Abstract

본원에 사용된 하기 용어들은 하기 의미들을 가진다:
"활성제" 및 "약물"은 서로 바꾸어 사용되며, 눈 질환을 치료하기 위해 사용된 임의의 물질을 의미한다.

첨가제들

적용예들

이식 방법

이식방법은 일반적으로, 첫번째로 눈 영역내 표적 영역에 바늘로 접속하는 단계를 포함한다. 표적 영역, 예를 들면, 유리체강내에 일단 접속되면, 휴대용 장치위의 레버를 눌러, 작동자가 플런저를 앞으로 구동시킨다. 플런저가 앞으로 움직임에 따라, 그것이 임플란트를 표적 영역으로 밀어낸다.

실시예

당뇨병성 황반부종("DME") 환자에서 700μg 및 350μg 덱사메타손 후방 세그먼트 약물전달시스템의 안전성 및 효능을 평가하기 위해, III단계의 다중심, 차폐된, 무작위, 위약-제어 실험이 진행되었다. 이 연구는 3년간 진행되었다. 상기 연구들의 결과들은 도 1~7에 도시되어 있다.
본 연구에 사용된 임플란트는 2등급의 PLGA(50:50 PLGA 에스테르 및 50:50 PLGA 산)으로 구성된, 50:50 폴리(D,L-락티드-코-글리코라이드) PLGA의 폴리머 매트릭스 및 덱사메타손을 포함하였다. 보다 상세한 내용은 표 1을 참조한다. 표 2에 개시된 두 PLGA 조합은 생분해성 폴리머 매트릭스를 위해 선택되었다. 선택된 PLGA의 일반적인 특성들은 표 3에 나타나 있다.

