 일부는 실제로 1상에서 시험되었다. (문헌[Roper & Rehm, 2009]) 2003년에, 사용된 백신 기술은 여러 약독화 생 바이러스, 일부 소단위 백신, 일부 아데노-기반 및 일부 DNA 백신을 포함한다.

추가 실시형태에서, 상기 항원은 호흡기 병원체, 예컨대 코로나바이러스 유래의 항원이다.
또 다른 특정한 실시형태에서, 상기 항원은 M(매트릭스), N(뉴클레오캡시드) S(스파이크) 항원 또는 바이러스-코딩된 비-구조적 단백질(NSP: non-structural protein); 특히 M(매트릭스), N(뉴클레오캡시드) S(스파이크) 항원이다.

실시예 1: 단기 위기(Short term crisis)
위기가 실제로 세계적 대유행(pendemic)으로 탈선하는 (가능성은 희박한) 시나리오에서, 비상 제품의 문턱은 빠르게 내려갈 것이다. (임박한) 독감 대유행과 관련하여, 완전히 새로운 보조 기술이 적용된 몇몇 백신이 해당 환경에서 신속하게 시험될 기회를 얻었다.
이러한 경우, 임의의 다음의 선택사항을 따를 수 있다:

실시예 4: 전임상 제품 개발 접근법
전임상 프로그램은 4개의 단계로 구성된다:
1. 호흡기 발현 및 분포 평가. 생체발광을 통해 생체 내에서 FLUC mRNA의 발현을 모니터링할 수 있는 고유한 가능성을 사용하여 마우스에서의 첫 번째 실험으로 우리의 2 내지 3개의 잠재적인 비강 전달 시스템(네이키드 mRNA, StemFect 및 사내(in-house) LNP)을 평가하고 비강에서의 전달 및 발현과 폐에서의 발현 부재 또는 낮은 발현을 확인한다. 전달 시스템은 이러한 분석법에서의 성능과 일반적인 제조가능성 특성을 기반으로 선택된다.

실시예 5: 임상 개발 접근법

제품의 안전성은 3가지 단계로 평가된다:

실시예 6: 마우스에서 생체 내 비강 내 투여
재료 및 방법
마우스
Charles River에서 총 48마리 마우스(Mus musculus)를 입수하고 연구 시작 전 14일 동안 순응시켰다. 순응 동안, 동물을 체중을 기준으로 군에 할당하고 꼬리 문신으로 식별하였다.
구조체 설계
인플루엔자 NP 단백질(인플루엔자 A/NL/18/94 H3N2)의 전장 코딩 서열을 MHC 복합체에서 처리 및 제시를 최적화하기 위해 신호 서열 및 DC 램프 서열에 인 프레임(in frame)으로 클로닝하였다. 발현을 개선하고 mRNA 구조체에 대한 면역 반응을 감소시키기 위해 N1 메틸 슈도우리딘 변형을 사용하였다.
TriMix mRNA와 함께, 면역원성 구조체를 고정된 1:1 비율로 사용하였다.
투여
0일차, 7일차, 14일차에, 후보 및 대조군 투여를 하기 기술된 바와 같은 군 속성에 따라 비강 내로 수행하였다(군 당 16마리 마우스)
42일차에, NP/TriMix 모드(생체 내 jetPEI)(군 1), NP-모드(생체 내 jetPEI)(군 2) 또는 PBS(군 3)의 모든 동물을 비강 내로 도전시켰다(1 LD50, 10 μl).
말기 시점(48일차)에, 동물에 안락사 전에 정맥 내로 주사된 CD45.2-BV605 항체(Biolegend, Clone 104, 3 μg)를 주었다. 폐를 수집하고 폐-침윤 면역(좌엽)을 바이러스 적정에 사용하였다.
0일차, 7일차 및 14일차에서 면역화:
0일차에, 모든 동물에게 30 μL(콧구멍 당 15 μL)의 후보 제제(군 1 및 2) 또는 PBS(군 3)를 마이크로피펫으로 비강 내로 투여하였다. 30 마이크로리터(30 μL)(콧구멍 당 15 μL)의 후보 제제(군 1 및 2) 또는 PBS(군 3)를 7일차 및 14일차에 마이크로피펫으로 비강 내로 투여하였다. 동물을 마취 하에 투여하였다.
비강 내 면역화(콧구멍 당 15 μL)를 0일차, 7일차 및 14일차에 3.75 μg/3.75 μg의 NP/TriMix 모드(생체 내 jetPEI)(군 1) 또는 7.5 μg의 NP-모드(생체 내 jetPEI)(군 2)로 수행하였다.
바이러스 감염:
NP/TriMix 모드(생체 내 jetPEI)(군 1), NP-모드(생체 내 jetPEI)(군 2) 또는 비히클(군 3)로 처리한 군의 모든 동물을 42일차에 인플루엔자 A PR8로 비강 내로(1 LD50, 10 ul) 도전시켰다.
폐 단리:
말기(48일차)에, 동물을 이산화탄소 질식으로 안락사시키고 기관 수집 전에 육안 부검을 수행하였다.
폐(좌엽)를 무균적으로 수집하고, 칭량하고 4℃에서 Precellys 튜브에서 0.5 mL의 수집 배지((49% DMEM(Gibco, Cat. 11965-084) 및 49% Medium 199(Gibco, Cat. 11150-059), 0.1%의 FBS(Gibco, Cat. 26140-079)로 보충됨)에 두었다. Precellys 튜브에서 폐를 균질화하고, 분취하고 바이러스 적정을 위해 동결하였다.
폐 조직 샘플에서 인플루엔자 바이러스 부하 추정: (TCID50)
바이러스 부하 추정(48일차)을 위해 수집한 폐 샘플을 5000 rpm에서 주기 사이의 5초 중단과 함께 2회의 20-초 주기로 파괴하였다. 0.5 ml의 DMEM/Medium-199, 0.1% FBS를 튜브에 첨가하기 전후에 조직 균질화물을 몇 초 동안 볼텍싱하였다. 조직 균질화물을 3200 × g 및 4℃에서 10분 동안 원심분리하여 조직 단편을 제거하였다. 맑은 상등액을 수집하고 분취하고 바이러스 적정을 위해 동결하였다.
폐 샘플을 Spin-X 튜브(Corning, Cat. 8160)를 사용하여 필터-멸균하였다(14000 x g 및 4℃에서 5분). 여과된 폐 샘플의 10배 희석을 적정 배지((49% DMEM(Gibco, Cat. 11965-084) 및 49% Medium 199(Gibco, Cat. 11150-059), 0.1% FBS(Gibco, Cat. 26140-079)로 보충됨, 1× GlutaMax(Gibco, Cat. 35050-061) 및 0.1% 겐타마이신(Gibco, Cat. 15750-060))에서, 1/2의 출발 희석으로, 멸균 마이크로적정 폴리프로필렌 튜브에서 제조하였다. TCID50을 V-바닥 96-웰 플레이트에서 50 μL의 바이러스 상등액과 0.5% 닭 적혈구를 혼합하여 달성된 혈구응집에 의해 평가하였다. 플레이트를 실온에서 1시간 동안 인큐베이션하고 혈구응집을 판독하였다.
결과