일반적인 특성들
본 실험에서, DME를 갖는 환자들은 700μg의 덱사메타손을 갖는 생체분해성 임플란트, 350μg의 덱사메타손을 갖는 생체분해성 임플란트, 또는 0μg의 덱사메타손을 갖는 생체분해성 임플란트(위약)를 이식받았다. 임플란트들은 각 환자의 한 눈의 유리체에 주입되었다. 환자들은 3개월마다 재치료받기 위해 1개월후 6회 방문하여 평가받았다. 재치료(즉, 다른 임플란트 투여)는 6개월마다 이루어졌다. 환자의 중심망막두께가 175μm 이상이거나, 남은 망막 부종의 증거가 있다면, 재치료가 허용되었다. 환자들은 눈마다 3년 연구동안 최대 7개의 임플란트를 허용되었다.
도 1~2에 도시된 바와 같이, 700μg 덱사메타손 임플란트를 받은 환자들 및 350μg 덱사메타손 임플란트를 받은 환자에게서, 위약 임플란트를 받은 환자들에 비해, 최대교정시력(“BCVA”) 점수가 통계적으로 유의미하게 개선되었다. 도 1은 700μg 덱사메타손 임플란트를 받은 환자들의 22.2%가 15개 이상의 문자들의 BCVA 개선을 증명했으며, 350μg 덱사메타손 임플란트를 받은 환자들의 18.4%가 15개 이상의 문자들의 BCVA 개선을 증명했다. 도 2는 700μg 덱사메타손 임플란트를 받은 환자들의 8.5%가 20개 이상의 문자들의 BCVA 개선을 증명했으며, 350μg 덱사메타손 임플란트를 받은 환자들의 11.0%가 20개 이상의 문자들의 BCVA 개선을 증명했다.
도 3은 700μg 및 350μg 덱사메타손-함유 임플란트들을 받은 그룹의 환자들이 위약 임플란트를 받은 환자들보다 3년 연구기간동안 BCVA의 큰 개선을 보여주었음을 설명한다. 도 3에 도시된 바와 같이, 덱사메타손 임플란트를 받은 환자들에게서 BCVA의 급속한 증가가 관찰되었다. 구체적으로는, 350μg 덱사메타손 임플란트를 받은 환자들을 위한 처음 약 3개월동안 기준선으로부터 약 6개의 문자들의 평균 BCVA 증가가 관찰되었으며, 및 700μg 덱사메타손 임플란트를 받은 환자들을 위한 처음 약 3개월동안 기준선으로부터 약 7개의 문자들의 평균 BCVA 증가가 관찰되었다.
도 4에 도시된 바와 같이, 중앙 서브필드 망막 두께(“CSRT”)에서 기준선으로부터 보통의 평균감소는 위약 임플란트를 받은 환자들에서보다 덱사메타손 임플란트를 받은 환자들에게서 더 컸다. 도 4는 700μg 덱사메타손 임플란트를 받은 환자들이 기준선으로부터 111.6μm의 CSRT 보통의 평균감소를 입증했으며, 350μg 덱사메타손 임플란트를 받은 환자들이 기준선으로부터 107.9μm의 CSRT 보통의 평균감소를 입증했음을 설명한다.
도 5에 도시된 바와 같이, 덱사메타손 임플란트는 기준선에서 환자의 렌즈 상태와 상관없이, 유의미한 BCVA 개선을 이끌었다.
본 연구로부터의 부작용 프로필은 하기 도 6에 도시되어 있다. 도면에 도시된 바와 같이, 본 연구에서 경험한 가장 일반적인 부작용들은 백내장 및 안압이었다. 그러나, 증가된 IOP의 부작용이 일어났음에도 불구하고, 놀랍게도, 매우 적은 환자들(연구 그룹당 수술 타입당 약 0.3%)이 IOP의 관리를 위한 연구동안 수술을 받았다. 수술을 받은 환자들의 수 및 받은 수술들의 종류가 도 7에 설명되어 있다.
도면들의 데이터에서 보여지는 바와 같이, 본원에 개시된 임플란트 및 방법들은 당뇨병성 황반부종을 갖는 환자들의 시력이 유의미하게 장기간 개선되었다. 15개 이상의 문자가 증가된 환자들의 비율은 3년간 위약 임플란트와 비교하여, 350μg 및 700μg 덱사메타손 임플란트에 의해 매우 높아졌다. 3년간 평균 4.1회 주사에 의해 치료 이익이 관찰되었다.

Claims (7)
- 당뇨병성 황반부종(DME)을 치료하기 위한 방법으로서,
약 6개월마다 1회 내지 약 9개월마다 1회의 빈도로 인간의 유리체에 생체분해성 임플란트(bioerodible implant)를 주입하는 단계를 포함하고,
상기 생체분해성 임플란트는 생분해성 폴리머 매트릭스 내에 균질하게 분산된 덱사메타손을 포함하는, 연속적, 이중압출성형 막대를 포함하며; 상기 생분해성 폴리머 매트릭스는 친수성 말단기를 갖는 폴리(D,L-락티드-코-글리콜라이드)(PLGA) 및 소수성 말단기를 갖는 폴리(D,L-락티드-코-글리콜라이드)(PLGA)의 혼합물을 포함하고; 상기 생체분해성 임플란트는 인간의 유리체 내에 이식하기 위한 크기로 되어 있으며; 그리고 상기 방법은 당뇨병성 황반부종(DME)를 치료하기에 치료적으로 효과적인, 방법. - 청구항 1에 있어서, 상기 인간은 당뇨병성 황반부종(DME)에 대한 항-VEGF 치료에 난치성인, 방법.
- 청구항 1 또는 2에 있어서, 상기 덱사메타손은 상기 생체분해성 임플란트의 총 중량을 기준으로 하여, 60 중량%의 양으로 상기 생체분해성 임플란트 내에 존재하는, 방법.

- 청구항 1 내지 5 중 어느 한 항에 있어서, 상기 인간은 인공수정체 렌즈(pseudophakic lens)를 갖는, 방법.
